As a Physical Therapy Assistant (PTA), you will:
Provide in-home patient visits based on the patient's plan of care under the direction of a Physical Therapist (PT)
Instruct and aid patients in active and passive exercises, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training
Observe, record, and report to the supervising provider, the patient's response to treatment and changes in the patient's condition
Educate the patient and family on how they can best participate, assist, and contribute to the patient's rehabilitation
To qualify as a Physical Therapy Assistant (PTA) with us:
Licensure: Current unrestricted license to practice as a Physical Therapist Assistant (PTA) in the state associated with this position
Current CPR/AED/BLS/First Aid certification
Valid state-issued driver's license, automobile insurance, and reliable transportation; willingness to travel
One (1) year of professional experience practicing as a Physical Therapy Assistant (PTA) in a home health or similar setting

The Physical Therapist Assistant treats patients for difficulties with mobility, function, strength and positioning and scope of practice as outlined by APTA and licensure laws in respective state under the supervision of the Licensed Physical Therapist. The PTA may also assist in conducting in-services and training for facility staff on an ongoing basis in restorative and functional maintenance programs. The Physical Therapist Assistant reports to Licensed Physical Therapist and/or Director of Operations and is responsible to Therapist Program Manager. The PTA communicates and collaborates with families, physicians and other health team members and maintains impeccable, professional documentation of services in the medical record. ESSENTIAL FUNCTIONS OF THE JOB
• Treat patients according to the treatment plan of care developed by and under the supervision of the Licensed Physical Therapist.
• Communicate with the Physical Therapist, the patient and/or family and facility staff any pertinent information concerning patient progress, problems and plans.
• Participate in Patient Plan of Care Meetings and Interdisciplinary Team Meetings if asked by the Physical Therapist.
• Participate in in-service training program for other staff in the facility.
• Record treatment notes, progress notes, provide objective input for discharge summaries and complete daily billing of services per policy and procedure.
• Instruct patients' families and/or nursing staff in maintenance programs under the supervision of the Physical Therapist.
• Participate in discharge planning under the supervision of the Physical Therapist.
• Achieve or exceed the minimum productivity standard established by CRI.
• Recommend any needed equipment to the Physical Therapist which would facilitate improved delivery of clinical services.
• Report any problems with department equipment to the appropriate personnel.
• Act as clinical trainer for affiliating Physical Therapist Assistant programs for area Universities and Colleges.
• Comply with the Physical Therapist Assistant Licensure and Certification Standards.
• Periodically work beyond usual working hours as needed for facility emergencies, weekends and holidays.

Communicate with the Physical Therapist, the patient and/or family and facility staff any pertinent information concerning patient progress, problems and plans.

Recommend any needed equipment to the Physical Therapist which would facilitate improved delivery of clinical services.
Report any problems with department equipment to the appropriate personnel.
Act as clinical trainer for affiliating Physical Therapist Assistant programs for area Universities and Colleges.
Comply with the Physical Therapist Assistant Licensure and Certification Standards.
Periodically work beyond usual working hours as needed for facility emergencies, weekends and holidays.

SUMMARY
The Physical Therapist Assistant (PTA) provides comprehensive skilled services under the direction of a Physical Therapist. The Physical Therapist Assistant educates patients and their family members and ensures safety of the patient. The PTA is accountable for completing accurate documentation and remains compliant with all legal rule and regulations. The Company adopts the physical therapy scope of practice as outlined by the state for its licensed PTA personnel.

QUALIFICATIONS & ATTRIBUTES:
Physical Therapist Assistant with a current license in the state of employment.
Minimum of one year of experience as an PTA in an acute setting is required.
Home care experience is desired.
Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers.
Knowledgeable on therapy best practices.
Ability to make appropriate physical therapy judgments.

