The Caregiver @ BridgePoint at Los Altos meet community residents' needs by helping with activities of daily living. Responsible for the delivery of quality care by encouraging independence and ensuring each resident's physical, social, cultural, and intellectual well-being.
We have part time and full time positions open for all shifts:
AM: 6am-2pm
PM: 2pm-10pm
NOC: 10pm-6am
We are looking for individuals to support our wonderful residents as a Caregiver and cross-train as a Medication Technician with flexible availability!
Pay range: $22-25/hr DOE
Hiring bonus: $1000 (FT) / $500 (PT)
What will I do every day?
Monitor residents' well-being in a general way and report any changes in condition to the appropriate associate/department immediately
Assist residents with simple requests as needed or find appropriate associate to assist
Learn resident preferences for opportunities to provide exemplary customer service, exceed resident expectations and enhance resident satisfaction
Supervise or assist with bathing, dressing, grooming, meals, ambulation and other activities of daily living
Provide companionship and develop strong resident relations
Respond to emergencies according to policies and procedures
Help residents with assistive devices (walkers, wheelchairs, hearing aids, etc.)
Encourage self-care of residents
Document care provided to residents consistent with documentation standards
Provide input to the Care Services Director and/or the Resident Care Coordinator/Dementia
Care Coordinator to assure that resident individual service plans are current
Review24 Hour Report and care check lists regularly
Maintain resident confidentiality and privacy
Practice standard precautions, infection control, and safe work practices
Observe and monitor residents to provide or assist with documentation regarding resident status
Cover the Customer Service desk in the Care Services lobby, if necessary
Answer telephone according to company policy and standards
Support, lead and participate in resident-centered wellness programs
Resolve resident questions according to set policies and procedures
Greet and direct family members, potential residents, prospects and other individuals to the appropriate staff contacts in the community
Assist and participate in community special events and marketing events
Assist residents in meeting their social needs through interaction with staff, family and other residents including encouraging and reminding residents of wellness programming
Assist in the general upkeep of the community common areas
Maintain upkeep of residents' rooms (e.g., making bed, taking out trash, washing dishes)
Assist with resident laundry as assigned
Support open and honest communication with and between all associates
Work as part of the team within your department and across the community
Maintain high standards of personal appearance and grooming, which includes wearing the proper uniform and name tag when working
Look for ways to reduce costs and create efficiencies on a daily basis
Ensure regulatory compliance and report any issues or concerns immediately to supervisor
Support continuous improvement
Adhere to all regulations, laws and company standards, including the Associate Handbook
Understand and support the community sales process
Perform other related duties as required

Ability to work a flexible schedule, including weekends and holidays as needed

The Care Partner is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our Terrace Club neighborhood based on the Designated Care Partner concept. The Care Partner role includes providing hands on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe, comfortable home like environment. RESPONSIBILITIES & QUALIFICATIONS
Essential Duties

Resident Care
* Review, read, notate and initial Daily Log to document and learn about pertinent information and any resident's physical and behavioral changes.
* Review and provide input to the Individualized Service Plans (ISP) and Leisure Assessment.
* Assist in the Terrace Club Program, based on individual resident interests and skills that provide stimulating programs, according to Terrace Club Guidelines and Policy.
* Partner with residents to help create successful days through prompting and cueing reminders.
* Assist in implementation of the Daily Program Calendar for each day's events and journal entry with the residents.
* Demonstrate effective use of written communication skills to assists residents with daily journal entries.
* Observe, report, and document symptoms and conditions of residents.
* Review designated assignments for the day.
* Responsible for their designated group of residents during the shift, know where their residents are and physically check on them throughout the shift.
* Attend Cross Over Meeting
* Notify supervisor and Resident Care Director if a resident has increased care needs.
* Check residents for changes in skin condition, side effects of medications, and changes in overall health conditions.
* Inform supervisor of any resident changes of condition and resident family concerns.
* Respond to security system quickly and appropriately and answer all call bells immediately and take appropriate action including resetting call bells.
* Answer, direct, and/or respond to all phone calls in a timely, courteous, and professional manner.
* Toilet all residents as indicated and dispose of all incontinence products properly to ensure sanitation of resident suite and community restrooms (universal care precautions).
* Communicate with families and is a resource as a Designated Care Partner.
* Help maintain the self-respect, personal dignity, and physical safety of each resident adhering to the standards of Resident Rights.
* Maintain an atmosphere of warmth, personal interest, and positive emphasis as well as a calm environment.
* Ensure the established safety regulations are followed at all times.
* Maintain universal care precautions of cleanliness, hygiene, and health standards.
Risk Management and General Safety
* Partner with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements and promotion of Risk Management programs and policies; adherence to safety rules and regulations.
* Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, MSDS (Material Safety Data Sheets), and Lockout Tagout procedures.
* Report all accidents/incidents immediately.
* Report all unsafe and hazardous conditions/equipment immediately.
* Ensure any cords, carts, equipment, and other hazards are kept out of the way at all times, not blocking exits, and in compliance with fire codes.
* Comply with all infection control techniques, placement of bio-hazard containers, and removal techniques, procedures, and policies.
* Understand and practice the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials.
* Report occupational exposures to blood, body fluids, infectious materials, sharp sticks, and hazardous chemicals immediately.
* Ensure oxygen tanks are stored safely, exchange guest/residents tanks when empty, and monitor to make sure liters of oxygen are at prescribed levels.
Housekeeping & Laundry Services
* Maintain and clean resident's room as assigned including making beds, emptying trash, putting clothes away appropriately, spot sweeping, and mopping as needed.
* Wash resident's laundry as noted in the ISP and as needed; encourage resident participation.
* Dispose of all incontinent products properly using universal care precautions of cleanliness and promote sanitation.
* Wash and fold dining room linens and napkins.
* Complete assigned housekeeping tasks.
* Maintain common areas clean and tidy at all times.
Dining Services
* Serve meals in the dining room and work in the dining room as assigned.
* Promote and ensure a pleasant dining experience during all meals.

