In this role, you'll provide high-quality care to a diverse patient population, collaborate with an experienced multidisciplinary team, and build meaningful relationships with patients and their families. You will have an opportunity to deliver exceptional care during life's pivotal moments.
Our comprehensive clinical and operational leadership team will support your transition into our medical practice with a comprehensive 3-month onboarding program along with a dedicated clinical field mentor and support from industry leading clinical experts in post-acute medicine. Growth and leadership opportunities within this market are available as well.
Our innovative population health data reports will guide you towards optimal and timely care for our vulnerable patient population. The medical practice is part of TeamACO, a dedicated long-term Accountable Care Organization participating in the Medicare Shared Savings Program for our clinicians, which could result in you receiving a portion of the shared savings if certain criteria are met.
Key Responsibilities
Perform comprehensive patient assessments, including medical histories and physical exams
Develop, implement, and adjust individualized care plans based on patient needs
Monitor progress and coordinate care to optimize outcomes
Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach
Educate patients and families on health management and preventive strategies
Maintain accurate, timely medical records in compliance with regulations
Qualifications
Current NP or PA license (State of New York) and DEA
Experience in post-acute, acute, emergency medicine, or clinic settings preferred, but not required; new grads welcome to apply
Strong clinical foundation and knowledge of healthcare regulatory standards
Excellent communication and interpersonal skills

Expected compensation is estimated salary range of $130,760 - $154,911 annually with no cap on productivity income potential. This full-time role is benefit eligible.

Essential Duties and Responsibilities:
Consult with MD/Psychiatrist/LCSW on patients via eCW, telephone encounter, and/or with patient visits
Consult with MD/Psychiatrist on medication management, via eCW, telephone encounter, and/or visit with patient as needed to address BH concerns

Consults with other mental health and medical provider staff regarding treatment of patients
Prescribes psychotropic medication as needed and monitors its effectiveness
Provides in-service training to professional and paraprofessional staff
Reviews case records to monitor quality of care
Must demonstrate knowledge and skill necessary to provide care appropriate to the age of the patients served
Must demonstrate knowledge of the principles of growth and development over the life span
Must be able to identify and categorize each patient's age-specific grouping of needs, such as those for infant, adolescent, or geriatric patients
Must be familiar with the process of diagnosing and treating patients with substance use disorder and make appropriate referrals when necessary
Minimum Education Requirement: Master's Degree

Required Skills & Knowledge:

The NursePractitioner provides high-quality, patient-centered primary care services at Joseph P Addabbo Family Health Center (JPAFHC). This role focuses on health promotion, disease prevention, chronic disease management, and patient education.
The NursePractitioner works collaboratively with an integrated care team to deliver culturally competent care to diverse and underserved populations.
Essential Duties & Responsibilities:
Clinical Care
• Provide comprehensive primary care across the lifespan (or specify adults, pediatrics, family).
• Perform health assessments, diagnose medical conditions, develop, and implement treatment plans, and provide follow-up care.
• Manage chronic illnesses such as diabetes, hypertension, asthma, COPD, and behavioral health conditions within NP scope.
• Provide preventive services, including screenings, immunizations, well visits, and patient counseling.
• Order, perform, and interpret diagnostic tests and screenings as appropriate.
• Prescribe medications in accordance with state regulations, including controlled substances where permitted.
• Provide care through in-person visits, telehealth, and outreach settings as needed.
Care Coordination & Team Collaboration
· Collaborate with physicians, nurses, behavioral health providers, dental services, case managers, and community health workers.
· Participate in daily huddles, case reviews, referral coordination, and interdisciplinary care planning.
· Facilitate smooth care transitions and ensure continuity of care for patients with complex needs.
· Respond to urgent care needs within the clinic's scope and protocols.
· All providers will share responsibility for on-call coverage, ensuring patient care needs are met outside regular clinic hours.
Documentation & Compliance
• Document all patient encounters accurately and promptly in the electronic health record (EHR).
• Adhere to HRSA, UDS, OSHA, and state regulatory standards.
• Support FQHC requirements including sliding fee scale procedures, enabling services documentation, and quality reporting.
• Maintain certifications and licensure in accordance with state and federal guidelines.
Quality Improvement & Population Health
• Participate in quality and performance improvement programs such as PCMH, HEDIS/UDS measures, chronic disease registries, and value-based care initiatives.
• Utilize evidence-based guidelines to improve clinical outcomes.
• Engage in peer review, clinical audits, and continuing education to maintain high standards of practice.
Community & Mission Engagement
• Promote the mission and values of JPAFHC, emphasizing health equity and culturally responsive care.
• Participate in health education, outreach events, and collaboration with community partners as needed.
• Advocate for patients and support access to enabling services.
Work Environment & Physical Requirements
• Outpatient clinic environment, with potential for occasional satellite or mobile clinic coverage.
• Ability to sit, stand, and walk for sustained periods; occasionally lift up to 25 lbs.
• May require occasional evening or weekend hours depending on health center's needs.

Qualifications:
Required

• Current DEA registration (if applicable) and BLS/CPR certification.
• Clinical experience or strong interest in caring for underserved or diverse populations.
• Must possess a New York State Medicaid identification.
• Meet New York State's 3,600-hour clinical experience requirement for independent NP practice.
Preferred
• Experience in an FQHC, community health center, or primary care setting.
• Experience with quality improvement, population health, or PCMH models.
• Bilingual proficiency (e.g., Spanish) highly desirable.
• Familiarity with eClinicalWorks EHR systems.
