As the PhysicalTherapist in Home Health, you will be completing assessments and evaluations of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, you will work to help determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate the patient's progress.
Primary Responsibilities:
Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care
Provides services within the scope of practice as defined by the state laws governing the practice of physical therapy, in accordance with the plan of care, and in coordination with other members of the health care team
Observes and reports the patient's response to treatment and any changes in his/her condition to the supervising nurse and/or the physician
Treats patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice
Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals
Visits patients according to Plan of Treatment, completes a progress note for each visit, and submits progress notes per LHC policy
Completes all patient evaluations and develops the PT plan of care within state specific guidelines
Reports outcomes of evaluation, goals, and anticipated projected frequency of care

Required Qualifications:
Current Physical Therapy licensure in state of practice
Current CPR certification

Pay Range
$$77,400 - $171,000 annual total cash target pay
$$44.65 - $98.65 per visit point
$$37.21 - $82.21 hourly rate
Annual total cash compensation for this role assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of “direct” and “indirect” patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. You will be paid your hourly rate for certain non-visit activities such as orientation. We comply with all minimum wage laws as applicable. Job Description
We have multiple openings for undergraduate students to engage in practical research experience to further their educational goals. Opportunities include participation in projects lead by LLNL scientists in the research areas including: Applied Physics (Optical and X-Ray Imaging, Astrophysics and Planetary Science (laboratory-based and observational), Space Science Vehicles & Sensors); Condensed Matter Physics (Theory, Modeling & Simulation, Experimental Static & Dynamic High Pressure Physics, Quantum Computing & Sensing); High Energy Density Physics (Experimental Laser and Magnetically-driven compression, Theory, Modeling and Simulation of HED states); and Plasma and Fusion Energy Science (Fusion Energy Modeling & Simulation, Tokamak development experiments & diagnostics, Applications of Plasma Science). Summer internships are typically for 3 months and start in the months of May to July.

"Coverage is roughly 8-5 M-F but the start is usually 830 with a 30 min lunch that varies between 11-130 depending on what is happening with the patients.
We have weekend coverage for physical therapy only, but the timing is not specific as it is based on the number of patients in the hospital that need to be seen. It is typically done at some point between 8-5 but occasionally runs late if the therapist has something else going on that day.
For a traveler - 25 bed Rural critical access hospital. 4 beds are ICU, 4 others assigned to women's health (birthing suites). Activity/Rehabilitation room is in 319.
Scheduling is consistent but dynamic based on patient load. There is additional scheduled coverage from our outpatient clinic daily (2-4 hours), and need is determined daily during huddle in the morning and communicated with our Outpatient Lead Therapist. Weekend schedule is done by me personally, however; the time for the coverage is not specifically set.
Charting is done currently through powerchart.
Requirements
Day Shift; M - F ; 8am to 5pm; outpatient PT position, minimal acute inpatient coverage from time to time.
EDUCATION AND EXPERIENCE: Bachelors Degree from four-year accredited college or university in physical therapy, or degree in a related field
EDUCATION AND EXPERIENCE: One to two years related experience and/or training, or equivalent combination of education and experience
LICENSES OR CERTIFICATIONS: A valid California drivers license when travel is required
LICENSES OR CERTIFICATIONS: Current BLS course completion
LICENSES OR CERTIFICATIONS: Current California Physical Therapy license, or eligible for registration
LICENSES OR CERTIFICATIONS: Member of APTA preferred
QUALIFICATIONS: Able to organize and monitor the work of others.
QUALIFICATIONS: Able to relate effectively with patients, nurses, physicians and other hospital/home health staff in a courteous, professional manner

Need Specifics: March 30th START DATE! Predominately working at Winslow but with option to help us cover in Dilkon as needed. Mostly outpatient clinic. Open to 8 or 10hr shifts. prefer 3 years' experience. Mostly Ortho cases but must be able to take care of anything that walks through the door. Need AZ or compact license. Candidates must have experience in outpatient Ortho and Neuro.
