In this role, you will ensure that intake and referral processes are completed efficiently and effectively to meet the needs of patients, families, referral sources, and the organization. You will play a vital part in building strong professional relationships, coordinating admissions, and supporting community awareness of hospice services. WEEKEND FULL TIME - Friday, Saturday and Sunday (3 12s, 36 hours) Essential Responsibilities + Receive and respond to referrals from hospitals, home health, physicians, case managers, skilled nursing facilities, and other healthcare professionals + Assess patient eligibility for hospice services by reviewing medical records, conducting clinical evaluations, and communicating with physicians and families + Educate patients and families on what hospice care includes, how it works, and why it matters-providing clarity, reassurance, and compassion + Complete necessary documentation and coordinate the intake and admission process, ensuring all legal, clinical, and payer requirements are met + Serve as a liaison between referral sources and our hospice care team-ensuring seamless handoffs, clear communication, and rapid response to needs + Make daily visits to hospitals and facilities, maintaining visibility and accessibility with discharge planners, case managers, and physicians + Build and maintain strong relationships with referral partners and identify new outreach opportunities to increase awareness of hospice services About You Education + Licensed Nurse with three to five years of hospice experience required. + BSN with five or more years of hospice experience preferred. Experience + Minimum of three years of hospice experience required. + Prior experience as a discharge planner, case manager, or educator preferred. Licenses and Certifications + Current RN license to practice nursing in the state of employment. + CPR certification. + Valid driver's license and automobile insurance. Monitor compliance with regulations for orthotics and prosthetics ordering and payment. Make appropriate/timely referrals, including documentation to post discharge providers/physicians. Ensure accuracy of discharge and payor-related information in the patient record. Participate in utilization review process\: data collection, trend review, and resolution actions. Participate in case management on-call schedule as needed. Qualifications • License or Certification: Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling). If licensure is required for one's discipline within the state, individual must hold an active license. Must meet eligibility requirements for CCM or ACM™ certification upon entry into this position OR within two years of entry into the position. CCM or ACM™ certification required OR must be obtained within two years of being placed in the Case Manager II position. • Minimum Qualifications: For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree. In this vital role, you will recruit, train, supervise, and support volunteers who provide essential services to patients and families. As a key member of the interdisciplinary team, you will be the voice and advocate for volunteers while ensuring the program aligns with federal hospice regulations and community needs. Key Responsibilities + Supervise all volunteer activity within the designated service area. + Maintain compliance with the Medicare Hospice Conditions of Participation 5% volunteer requirement. + Assess patient and family needs for volunteer services and coordinate appropriate placements. + Recruit, train, and retain Administrative, Direct Care, Vigil, and Bereavement volunteers. + Facilitate volunteer orientation and annual training requirements. + Ensure completion of health screening requirements (e.g., TB tests, health questionnaires). + Serve as liaison between volunteers and staff to promote strong communication. + Represent the volunteer program at interdisciplinary team meetings and in the community. + Maintain accurate and up-to-date volunteer records, including visit documentation, evaluations, and monthly statistics. + Participate in the hospice's quality assessment and performance improvement activities. + Support volunteers with regular communication and mentoring. + Serve as a backup volunteer when needed. + Assist with reviewing and updating volunteer policies and procedures. Responsibilities * Possesses clinical knowledge, experience and acumen to identify, present and discuss potential home recovery care patients to providers for program inclusion. These direct discussions usually include ED providers and hospitalists but may span across multiple subspecialties. * Facilitates communication and coordination between all members of the care team to coordinate admissions. * Coordinates referrals and appropriate resources to assist patient and/or caregiver in continuation of care in the outpatient setting. * Interacts with the patient and the multidisciplinary team to coordinate the services ordered by our partners' physicians. * Communicates to the appropriate providers about any barriers to the patients' fulfillment of our partner provider's care plan. * Watches for trends and hurdles involved in health care system and incorporates operational solutions for system challenges, including patient, family and physician responses into an evolving process and model that increases quality and satisfaction- patient or physician. * Builds and maintains collaborative professional working relationships with physicians, medical directors, clinicians, and community at large to develop and implement a successful cross-continuum care management process. * Works with partner physicians to educate patients on the home recovery care model; provides education regarding use of the telehealth system and processes. * Ensures that patients have access to appropriate services to meet their provider-directed care plan needs. * Performs other related duties as assigned.