* Participate in pre-meal meetings.
* Follow residents diets as indicated on ISP.
* Observe, note, chart, and report residents changes in eating habits.

This may include bathing, grooming, personal and dental hygiene, bowel and bladder functions, dressing, hair care, escorting to and from meals and activities, and tidying resident room.
Support team members as they encourage personalized and meaningful resident participation in life enrichment programming and ADLs.
Assist with meal service in dining room. Coach team members on hospitality standards during meal service.
Document care and services provided to residents accurately, review care manager documentation throughout shift, and identify and bridge gaps in documentation by coaching team members.
Ensure compliance with Sunrise's Timekeeping and Meal/Rest Period policies, monitor team member break schedule and duration, ensure care managers clock in/out using correct job codes, and coach team members as required.
Display and encourage flexibility in work schedule by working holidays, evenings, weekends, and additional shifts when necessary.
Prepare for shift by obtaining resident group assignments before care managers arrive and print and hand out to appropriate care managers.

Participate/lead and assist residents with activities of daily living (ADLs) and Invite, Encourage, and Assist (IEA) residents to participate in activities as indicated on the ISP.
Engage residents in life skills and other life enrichment activities throughout the day in reminiscence.
Strive to understand and respond to each resident with empathy, always remaining mindful of the resident's unique communication patterns or history and basic human needs.
Maintain an atmosphere of warmth, personal interest, and positive emphasis as well as a calm environment.

Risk Management and General Safety
Partner with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements, promoting Risk Management programs and policies, and adhering to safety rules and regulations.
Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
Report all accidents/incidents immediately.
Reports all unsafe and hazardous conditions/equipment immediately.
Ensure any cords, carts, equipment, and other hazards are always kept out of the way, not blocking exits, and in compliance with fire codes.
Understand and practice the proper method of attending to and disposing of, and the possibility of exposure to, blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials.
Report occupational exposures to blood, body fluids, infectious materials, sharp sticks, and hazardous chemicals immediately.
Ensure oxygen tanks are stored safely, exchanged when empty and monitored to make sure liters of oxygen are at prescribed levels.
Housekeeping and Laundry Services
Maintain and clean resident's room as assigned including making beds, emptying trash, putting clothes away appropriately, spot sweeping, and mopping as needed.

Blend a variety of multi-sensory experiences into the resident's day.
Participate in monthly letters home and letter writing with the residents and their families.

The Caregivers a BridgePoint at Los Altos meet community residents' needs by helping with activities of daily living. Responsible for the delivery of quality care by encouraging independence and ensuring each resident's physical, social, cultural, and intellectual well-being. This position will specifically be working with our residents with Mild Cognitive Impairment (MCI).
This is a full time position, Monday-Friday, 9:30am-5:30pm with flexibility to adjust hours as needed for business needs.
Pay range: $22-23/hr

What will I do every day?
* Actively participating in programs and activities to keep residents engaged.
* Assisting residents as needed, including toileting support and escorting them to and from common areas or their apartments.
* Monitor residents' well-being in a general way and report any changes in condition to the appropriate associate/department immediately
* Assist residents with simple requests as needed or find appropriate associate to assist
* Learn resident preferences for opportunities to provide exemplary customer service, exceed resident expectations and enhance resident satisfaction
* Supervise or assist with bathing, dressing, grooming, meals, ambulation and other activities of daily living
* Provide companionship and develop strong resident relations
* Respond to emergencies according to policies and procedures
* Help residents with assistive devices (walkers, wheelchairs, hearing aids, etc.)
* Encourage self-care of residents
* Document care provided to residents consistent with documentation standards
* Provide input to the Care Services Director and/or the Resident Care Coordinator/Dementia
* Care Coordinator to assure that resident individual service plans are current
* Review24 Hour Report and care check lists regularly
* Maintain resident confidentiality and privacy
* Practice standard precautions, infection control, and safe work practices
* Observe and monitor residents to provide or assist with documentation regarding resident status
* Cover the Customer Service desk in the Care Services lobby, if necessary
* Answer telephone according to company policy and standards
* Support, lead and participate in resident-centered wellness programs
* Resolve resident questions according to set policies and procedures
* Greet and direct family members, potential residents, prospects and other individuals to the appropriate staff contacts in the community
* Assist and participate in community special events and marketing events
* Assist residents in meeting their social needs through interaction with staff, family and other residents including encouraging and reminding residents of wellness programming
* Assist in the general upkeep of the community common areas
* Maintain upkeep of residents' rooms (e.g., making bed, taking out trash, washing dishes)
* Assist with resident laundry as assigned
* Support open and honest communication with and between all associates
* Work as part of the team within your department and across the community
* Maintain high standards of personal appearance and grooming, which includes wearing the proper uniform and name tag when working
* Look for ways to reduce costs and create efficiencies on a daily basis
* Ensure regulatory compliance and report any issues or concerns immediately to supervisor
* Support continuous improvement
* Adhere to all regulations, laws and company standards, including the Associate Handbook
* Understand and support the community sales process
* Perform other related duties as required
What will I need to be successful in this role?

* Nursing Assistant Certification (CNA) is desirable
* Basic competency in the use of multiple forms of technology, including: Microsoft Office applications, internet-based and custom applications
* Ability to work a flexible schedule, including weekends and holidays as needed
