Hospice RN Case Manager
to join our growing team! You will be responsible for the hospice case management of your assigned patients on an intermittent basis in their place of residence. You will also coordinate with the interdisciplinary team to ensure an optimal care experience for the patient!
Areas of Coverage: Winchester and Surrounding Areas
Schedule: M-F 8:00 AM - 5:00 PM

Brief Overview of Responsibilities for Hospice Registered Nurse:
Complete an initial assessment of the patient and family to determine hospice needs
Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
Provides coordination amongst the clinical team supporting the patient, and supervising LPNs and Hospice aids who may be involved in supporting patients.
Spend quality time with the patient to assist with pain management and support
Modify and re-evaluate the plan of care when necessary
Monitor, record, and report symptoms and changes in the patient's condition
Obtain and receive physician orders as required
Participates in on-call duties as defined by the on-call policy
Other duties as assigned
Qualification of Registered Nurses:
Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia
At least one year of hospice experience is required.
Demonstrated flexibility and willingness to provide care for Home Health patients as needed to support overall team operations.
Graduate of an accredited nursing school.
Must have a valid driver's license and a reliable car available for use during work hours.

JOB SUMMARY
The Hospice Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of First Call Hospice, either directly or through coordination of care with other spiritual counselors.
DUTIES & RESPONSIBILITIES
Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
Provides direct spiritual care to patients and families/caregivers.
Serves as liaison and support to community chaplains and spiritual counselors.
Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
Documents direct services and ongoing communication with community chaplains and spiritual counselors.
Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group.
Provides consultation, education, and support to the interdisciplinary group on spiritual care.
Recruits community chaplains, spiritual counselors, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care.
Actively participates in quality assessment performance improvement teams and activities.
Provides for funeral or memorial services for patients as requested.
Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
Assists in supervision of spiritual care volunteers when assigned to patients/ families/caregivers.
JOB REQUIREMENTS
Graduate of accredited seminary or school of theology Preferred (BD, MDiv or equivalent theological degree)
Experience working with death and dying, individuals/family/caregiver.
Hospice/ experience preferred.
Ability to work as member of interdisciplinary team/group.
Comfort in an interfaith setting.
Ability to accept different lifestyles, cultures, beliefs, and values.
Ability to network with community clergy and congregations.
Knowledge of and commitment to hospice philosophy of care.
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
Pay Range: $29/hour - $34/hour

The Wound, Ostomy, and Continence Nurse (WOCN) Consultant provides expert consultation and recommendations for the management of wounds, ostomies, and incontinence in hospice patients. This role is advisory in nature and does not involve direct patient care. The consultant collaborates with interdisciplinary teams to optimize patient comfort and quality of life.
DUTIES & RESPONSIBILITIES
Consultation & Assessment
Conduct remote or in-person consultations with hospice teams regarding complex wound, ostomy, and continence cases.
Review patient records and provide recommendations on appropriate wound care interventions, ostomy management, and incontinence strategies.
Guide hospice nurses and caregivers in assessing wound progression and making necessary care adjustments.
Assist in selecting appropriate wound care products and continence management supplies to enhance patient comfort.
Provide pre- and post-operative ostomy guidance to hospice teams to support patient care.
Education & Training
Educate hospice nurses and caregivers on best practices for wound, ostomy, and incontinence management.
Develop and deliver training materials, in-services, and clinical guidelines for wound and continence care in hospice settings.
Offer guidance on preventative measures to minimize complications and improve patient outcomes.
Communication & Documentation
Prepare and update clinical notes detailing consultation findings and recommendations.
Communicate effectively with hospice nurses, physicians, and caregivers regarding patient needs and care modifications.
Collaborate with interdisciplinary teams to ensure seamless integration of wound, ostomy, and continence care into overall hospice care plans.
Quality Improvement & Compliance

JOB REQUIREMENTS
Must be a Registered Nurse with an active license in the state of practice.
Wound, Ostomy, and Continence Nursing Certification (WOCNCB) preferred.
Minimum of two years of clinical experience in wound and ostomy care, with hospice or palliative care experience preferred.
Strong ability to assess and recommend care strategies without direct hands-on patient care.
Excellent verbal and written communication skills for effective team collaboration.
Self-directed, organized, and capable of managing multiple consultations effectively.
WORKING ENVIRONMENT
Remote and/or on-site consultation as needed.
Minimal physical demands as this role does not involve direct patient care.
Requires occasional travel for training or team meetings.
