Responsibilities
* Serves as clinical liaison to the newborn nursery to the hospitalist group.
* Collaborates with hospital care team to ensure that all current labs, test results, consults, orders, etc. are carried out in a timely fashion.
* Educates parents on the role of the hospitalist.
* Initiates proactive planning and coordination of the patient's plan for the day and plan for the stay with the rest of the team.
* Performs nursing assessments, identifies resource needs, and initiates appropriate referrals to multi-disciplinary services.
* Reviews medical history of the mother and updates the hospitalist and care team as needed.
* Prioritization of patient rounds related to census and level of service.
* Monitors and coordinates interdisciplinary plan of care and discharge plan across the health care continuum with internal and external resources to meet standards of care for transition home or to appropriate level of care; to include clinical needs, barriers to quality care, and effective utilization of resources.
* Verifies primary care provider and determines any barriers to discharge & set expectations for hospital stay.
* Assures coordination with post discharge care services including follow-up appointments and needed referrals.
* Assess with patients & their family members that their needs are being met& collaborates to set up follow-up needs upon discharge
* Verifies and/or facilitates completion of appropriate newborn screenings and testing per protocol, including but not limited to head circumference measurement, POC blood glucose testing, transcutaneous bilirubin screening, etc.
* Reviews infant intake and consults lactation specialists when indicated.
* Implements care protocols as needed related to nursing assessments performed.
* Collaborate with Social Services regarding any drug testing and/or sensitive social situations to be addressed prior to discharge.
* Performs follow up phone calls with patients post discharge
* When needed organizes meetings with parents and care team to assure an appropriate plan of care.
* Participates actively in quality improvement initiatives
* Required to occasionally set-up care team meetings with both physicians and families about ongoing hospital care, safe discharge plan & disposition
* Serve as continuity for patients during physician hand-offs
Qualifications
Education:
* Graduate of an accredited school of nursing or IA Board of Nursing approved nursing program.
