Job Purpose
The Day Program Facilitator provides directsupport to clients who participate in the Day Program including developing and facilitating groups, as well as, individual support. The facilitator documents all information required to determine the client's progress towards the achievement of group and individual participant goals.
Essential Functions
Create and facilitate groups that help clients achieve goals and outcomes identified in their Individual Service Plan
Understand and follow client's Individual Service Plan to ensure excellent service provision; provide assistance to program participants as needed
Transport clients as assigned (able to drive a 15-passenger van)
Foster the growth of individuals by teaching and offering opportunities for choice, initiation, and risk-taking
Make preparation and serve lunch for attendees
Provide crisis intervention, conflict resolution, and mediation as needed.
Participate in all staff meetings, trainings and client meetings as assigned.
Must check email and voicemail daily
Utilize Electronic Health Record System to create accurate and timely notes on all services delivered and relevant client information that meets or exceeds agency and state standards
Complete other departmental documentation
Correct all documentation as directed by supervisor
Attend all electronic health record (Foothold) training as required

The DirectSupportProfessional role is a Non-Exempt position requiring ability in implementing training programs and maintaining a safe environment for adults with intellectual /developmental disabilities (I/DD) for the purpose of increasing the extent to which they are independent, integrated members of the community. All tasks must be implemented in a humane and competent manner. The DirectSupportProfessional role is not specific to one location, building, classroom, day habilitation or group home. Responsibilities:
Responsible for reading, knowing and complying with all appropriate governing documents to include contracts, agency policies and procedures, State or Federal standards, specific program operations manuals and circulars both internal and external.
Ensures compliance with the agency's Operations and Personnel Manuals and the Division of Developmental Disabilities Standards Manual.
Completes and submits in a competent, and timely manner all reports, forms, and documentation as required by the agency and Division of Developmental Disabilities.
Understands basic developmental disabilities and learning styles, using motivation techniques to help clients improve in areas with behavioral or medical challenges.
Must complete all CDS trainings and refreshers as directed.
Attend staff meetings, all other relevant meetings and training as directed.
Assist clients with goal outcomes established on the Individual Service Plan (ISP).
Responsible for reading the ISP and working within the documented expectation for client development.
Provides a consistent means of relaying accurate and complete information to direct supervisors.
Implement behavior management strategies to promote positive behaviors and reduce challenging behaviors according to plans that are in place.
Provides training and supervision to clients in all aspects of community experience living.
Serves as a positive role model for clients and other staff.
Provides safe transportation in agency vehicles for clients as necessary.
Maintains confidentiality and professionalism when communicating with family members or group home staff, Support Coordinators and any outside agency personnel.
Maintains active, positive engagement with clients at all times.

Proactively accompany and support clients during medical appointments, ensuring relevant information is provided to the healthcare professional. Clearly and accurately communicate details from the appointment to the Residential Coordinator, Medical Compliance Officer, and/or designee.
Follow and demonstrate understanding of medication policy and procedures in the monitoring and administration of the client's medications. Completes Medication Error Report when needed.
Ensures home, grounds, yard and van are maintained in clean, safe, and sanitary condition.
Ensures meals are prepared and planned based on clients' individual diet, client input, and the planned menu for the month.
Ensures that the Recreation Calendar is carried out as per client input and as per the planned calendar completed each month.
Responsible for competing all required documentation to include but not limited to, daily logs, communication logs, and behavior reports.
Ensures line of sight for the individual(s) being supported is maintained and completes reports accordingly.
Responsible for adequately and completely cleaning the home during any shift worked.
Ensures that a client's dignity is not compromised in any way.
Responsible for reporting all emergencies or unusual incidents to direct supervisor.
Cooperates with the licensee and Department staff in any inspection, inquiry or investigation.

Maintains Carelogic expectations.
Checks CAU e-mail account on a regular basis.
Provides appropriate directsupports to members.
Cooperates with Agency personnel, legal authorities, and Department of Human Services, or any part or successor thereof, in any inspection or investigation.
All other duties as assigned.
SHIFT: 3pm-11pm with some variation. Weekends required. Days vary by program need.

Position Overview:
Respite Connections is a Saturday respite program for youth ages 3-21 with intellectual and developmental disabilities. The program takes place two Saturdays per month and provides enrichment activities to enhance the social skills and confidence of program participants. Respite Connections is designed to support the development of hobbies, social interaction, and character development. The Respite Connections DSP assists the Program Lead to ensure safe and appropriate services are delivered and supervises youth enrolled in the program.
Key Responsibilities:
Ensures that all work is carried out in a manner that is both respectful and empowering to families and individuals with developmental disabilities.
Assists the Program Lead with care and supervision of children attending the Respite Connections program.
Participates in the program's wide array of indoor and outdoor activities including arts, crafts, games, music, dance and educational projects, and group activities.
Assists in ensuring the comfort, health safety, and well-being of all program participants.
Is attentive, flexible, and supportive of youth enrolled in program.
Encourages and models social behavior and appropriate self-expression.
Provides input at staff or family meetings and as needed discussing participant wellbeing, behaviors, and program quality.
Based on need, may be required to travel to other after-school program locations to fill in if short staffed.
Maintains CPR/First Aid certification (every two years) and Handle with Care annually, along with other job requirements as necessary.
Completes 6 professional development hours annually.
Other duties as assigned by your supervisor and/or Family Support Services Manager.
Qualifications
High School Diploma or equivalent required. Associate Degree preferred.
Must be 18 years old. Minimum of one-year experience in working directly with individuals with developmental disabilities. Related course work or experience with children with challenging behaviors a plus.
