A LicensedPracticalNurse is a trained healthcare worker, who, under the direction and supervision of a Registered
Nurse, gives Nursing care to residents and who assists the Registered Nurse with the care of the acutely and chronically
ill resident. The work performed is assisting and technical in nature and is regulated by the policies and procedures of
the organization. The judgment required is based upon a minimum of scientific knowledge and is limited to those
activities, which are largely repetitive and routine.
GENERAL REQUIREMENTS:

JOB SPECIFIC COMPETENCIES:
· Plans and organizes resident care under guidance of ADON/DON.
· Fills out work assignment from resident activities record at beginning of work tour.
· Provides for elimination: bedpan, enemas, catheterization, and insertion of Foley catheters. Collects specimens: Urine, stool, sputum, cultures.
· Prepares resident for meals, assists with serving and feeding residents, gives gastric feeding. Does not insert gastric tubes unless LPN has had special training for this procedure.
· Measure and records intake and output.
· Takes and records, temperature, pulse, respiration, and blood pressure. Report any deviations from the normal to Registered Nurse.
· Watches intravenous infusions, removes needles as directed by RN and applies Band-Aid. An LPN can only start IV's if she has had special training for this procedure and has been approved to do so.
· Observes and records accurately the results of treatments and nursing care on nursing records.
· Reports observable changes in residents' condition and other pertinent information to the RN.
· Works with the RN in the care of those residents receiving the skill of a professional nurse.
· Carries out that part of the nursing care plan delegated by the RN.
· Gives irrigations.
· Cares for the resident receiving oxygen by mask, catheter, or cannula.
· Does nose and throat suction with bulb syringes or suction machine.
· Does simple dressings; assists with complicated dressings.
· Gives first aid.
· Positions residents under direct orders from the RN.
· Gives medications as assigned by the RN.
· Contacts attending physician regarding changes in resident's condition.
· Complete informative and accurate summaries monthly:
a. Skilled resident - daily
b. ICF - every two weeks
· Charts result on all PRN medications.
· Fill out outpatient forms, infection control forms, Lab forms and X-ray requisitions as needed.
· Provides adjustment in environmental factors such as: lighting, ventilation, cleanliness, and order of bedside unit and equipment.
· Gives informative shift reports to oncoming shift.
· Pass medications per 5 rights of administration.
· Complete all occurrence reports in a timely manner and turn into ADON.
· Consistently put new orders in Matrix in a timely manner.
· Consistently complete documentation for behavioral changes in chart.
· Completes weekly skin assessment assigned accurately and give copy of WCN
· Complete behavioral form Q shift.
· Update nursing and CNA care plan as needed
· Complete documentation to MAR, TAR, falls, med A, new/readmits daily.
· Initiates new event charting in chart fir concerns/new findings with residents
· Reviews message board throughout each shift, address as appropriate, and archive messages.
PROFESSIONAL REQUIREMENTS:

· Completes annual educational requirements.
· Maintains regulatory requirements.

Position Purpose
Supervises the day-to-day nursing activities of the facility in accordance with current federal, state, and local standards governing the facility, and as may be directed by the Nurse Manager, Assistant Director of Nursing and Senior Director of Nursing, to ensure that the highest degree of quality care is maintained at all times. Exercises supervisory authority, even at times when the RN is on duty and must use discretion and independent judgment to ensure proper discipline and productivity.
Required Qualifications
Graduate of an accredited school for LicensedPracticalNursing and currently licensed as an LPN in Georgia.
Three to five years of recent LPN experience in GA supervisory experience required.
Must be able to supervise and instruct CNAs and others , and to exercise independent judgment.
Leadership, training, goal setting and motivation skills preferred.
Ability to respond to emergencies; ability to respond to and seek supervision.
Able to read and write in English, to understand and follow written and oral instructions, and able to communicate with residents, personnel, vendors, and consultants.
Must be at least 18 years of age, have a valid Class C Georgia Driver's License, proof of vehicle insurance, and meet company policy regarding MVR requirements.
Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting.
Must undergo and pass a drug screening and screening for Tuberculosis.
Major Duties and Responsibilities
Directs the day-to-day functions of nursing activities to include supervising CNAs to ensure residents needs are met
Manage CNA duties, documentation completion, initiate and oversee daily shift huddle meeting
Educate and train new staff and introduce new procedures and protocols including orientation
Communicates with management regarding CNA performance, staff issues and changes to treatment and protocols. Providing and documenting teachable moments
Works with Scheduler to ensure and regulatory coverage is met, managing overnight and weekends call outs
Evaluates resident complaints to determine the appropriate plan of care, communicates with physicians and other healthcare providers as necessary to ensure timely treatment and resolution.
Carries out physician's orders regarding diet, medication and treatment
Performs assessments; changes dressings; monitors and ensures quality care through observation and evaluation.
Reports on physiological and mental health status of residents.
Documents nursing interventions on medical records.
Distributes and maintains records of controlled drugs and ensures their security
Makes decisions in routine cases; instructs residents, staff, family members regarding treatments, nursing plans and related patient care issues
Remain calm under pressure, competent in making quick decisions in emergency situations, evaluating the quality of resident care, resolving conflicts, and anticipating problems before they arise
Maintains availability of medications and supplies
Will work with developmental disability or intellectual disability or an acquired brain injury residents
Maintains privacy and confidentiality; treats residents with kindness, dignity, and respect; and ensures that all nursing personnel know and comply with the Resident's Rights rules.
Communicates medical information, change in resident health status, and health related concerns to the resident's family/guardian.
Performs and/or supervises the medication administration processes, administration of prescribed treatments and other therapies. Assumes this responsibility for both units during occasional staffing shortages.
Monitors, evaluates and documents residents prescribed blood sugar levels, vital signs and weights alerting the physician of any findings outside of established normal ranges.
Collaborates with the Dietician and the Culinary Operations Director in planning for those residents requiring special diets.
Collaborate with therapy regarding plan of care
Receives Physician's orders, updates MAR's with new medication orders and change orders and communicates same to other staff assisting with medications.
Performs admission, transfer and discharge processes.
Facilitates the development of the resident's preliminary and comprehensive assessment and written care plan.
Completes nursing documentation to ensure accuracy, timeliness, and completeness.
Administers professional services and provides direct hands-on-care as necessary.
Maintains professional competencies through education and training.
Attends all required and assigned trainings.
Performs other duties as assigned that may be necessary in the best interest of Annandale at Suwannee, Inc. to meet the patient care needs of the Residents.
Serves as a member of or provides input to the Professional Staff Committee, the Process Improvement Committee and the Resident's Council.

