Job Description Proposed Schedule: 40 hours per week. Monday-Friday 8am-4:30pm.
The Program of All-Inclusive Care for the Elderly (PACE) is a community-based program that provides coordinated medical and social services to eligible older adults who want to continue living in their own home despite chronic care needs. The Licensed Practical Nurse, LPN under direct oversight of the Clinical Leader, is an integral member of the PACE at Hudson Headwaters (PHH) team. The LPN helps to provide the necessary care and support to keep Participants as independent as possible. The LPN is responsible for assisting with implementation of Participants treatment plan under the direction of the Registered Nurse, RN, assists the clinical team by providing direct personal and clinical care for the Participants while at the day center, and provide education in the home when needed. The LPN will collaborate with other members of the IDT to ensure Participants' care needs are met and following the current plan of care that was developed to meet the needs of each Participant.
Essential Duties and Responsibilities
Under the direction of the RN, provide clinical services to Participants to include medication administration, monitor Participants' health status, skin integrity, vital signs, appetite, weight, and any other clinical tasks assigned.
Assist with creating, documenting, and tracking authorization for services in EMR(s).
Ensure concise, pertinent, and complete Participant documentation is completed timely and any changes in Participants' condition or functional status is reported to the RN.
Follow plan of care and assist Participants with activities of daily living such as bathing, dressing, grooming, hair care, mouth care, eating, and toileting as assigned or needed.
Provide, report and document care in accordance with established policies, guidelines, and applicable regulations.
Participate in the care planning of Participants through collaboration with multidisciplinary teams and as part of the Interdisciplinary Team (IDT).
Adhere to PHH safety requirements and Participant care plans regarding use of assistive devices (e.g., gait belts, Hoyer lifts, slide boards, etc.).
Provide supervision and support for Participants including monitoring and responding to behavioral disturbances such as wandering, agitation, restlessness, exit seeking, etc.
Respond appropriately and in a timely manner to personal alarm devices.
Assist Participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, filling oxygen, and providing other support as assigned or needed.
Follow clinical instructions as assigned.
Assist with meal service and clean-up of dining areas.
Maintain personal care areas including but not limited to, coatroom, bath/shower rooms, personal care/linen supply areas, Participant rooms, and laundry areas.
Assist with stocking and maintaining personal care supplies for use in Day Center.
Work collaboratively with other departments to ensure that the needs of Participants are consistently met and that a clean, sanitary, and safe environment is maintained.
Escort Participants to outside appointments, as necessary.
Assist drivers with loading and unloading Participants from vehicles.
Assist the transportation department with Participants' take-home supplies.
Understand and adhere to PHH policies and procedures and PHH's Participant Rights Policy.
Protect privacy and maintain confidentiality of sensitive Participant information.
Attend staff and Participant care planning meetings as requested, and department in-services as required.
Report any event, accident, or unusual occurrence to the appropriate personnel.
Additional work functions and duties may be assigned.
Qualifications
The requirements listed below are representative of the knowledge, skill, and ability to perform the essential functions:
Graduate from an accredited School of Practical Nursing or equivalent required.
Current licensure in the State of New York
Current BLS certification or willing to complete certification
Requires reliable transportation
Have 1 year of experience working with a frail or elderly population or be willing to receive appropriate training upon hire
Ability to work within a care team with initiative, imagination, resourcefulness, and flexibility
Physical Requirements
While performing the duties of this job, the employee is regularly required to:

Requires the exertion of 50 pounds of force occasionally, up to 20 pounds of force frequently, and up to 10 pounds of force constantly to move objects

Qualifications
Unrestricted current LPN license to practice nursing in New York State.

Hours of Operation: Monday-Thursday 8am-8pm, Friday 8am-5pm, Saturday 8am-1pm.
Position Summary:
The Licensed Practical Nurse provides age appropriate, population specific, patient care in a professional, compassionate, caring, culturally and ethnically sensitive manner, while embracing the Core Values of Quality, Appreciation, Creativity and Sustainability in pursuing the HHHN Mission and Vision. The LPN administers care to patients under the direction of the responsible Registered Nurse or medical provider. To perform this job successfully, an individual must be able to perform each duty satisfactorily.
Essential Duties and Responsibilities:
* Serve as a mentor and resource for nursing standards, competencies, policies and procedures.
* Collaborates with staff and other departments in the development of strategies to continuously improve quality of care and the patient experience.
* Utilize data to continuously meet care coordination needs across settings in conjunction with the patient, family and/or other care providers.
* Provide direct and indirect care that is safe, ethical, patient and family-centered, and evidence-based.
* Protect patients and employees by adhering to infection control policies and protocols.
* Prepare the patient for encounter by following established protocols.
* Demonstrate competence in clinical skill set.
* Demonstrate proper medication and vaccine management processes.
* Reconcile the medication list and prepares prescription renewals.
* Demonstrate complete, accurate, and timely documentation in the electronic medical record.
* Respond to patient phone calls and engage provider or RN appropriately.
* Reinforce patient and family education plans that meet the needs of the patient.
* Perform ordered point of care testing (CLIA), including specimen collection, and proper documentation.
* Assist with patient encounter flow and trouble-shooting issues in collaboration with supervisor.
* Provide guidance and mentors new and incumbent staff based on skill set.
* Attend meetings and serves on various organization committees.
* Assist supervisor with compliance and staff development duties.
* Assist with inventory and receipt of medical supplies, pharmaceuticals, and equipment.
* Complete health center daily, weekly or monthly tasks as assigned.
* Ensure operation of medical equipment by following manufacturer's instructions, troubleshooting malfunctions, and requesting repairs.
* Perform additional duties as assigned.
Qualifications: The requirements listed below are representative of the knowledge, skill and ability to perform the essential functions:
* Unrestricted current LPN license to practice nursing in New York State.
* Must possess a certificate from an accredited school of nursing.
* Minimum of 1-year clinical experience preferred.
* Ability to travel throughout the Network.
* Current BLS certification.
* Proficiency with Microsoft applications and electronic medical records.
* Excellent verbal and written communication skills.
* Self-directed, detail-oriented, motivated and ability to work independently as well as in a team atmosphere.

Demonstrate complete, accurate, and timely documentation in Athena, our electronic medical record system.
Answer patient phone calls and engage medical provider or RN appropriately
Reinforce patient and family education plans that meet the needs of the patient
Perform ordered point of care testing (CLIA), including specimen collection, and proper documentation.
Help keep patient visits running smoothly and work with the supervisor to troubleshoot any issues
Mentor and train new and current staff members based on skill set
Participate in team meetings and serve on committees to support clinical quality initiatives
Help manage and track medical supplies, medications, and equipment deliveries
Complete routine health center tasks as assigned (daily, weekly, or monthly).
Perform other duties as requested.
Qualifications
Unrestricted current LPN license to practice nursing in New York State.
Must possess a certificate from an accredited school of nursing.
Minimum of one-year clinical experience (preferred).
Current Basic Life Support certification required.
Ability to travel to other Health Centers throughout the Network, as needed.
