SUMMARY OF JOB RESPONSIBILITIES: The Youth Counselor provides counseling, education, and support to bereaved clients you are coping with serious illness and/or death of a loved one through phone calls, counseling sessions, and group support. This position provides school-based grief groups, individual and/or family counseling and serves as a resource on psychosocial issues related to child grief and loss internally and externally which includes educational presentations. The youth counselor will also assist with planning and facilitating the child and teen grief summer camps.

QUALIFICATIONS:
Master's Degree in a counseling field and/or social work is required.
Must hold and maintain an active, independent, unrestricted license, such Licensed Professional Counselor (LPC), Licensed Social Worker (LCSW), or Licensed Marriage/Family Therapist (LMFT) with the State of Colorado Department of Regulatory Agencies (DORA) prior to first day of work if not already obtained at time of job offer.
At least one (1) year of experience in counseling and grief work.
At least one (1) year of experience working with children and teens in a related setting.
Valid Colorado driver's license and unlimited access to reliable transportation.

Requirements
EDUCATION AND EXPERIENCE

Summary/Objective

Ministry Program
The Behavioral Health Specialist organizes and implements behavioral health services to Senior CommUnity Care participants and families. Provides behavioral health assessments, individual and group counseling in formats appropriate to clinical populations served. Conducts assessments of clients' mental health and cognitive functioning and develops appropriate treatment plan. Collaborates as part of an interdisciplinary team and promotes professional working relationships with both internal and external customers. Represents Senior CommUnity Care PACE in community settings via formal presentations and community committees as appropriate.
QUALIFICATIONS:
Education: Master's Degree from an accredited school of social work or counseling required. Be legally authorized, currently licensed, registered or certified if applicable in the state of employment. Licensed Professional Counselor or Licensed Clinical Social Worker required.
Experience: A minimum of one year's experience working with frail or elderly population required. Experience working on a multi-disciplinary team in a hospital, nursing home or community-based setting is preferable. Experience and training in bio-psychosocial assessments, dialectical behavioral therapy, cognitive behavioral therapy, solution-focused brief therapy, grief counseling, treatment of substance uses disorders and non-violent crisis intervention a plus. Experience with severe and persistent mental illnesses and personality disorders required.
Skills and Knowledge:
Experience with frail/chronically ill elderly people.
Ability to provide psychosocial assessment and individual, family and group counseling.
Thorough knowledge and understanding of the DSM5/ICD-10 required.
Effective verbal/written communication skills with the ability to maintain accurate records and to prepare clear and concise reports, correspondence and other written materials.
Ability to use and navigate on a computer, using MS Word and other applications. Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
Training and/or mentoring experience and ability to complete performance objectives, measures and evaluations.
Good public speaking skills with all size groups.

ESSENTIAL FUNCTIONS:
Upon referral from Interdisciplinary Team (IDT) or Social Worker, performs in person initial assessments of Senior CommUnity Care participants to obtain a complete psychosocial history, which may include descriptions of cognitive status, social supports, family dynamics mental health and substance dependency and other issues and needs. Coordinates with the IDT to develop a comprehensive plan of care for each participant receiving behavioral health services. Reassesses participants as needed to assess progress in therapy. Keeps IDT apprised of changes and progress in treatment.
Functions as a consultant to the IDT. Attends IDT when necessary to present updates related to significant changes with participant status, education related to care planning for participants receiving behavioral health services, or in absence of the social worker.
Ensures that Social Workers are kept informed of behavioral health services offered to participants and coordinates care in conjunction with the Social Work Team.
Provides ongoing support, counsel, and education to participants and family regarding a variety of issues, including but not limited to: mental health disorders, coping strategies, grief and loss, the aging process, and treatment for behavioral health.
Presents recommendations to IDT for recommended services to support participants with mental health needs and psychiatry referrals. Supports Social Workers with coordination of care with psychiatry and external counseling services.
Supports participants with access to community support regarding mental health services, self-help and other resources related to behavioral health.
Follows all mandated reported guidelines regarding reporting of suspected abuse or neglect of an adult and coordinates with social worker as needed.
Acts as a resource to other team members, including nursing, transportation, day center and home care staff, and therapy staff regarding topics such as severe and persistent mental health diagnoses, dementia, difficult behaviors, and difficult personalities.
Completes and ensures completion of documentation of clinical service, in participant's medical records including initial assessments, re-assessments, counseling sessions, home visits and other significant events according to Senior CommUnity Care documentation requirements.
Assists participants and caregivers in filing grievances.
Advises on day center or other programming offered via Senior CommUnity Care to meet the behavioral health needs of participants and caregivers.
Acts within scope of his or her authority to practice.
Follow all Senior CommUnity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants, and families.