Job Summary:
The Nuclear Medicine technologist assists with providing safe, age appropriate care to the patient by performing all exams provided by the department per department procedures. The technologist has the knowledge and ability to properly and safely obtain a quality diagnostic study. He/she is responsible for performing a wide variety of technical procedures requiring independent judgment, ingenuity and initiative in the utilization of the equipment for the diagnosis and/or treatment of diseases. The technologist can independently perform job functions. He/she sets up the equipment to provide imaging service and positions patient for the procedure.
Core Responsibilities and Essential Functions:
Patient Care * Verifies proper clinician order for procedure. * Obtains complete clinical history, gathers examination data and assesses for contraindicating conditions. * Follows established procedures for the administration of radiopharmaceuticals so that no patient condition deteriorates, and no patient suffers undue pain, due to error or poor technique. * Ensures that all studies have proper patient identification and right and left marker are utilized appropriately. * Reviews images for clinically acceptable results and releases patients. * Responds to emergency situations and provides for the restocking of the crash cart and maintenance of life support equipment. * Assists other technologists in all modalities when needed. * Observes patient condition and properly evaluates situations when physician intervention is necessary so that no patient condition deteriorates. * Verifies correct identity of patient using 2 unique identifiers prior to performing a procedure per hospital policy and procedure. * Ensures patient's privacy and comfort is accommodated. Maintains patients dignity throughout the exam/procedure. * Demonstrates competence in the determination and application of appropriate procedures, equipment, supplies and techniques based on the age of patients. * Organizes schedule, time and priorities so that required activities are accomplished within designated time frames. * Follows physicians instructions when assisting with procedures and exams. * Uses critical thinking skills. * Dialogs with patient/family. * Asks clarifying questions. * Seeks clarification on ambiguous orders. * Understands, demonstrates and documents preprocedural verification and Time Out process when appropriate. * Demonstrates and understands pre‐and‐post procedural care. * Preps the patient for the procedure. * Demonstrates and understands sterile technique * Maintains competency with venipuncture. Education and Communications * Patiently and clearly explains purpose and nature of exam to patient prior to starting of procedure, to minimize anxiety and facilitate cooperation using AIDET; Acknowledge, Introduce, Discuss, Explain, and Thank you of all patients and family members. * Ensures that patient's questions, concerns or contraindications are satisfactorily addressed, notifying physician when this cannot be accomplished, prior to the initiation of the procedure. * Participates in the orientation and training of departmental employees. * Required documentation is concise, legible and includes precise terminology. * Reads email each day scheduled to work and follows through with assignment. * Completes mandatory departmental and Hospital‐wide education without prompting. Quality Control * Consistently produces acceptable images, as evidenced by radiologist feedback and quality audits. * Ensures that equipment is fully operational prior to each use, by checking for proper calibration and operating deficiencies. * Sends all studies to PACS immediately after completion, prints CDs as needed. * Studies that have been transmitted after hours to teleradiography are printed and scanned in PACS, if applicable. * Completes all studies in RIS before completing in PACS. * Prints completed log sheet from teleradiography and submits to Radiologist, if applicable. Operations * Restocks linens and supplies, and maintains NM department in clean and orderly condition. * Transports patients, positions patients, and helps them on and off procedure table. * Performs patient charging activities on the day the service is rendered. * Applies supportive and immobilization devices and equipment. * Willingly lends knowledge and assistance to others upon request as needed and routinely as time is available, without prompting, so that all working time is used for departmental advantage. * Performs routine duties of clerical and support personnel in accordance with procedure when warranted to ensure continuity and quality of services. * Keeps supervisor informed on all responsibilities on an ongoing basis, ensuring prompt notification in the event of problems or potential problems. * Maintains an individual productivity rate equal to other co‐workers. * Ability to operate NM equipment within the department. * Maintains Joint Commission standards and any other regulatory agency standards. * Any other duties as assigned. * Takes call when necessary. The RN Clinical Charge Nurse is responsible for directing and overseeing the daily activities of the unit on an assigned shift to assure successful safe delivery of care. The Charge Nurse is a caregiver and often takes modified patient care assignment. They practice in a clinical environment that is administered by Nurse Managers and other leaders and is supported through the WellStar Shared Governance Model. The RN Clinical Charge Nurse is a proactive member of an interdisciplinary team of licensed and unlicensed care givers who ensure that patients, families and significant others receive individualized high quality, safe patient care. Expectations of practice include: accountability; responsibility and authority for the practice of nursing; effective teamwork; delegation; communication; respect; personal development; and development of team members.
