Position Summary:

The LPN administers care to patients under the direction of the responsible Registered Nurse or medical provider. To perform this job successfully, an individual must be able to perform each duty satisfactorily.
Essential Duties and Responsibilities:
Serve as a mentor and resource for nursing standards, competencies, policies and procedures.
Collaborates with staff and other departments in the development of strategies to continuously improve quality of care and the patient experience.
Utilize data to continuously meet care coordination needs across settings in conjunction with the patient, family and/or other care providers.
Provide direct and indirect care that is safe, ethical, patient and family-centered, and evidence-based.
Protect patients and employees by adhering to infection control policies and protocols.
Prepare the patient for encounter by following established protocols.
Demonstrate competence in clinical skill set.
Demonstrate proper medication and vaccine management processes.
Reconcile the medication list and prepares prescription renewals.
Demonstrate complete, accurate, and timely documentation in the electronic medical record.
Respond to patient phone calls and engage provider or RN appropriately.
Reinforce patient and family education plans that meet the needs of the patient.
Perform ordered point of care testing (CLIA), including specimen collection, and proper documentation.
Assist with patient encounter flow and trouble-shooting issues in collaboration with supervisor.
Provide guidance and mentors new and incumbent staff based on skill set.
Attend meetings and serves on various organization committees.
Assist supervisor with compliance and staff development duties.
Assist with inventory and receipt of medical supplies, pharmaceuticals, and equipment.
Complete health center daily, weekly or monthly tasks as assigned.
Ensure operation of medical equipment by following manufacturer's instructions, troubleshooting malfunctions, and requesting repairs.
Perform additional duties as assigned.

Unrestricted current LPN license to practicenursing in New York State.
Must possess a certificate from an accredited school of nursing.
Minimum of 1-year clinical experience preferred.

Current BLS certification.

The Program of All-Inclusive Care for the Elderly (PACE) is a community-based program that provides coordinated medical and social services to eligible older adults who want to continue living in their own home despite chronic care needs. The LicensedPracticalNurse, LPN under direct oversight of the Clinical Leader, is an integral member of the PACE at Hudson Headwaters (PHH) team. The LPN helps to provide the necessary care and support to keep Participants as independent as possible. The LPN is responsible for assisting with implementation of Participants treatment plan under the direction of the Registered Nurse, RN, assists the clinical team by providing direct personal and clinical care for the Participants while at the day center, and provide education in the home when needed. The LPN will collaborate with other members of the IDT to ensure Participants' care needs are met and following the current plan of care that was developed to meet the needs of each Participant.
Essential Duties and Responsibilities
Under the direction of the RN, provide clinical services to Participants to include medication administration, monitor Participants' health status, skin integrity, vital signs, appetite, weight, and any other clinical tasks assigned.
Assist with creating, documenting, and tracking authorization for services in EMR(s).
Ensure concise, pertinent, and complete Participant documentation is completed timely and any changes in Participants' condition or functional status is reported to the RN.

Position Summary
The LicensedPracticalNurse provides age appropriate, population specific, patient care in a professional, compassionate, caring, culturally and ethnically sensitive manner, while embracing the Core Values of Quality, Appreciation, Creativity and Sustainability in pursuing the HHHN Mission and Vision. The LPN administers care to patients under the direction of the responsible Registered Nurse or medical provider. To perform this job successfully, an individual must be able to perform each duty satisfactorily.
Essential Duties and Responsibilities:
* Serve as a mentor and resource for nursing standards, competencies, policies and procedures.
* Collaborates with staff and other departments in the development of strategies to continuously improve quality of care and the patient experience.
* Utilize data to continuously meet care coordination needs across settings in conjunction with the patient, family and/or other care providers.
* Provide direct and indirect care that is safe, ethical, patient and family-centered, and evidence-based.
* Protect patients and employees by adhering to infection control policies and protocols.
* Prepare the patient for encounter by following established protocols.
* Demonstrate competence in clinical skill set.
* Demonstrate proper medication and vaccine management processes.
* Reconcile the medication list and prepares prescription renewals.
* Demonstrate complete, accurate, and timely documentation in the electronic medical record.
* Respond to patient phone calls and engage provider or RN appropriately.
* Reinforce patient and family education plans that meet the needs of the patient.
* Perform ordered point of care testing (CLIA), including specimen collection, and proper documentation.
* Assist with patient encounter flow and trouble-shooting issues in collaboration with supervisor.
* Provide guidance and mentors new and incumbent staff based on skill set.
* Attend meetings and serves on various organization committees.
* Assist supervisor with compliance and staff development duties.
* Assist with inventory and receipt of medical supplies, pharmaceuticals, and equipment.
* Complete health center daily, weekly or monthly tasks as assigned.
* Ensure operation of medical equipment by following manufacturer's instructions, troubleshooting malfunctions, and requesting repairs.
* Perform additional duties as assigned.
Qualifications:
* Unrestricted current LPN license to practicenursing in New York State.
* Must possess a certificate from an accredited school of nursing.
* Minimum of 1-year clinical experience preferred.
* Ability to travel throughout the Network.