Communicate any problems, concerns, or issues to the resident's family members as needed.

Observe, monitor, and record resident's physical and emotional well-being, and promptly report any changes to nurse, lead, supervisor, director or administrator.

The Social Work Supervisor leads and supervises the social work team by:
Ensuring the delivery of high-quality psychosocial services that support older adults in remaining safely in their home and community.
Maintain oversight of social documentation, ensuring compliance with policies and regulations.
Ensure Social Workers engage meaningfully in IDT care planning and evaluations.
Carry a caseload of participants by conducting home visits, completing assessments, and providing clinical care as needed.
Provide crisis management and guidance for complex cases .

You Bring:
Master's degree in social work from an accredited social work program. LCSW required
Employees must maintain registration or licensure thought their employment with HopeWest.
3+ years experience as a accredited social worker (LCSW)
1+ years' experience in a leadership or supervisory role.
Valid Colorado driver's license, access to reliable transportation, and adequate automobile insurance maintained throughout employment.
Able to work in a variety of environments.
Knowledge base of community resources.
Current CPR certification is maintained throughout employment.
Excellent interpersonal and communication skills.

The incumbent in this position functions as a Case Manager for VA Western Colorado Health Care System (VAWCHCS) HUD-VASH Program. The HUD-VASH Program falls within the Care Management and Social Work Service Line. HUD-VASH is an intensive case management program for chronically homeless veterans aimed at providing psychosocial services to veterans while preparing for and residing in stable housing through HUD's Section 8 voucher system.
* Establishes and maintains effective working relationships and communicates with Veterans, staff, and other parties as appropriate.
* Serves as a liaison between Veterans and/or their families and VA and community resources to ensure thorough delivery of services.
* Communicates effectively, both orally and in writing with people from varied backgrounds.
* Utilizes knowledge of community resources, makes referrals to the community and other government agencies as appropriate, and coordinates services.
* Facilitates action for community placements through collaboration with Veterans and their families as well as interdisciplinary treatment team members.
* Provides consultation and education to Veterans and their families regarding community resources, VA benefits, and specialty programs.
* Conducts psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds.

SUMMARY OF JOB RESPONSIBILITIES: The Social Work Supervisor provides leadership, coordination, and oversight for hospice and palliative care social work services. This role ensures the delivery of high-quality, evidence-based, trauma-informed psychosocial care across HopeWest.
Key responsibility of the supervisor is to serves as a clinical resource and promotes excellence in delivery of patient care, documentation, development of standards of care, and interdisciplinary collaboration. Additionally, the Social Work Supervisor assists in the oversight of time-off requests and scheduling for Social Workers, coverage coordination, supervision of variable staff, compliance monitoring, and facilitation of discipline-specific meetings.
QUALIFICATIONS
Must be a Licensed Clinical Social Worker (LCSW) with active registration through Colorado DORA.
2 years of LCSW experience preferred.
Advanced Certification in Hospice and Palliative Care preferred.
Must be able to provide clinical supervision for LCSW candidates.
Advanced training in trauma-informed care, EMDR, DBT, and crisis intervention preferred.
Valid Colorado driver's license, adequate automobile insurance, and unlimited access to reliable transportation maintained throughout employment.
COMPETENCIES & REQUIREMENTS:
Computer proficiency in Microsoft products (Word, Excel, and Outlook) and EMR.
Ability to cope with patient/family emotional stress within the framework of the hospice philosophy.
Participates in the on-call, holiday, weekend, and after-hour's rotation.
Strong ability to develop policies, lead staff training, and provide clinical supervision.
Excellent interpersonal, problem-solving, and leadership skills.
Ability to work collaboratively within an interdisciplinary team and adapt to evolving patient needs.
Ability to interact appropriately with geriatric populations and clients with cognitive behavior impairment and other various barriers to communication.
Demonstrates a driven curiosity for knowledge of available community resources for patients/families.
Current CPR certification is maintained throughout employment.
