HSHS is seeking a compassionate and resourceful Licensed SocialWorker (LSW) to serve as a Case Manager on our patient care team. This role focuses on supporting patients and families through complex medical and psychosocial situations, ensuring safe and effective discharge planning, and connecting patients to community resources. You will collaborate with interdisciplinary teams to advocate for patient needs and improve outcomes across the continuum of care.
Key Responsibilities:
Conduct psychosocial assessments and develop individualized care plans
Coordinate discharge planning and referrals to community services
Serve as a liaison between patients, families, healthcare providers, and external agencies
Support crisis intervention and facilitate transitions of care
Maintain compliance with documentation, regulatory standards, and ethical practices
Qualifications:
Active LSW license in the state of Illinois (required)
Bachelor's degree in Social Work (BSW) required; Master's preferred (MSW)
Previous experience in hospital or healthcare setting strongly preferred
Strong communication, problem-solving, and advocacy skills
Ability to work collaboratively in a fast-paced clinical environment
Education Qualifications
Master's Degree in social work is required.
Bachelor's degree in social work plus five years of social work or social service clinical experience may be considered in lieu of a master's degree.
Experience Qualifications
Prior experience in social work or past social service clinical experience directly transferrable to a healthcare environment is preferred.
2 years of home care, hospice, discharge planning or related experience is preferred.
Certifications, Licenses and Registrations
Licensed as a Certified SocialWorker (CSW) in the state of Wisconsin or Licensed SocialWorker (LSW) in the state of Illinois is required.
Wisconsin: Licensed as a Certified Advanced Practice SocialWorker (CAPSW), Certified Independent SocialWorker (CISW), or Licensed Clinical SocialWorker (LCSW) is preferred. Illinois: Licensed as a Licensed Clinical SocialWorker (LCSW) is preferred.
Basic Life Support (BLS) is required.
In roles which travel is required, a valid driver's license, automobile insurance and own transportation is required.

As a SocialWorker, you will play an essential role on our interdisciplinary team, supporting patients and families through the end-of-life journey. Your work includes:
Completing initial psychosocial assessments and establishing individualized plans of care.
Providing emotional support, education, crisis counseling, and advocacy.
Communicating regularly with patients, families, caregivers, and facility partners.
Assessing bereavement risk and supporting grief services as needed.

Maintaining accurate, timely documentation and adhering to the Social Work Code of Ethics.
Participating in the 24/7 coverage rotation and maintaining dependable attendance.

On-call responsibilities include either one full weekend every other month or one weekend shift per month, whichever fits your preferred rotation. In this role, the individual assist with discharge planning, facilitating adjustment to nursing home placement with residents and their families, and maintaining appropriate records accordingly.

Responsibilities
* Maintains and actively promotes effective communication with residents/patients/clients and family members / personal representatives.
* Responsible for maintaining a positive image of the entity in the community keeping in alignment with our mission, vision, and values.
* Schedules and facilitates quarterly care plan reviews. Provides interdisciplinary staff with appropriate information. Reviews information on the overall plan of care and records appropriate information on the same. Completes MDS as assigned.
* Assists in completing a social history on a resident within two weeks of his or her admission. The social history shall include identifying information, previous life experiences, significant inter-personal relationships, family relationships, illnesses, significant events, and any other interest which may be used by the inter disciplinary team in coordinating the overall plan of care.
* Maintains resident and family contact, especially when special problems/needs arise.
* Assists residents/responsible party in completing and signing necessary forms, including Advance Directives.
* Facilitates resident and family sharing of concerns or questions.
* Recommends referrals to appropriate agencies in cases of financial or psycho-social problems or community discharge.
* Assists residents and their support system with discharge planning, as appropriate.
* Compiles information about community health and social services available and keeps a current resource file on these services.
* Accepts and investigates all formal and written grievances submitted, as well as informal complaints, and then consults with the SocialWorker Supervisor and/or the Administrator or DON in the timeliest manner appropriate. Maintains a file containing grievance forms and completed, resolved grievance issues.
