The Cystic Fibrosis (CF) Clinical Pharmacist position will provide pharmacotherapeutic support to the pediatric and adult patients living with cystic fibrosis in the ambulatory care setting. The CF clinical pharmacist will ensure optimal medication use, support adherence, and actively contribute to individualized care planning while working alongside pulmonologists, nurses, dietitians, respiratory therapists, and social workers as part of an accredited Cystic Fibrosis Care Center. This pharmacist will also participate in multidisciplinary committees and serve as a preceptor in the Atlantic Health System PGY1 Pharmacy Residency Program. Additional responsibilities include: Provide comprehensive pharmaceutical care to patients with cystic fibrosis, including review and recommend optimal, evidence-based medication therapy; provide drug information and pharmacokinetic consultations as required; document any changes in patient and pharmacy records and inform others of medication changes; prevent, detect, monitor, document, and report adverse drug reactions and medications errors.