MAJOR AREAS OF RESPONSIBILITY:
Provides skilled services in a patient's home as developed by the Physical Therapist plan of care
Observes, records, and reports patient reactions, changes, and progress
Performs physical therapy procedures and diagnostic test on patients for whom they have been medically prescribed
Treats patients to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as, exercise, massage, heat, water, etc.
Assesses functional problems and provided intervention and, in conjunction with the physical therapist, works toward goals to improve patient functional status
Communicates effectively with physician, therapist, and coworkers
Participates in case conferencing promoting coordination of care
Recommends prosthetic/orthotic devices for patients or evaluates their device and make minor adjustments or recommend modification
Educates staff in carrying out specific physical therapy modalities when appropriate
Educates patient and family on therapy techniques involved, safety measures, and supportive activities
Instructs patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices
Listens to patient and family members
Fosters independence in patients and their families to prevent dependence on the agency
Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
Documents all aspects of treatment, assessments, and patient education
Completes chart audits at agency location as requested
Communicates with scheduler any changes as needed
Participates in all on-call requirements and case conferences
Attends in-service trainings and mandatory agency meetings
Participates in orientation/training of new employees
Participates in case conferences as needed
ESSENTIAL FUNCTIONS:
Drives to patient's primary location per scheduled visit.
Daily attendance at assigned visit locations.
Documents all aspects of subsequent, discharge, eval/recert/resumption of care visits within 24 hours of visits. Documents all aspects of start of care visits within 48 hours of visits.
Completes and submits all required documentation within specified company requirements.
Follows plan of care as permitted within the scope of practice for a Licensed Physical Therapist Assistant, per the state of employment.
Per visit compensation refers to a lump sum payment for all activities in connection with a visit to a patient. It is compensation not only for the time involved in doing patient care but for preparation time, travel, lab drops, picking up supplies, charting, and communication with the patient's family, physician, and other health care providers.
HEALTH QUALIFICATIONS:
Health Requirements: Must be able to meet all physical position requirements. Evidence required of a current negative tuberculin skin test or x-ray or receive TB testing at the date of orientation. Thereafter an annual tuberculin skin test is required, unless the employee has documentation of a previous TB test. Employee must be free of physical/medical conditions, which would limit or restrict their ability to perform the essential functions. Employees in this position may be at risk for exposure to bloodborne pathogens.

The PT reports to the Therapy Program Manager and ultimately to the Director of Operations. CRI's PTs model and promote competent, courteous and professional services to internal and external customers to develop and maintain a competitive advantage. ESSENTIAL FUNCTIONS OF THE JOB
Evaluate Patients for rehabilitation services within 24 to 48 hours of physician's orders.
Develop effective treatment plans.
Treat patients according to the treatment plan of care.
Supervise the Licensed Physical Therapy Assistant in the delivery of clinical services and interactions with other staff, patients and family members.
Communicate with physicians, clinical managers, and other health team members, patients, family and facility staff regarding patient progress, problems and plans.
Participate in Plan of Care Meetings upon request and Interdisciplinary Team Meetings.

Participate in in-service training programs for other staff in the facility.
Record treatment notes, progress notes, provide objective input for discharge summaries, and complete daily billing of services per policy and procedure.
Instruct patient's families and/or facility staff in maintenance programs.

Recommend any needed equipment to the appropriate personnel which would serve to facilitate improved delivery of clinical service.
Report any problems with department equipment to the facility personnel.
Act as a clinical supervisor for affiliating Physical Therapy and Physical Therapy Assistant programs for area universities and colleges.
Periodically work beyond usual working hours as needed for facility emergencies, weekends and holidays.

Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
Documents all aspects of treatment, assessments, and patient education

Per visit compensation refers to a lump sum payment for all activities in connection with a visit to a patient. It is compensation not only for the time involved in doing patient care but for preparation time, travel, lab drops, picking up supplies, charting, and communication with the patient's family, physician, and other health care providers.
HEALTH QUALIFICATIONS:
Health Requirements: Must be able to meet all physical position requirements. Evidence required of a current negative tuberculin skin test or x-ray or receive TB testing at the date of orientation. Thereafter an annual tuberculin skin test is required, unless the employee has documentation of a previous TB test. Employee must be free of physical/medical conditions, which would limit or restrict their ability to perform the essential functions. Employees in this position may be at risk for exposure to bloodborne pathogens.
