    Job Purpose The Day Program Facilitator provides direct support to clients who participate in the Day Program including developing and facilitating groups, as well as, individual support. The facilitator documents all information required to determine the client's progress towards the achievement of group and individual participant goals. Essential Functions Create and facilitate groups that help clients achieve goals and outcomes identified in their Individual Service Plan Understand and follow client's Individual Service Plan to ensure excellent service provision; provide assistance to program participants as needed Transport clients as assigned (able to drive a 15-passenger van) Foster the growth of individuals by teaching and offering opportunities for choice, initiation, and risk-taking Make preparation and serve lunch for attendees Provide crisis intervention, conflict resolution, and mediation as needed. Participate in all staff meetings, trainings and client meetings as assigned. Must check email and voicemail daily Utilize Electronic Health Record System to create accurate and timely notes on all services delivered and relevant client information that meets or exceeds agency and state standards Complete other departmental documentation Correct all documentation as directed by supervisor Attend all electronic health record (Foothold) training as required Commitment to Core Values Adhere strictly to HIPAA requirements and all agency confidentiality guidelines. Establish positive, balanced relationships with clients, as demonstrated by: treatment of individuals with respect and dignity and in an age and culturally appropriate manner; a commitment towards the individual's rights and responsibilities; and establishment of clear boundaries that allow for productive interactions. Assists the Program Lead with care and supervision of children attending the Respite Connections program. Assists in ensuring the comfort, health safety, and well-being of all program participants. Is attentive, flexible, and supportive of youth enrolled in program.
