Principal Accountabilities: Reviews prescriptions issued by physician, or other authorized prescriber; assures accuracy and determines formulas and ingredients needed.
1. Directs pharmacy workers engaged in mixing, packaging, and labeling pharmaceuticals.
2. Answers questions and provides information to pharmacy customers on drug interactions, side effects, dosage, and storage of pharmaceuticals.
3. Maintains established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs.
4. Enters data, such as patient name, prescribed medication, and cost, to maintain pharmacy files, charge system, and inventory.
5. May assay medications to determine identity, purity, and strength.
6. May instruct interns and other medical personnel on matters pertaining to pharmacy or teach in college of pharmacy.
7. Other relevant duties as assigned
Education/Experience
Graduation from an accredited School of Pharmacy with PharmD required
Current active registration and licensure with the State of New Jersey Board of Pharmacy required
Previous hospital pharmacy experience (2+ years) preferred.

POSITION SUMMARY
The hospital Pharmacist ensures accurate and timely distribution of medications and other pharmaceutical supplies according to established procedures and standards, in support of high-quality patient care. The Pharmacist interprets, evaluates and verifies medication orders accurately assessing for drug-drug, drug-allergy and drug-disease state interactions. The Pharmacist provides drug information to clinicians and patients. It is the responsibility of the Pharmacist to maintain accurate and complete records on all patient profiles and controlled drug substances as required by law. The Pharmacist properly manages pharmaceutical inventories and maintains emergency drug supplies, automated dispensing machines and general medication security on the nursing units. This position directs and verifies the Pharmacy Technician's work. Pharmacists will be responsible for maintaining competency and participating in various clinical programs such as Anticoagulation Management, Pharmacokinetic Monitoring, Adverse Drug Event Monitoring, patient care rounding, pharmacotherapy optimization and general medication safety.

QUALIFICATIONS
EDUCATION: Doctor of Pharmacy or Bachelor of Science in Pharmacy degree
LICENSE/CERTIFICATION: Current New Jersey State license as a Registered Pharmacist required. ACLS Preferred, CPR Preferred for Hospital Pharmacist.
EXPERIENCE: Two years' experience preferred. Two years hospital experience preferred for Hospital Pharmacist. Demonstrates an in-depth understanding of work processes. Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be assessed and evaluated for this position within 90 days from date of hire.
WORK ENVIRONMENT
Potential for exposure to the hazards and risk of the hospital environment, including exposure to infectious disease, hazardous substances, and potential injury. POSITION SUMMARY
The hospital Pharmacist ensures accurate and timely distribution of medications and other pharmaceutical supplies according to established procedures and standards, in support of high-quality patient care. The Pharmacist interprets, evaluates and verifies medication orders accurately assessing for drug-drug, drug-allergy and drug-disease state interactions. The Pharmacist provides drug information to clinicians and patients. It is the responsibility of the Pharmacist to maintain accurate and complete records on all patient profiles and controlled drug substances as required by law. The Pharmacist properly manages pharmaceutical inventories and maintains emergency drug supplies, automated dispensing machines and general medication security on the nursing units. This position directs and verifies the Pharmacy Technician's work. Pharmacists will be responsible for maintaining competency and participating in various clinical programs such as Anticoagulation Management, Pharmacokinetic Monitoring, Adverse Drug Event Monitoring, patient care rounding, pharmacotherapy optimization and general medication safety.
This position supports organizational goals and values by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork, respect and cooperation. The Pharmacist actively pursues opportunities for professional growth and development.
QUALIFICATIONS
EDUCATION: Doctor of Pharmacy or Bachelor of Science in Pharmacy degree
LICENSE/CERTIFICATION: Current New Jersey State license as a Registered Pharmacist required. ACLS Preferred, CPR Preferred for Hospital Pharmacist.
EXPERIENCE: Two years' experience preferred. Two years hospital experience preferred for Hospital Pharmacist. Demonstrates an in-depth understanding of work processes. Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be assessed and evaluated for this position within 90 days from date of hire.
WORK ENVIRONMENT
Potential for exposure to the hazards and risk of the hospital environment, including exposure to infectious disease, hazardous substances, and potential injury. Requires the ability to stand approximately 60% of the time and requires occasional reaching. The department specific Assessment and Evaluation Tool lists the essential functions of this position.
REPORTING RELATIONSHIP
This position reports to department leadership and supervises incumbents as assigned.

The Oncology Clinical Pharmacy Specialist performs a variety of clinical, logistical, and operational functions to streamline care for oncology patients at the MD Anderson Cancer Center at Cooper. The functions include but are not limited to:
Collaboration with interdisciplinary care teams to propose best treatment practices in accordance with the National Comprehensive Cancer Network (NCCN) guidelines and primary literature
Provides clinical pharmacy services for oncology patients, including patient counseling, selection of appropriate therapies, monitoring parameters and frequency of monitoring, management of adverse effects, and identification/ management of drug-drug interactions
Deliver new oncology therapy in-services to the oncology healthcare team
Lead hematology/oncology pharmacology and therapeutics lectures for hematology/oncology fellow physicians
Navigate oncology drug shortages, including recommendation of alternative therapies when appropriate
Precept pharmacy residents and students on ambulatory oncology elective rotation
Build treatment plans for chemotherapy regimens into EPIC and partake in necessary workflows to add new oncolytic agents to hospital formulary
Actively participate in oncology pharmacy & therapeutics (P&T) subcommittee and hospital P&T committee meetings
Assist with medication acquisition for oral and injectable oncolytic therapies, including prior authorizations, appeal letters, and peer-to-peer reviews
Identify and complete patient care quality improvement projects
Assist with policy development with regards to oncology patient care
