Responsibilities
Evaluation and Treatment Implementation
Conduct comprehensive evaluations of patients across all age groups-from neonates to geriatrics-using age- and disability-appropriate assessments to establish baselines and develop individualized treatment plans that promote functional improvement.
Assess patient abilities and limitations including neuromuscular movement and stability, sensation, perception, muscle strength, range of motion, body mechanics, vocational and prevocational abilities (adults), and developmental milestones (pediatrics).
Evaluate patients' home and work environments and recommend necessary physical barrier modifications or ergonomic adaptations to support safety and function.
Treatment Delivery and Monitoring
Administer therapy consistent with the treatment plan, employing suitable procedures, equipment, and techniques within the scope of physical therapy practice.
Recommend, justify, and arrange for the delivery of therapeutic equipment as needed.
Continuously re-evaluate patients, monitor progress, and adjust treatment plans to ensure optimal outcomes. Make discharge recommendations when appropriate.
Collaborate and co-treat with therapists and other healthcare professionals to provide integrated, multidisciplinary care.
Facilitate the use of orthotic and prosthetic devices to enhance patient mobility and function.
Documentation and Communication
Maintain clear, accurate, and timely documentation of patient care, including medical records and correspondence with physicians. Ensure that all information is concise, legible, and relevant to the patient's plan of care, condition, treatment response, and functional outcomes.
Tailor verbal and written communication to the age, educational level, and needs of patients, families, and other stakeholders, ensuring information is delivered clearly and appropriately.
Patient and Family Education
Provide education to patients and families across all age groups to support recovery, self-care, health maintenance, health promotion, and wellness.
Supervision
Supervise the patient care activities of aides, assistants, attendants, and volunteers, ensuring high standards of care and support are maintained.
Qualifications
Education:
Bachelor's degree from an accredited Physical Therapy program approved by the American Physical Therapy Association.
Skills:
Ability to provide patient services and supervise Licensed Physical Therapy Assistants (LPTAs), Therapy Aides, and students.
Demonstrate excellent organizational skills to support program development.
Competently assess and interpret patient information across all ages, with knowledge of normal growth, development, and treatment needs.
Skilled in developing home programs and conducting family training.
Knowledgeable in the appropriate selection and use of durable medical equipment (DME) such as wheelchairs and walkers.
Proficient in computer skills relevant to documentation and patient care management.
Licensure

Responsibilities
Provides social work services in a variety of adult and pediatric settings across the health care continuum with a focus on holistic patient-centered care and attention to the needs of vulnerable populations. Works both independently and collaboratively as part of an interdisciplinary team.
Key services include (but are not limited to) comprehensive evaluation of psychosocial and mental/behavioral health status, legal and safety considerations, substance use or addiction, chronic illness, or end of life issues with the use of brief counseling, transition planning, crisis management and planning and coordination with internal and community stakeholders. Social work intervention may be with individuals, family members or groups.

Our department operates as a system department and provides services to Acute Care, Inpatient Rehabilitation Facility, Home Health and Outpatient settings across four campuses. Our Rehabilitation departments offer a variety of specialty services across the various services delivery areas.
POSITION OVERVIEW: The Occupational Therapist evaluates, plans, and provides treatment and rehabilitation as prescribed by a referring physician. Works with family and team members regarding patient's abilities and goals.
This position does not directly manage any other caregivers.
ESSENTIAL FUNCTIONS AND DUTIES: RESPONSIBILITIES:
Adheres to Oregon State Occupational Therapy Licensing Board.
Provides occupational therapy evaluation and treatment to achieve desired outcomes.
Provides staff consultation and training.
Provides community resource knowledge and referral services.
Provides occupational therapy services to other hospital service areas.
May be required to drive Rehabilitation van and meet all related SCHS vehicle policy requirements.

Responsibilities
Provides social work services in a variety of adult and pediatric settings across the health care continuum with a focus on holistic patient-centered care and attention to the needs of vulnerable populations. Works both independently and collaboratively as part of an interdisciplinary team.
Key services include (but are not limited to) comprehensive evaluation of psychosocial and mental/behavioral health status, legal and safety considerations, substance use or addiction, chronic illness, or end of life issues with the use of brief counseling, transition planning, crisis management and planning and coordination with internal and community stakeholders. Social work intervention may be with individuals, family members or groups.
Throughout the care continuum, social workers will utilize a strengths-based approach with the aim to enhance the capacity of people to address their own needs.
Qualifications
Clinical Social Worker I CSWA Position Requirements:
Education:
Master of Social Work degree from an accredited School of Social Work required.
Experience:
Preferred: 1 year of relevant experience in care management, mental health and/or addictions services, medical social work, or social service organization.
Clinical Social Worker II LCSW Position Requirements:
Education:
Master of Social Work degree from an accredited School of Social Work required.
Experience:
2 years relevant experience in care management, mental health and/or addictions services, medical social work, or social service organization. Preferred: 3 years' relevant experience.
Skills:
* Broad knowledge of the principles of social work and the ability to apply them with individuals, families and/or groups integrating knowledge of normal development and the aging process, changes related to illness, and the needs of patients with acute, chronic, and terminal illness in assessment and intervention with attention to cultural and literacy implications.
* Knowledge of medical, mental health and substance use diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology
* Strong organizational, communication and collaboration skills, including an ability to effectively triage patient and family needs, function as member of an interdisciplinary team, identification and collaboration with community stakeholders or resources and written documentation.
* Knowledge about social determinants of health, principles of diversity/equity/inclusion and culturally responsive practice with populations who have experience historical marginalization
* Demonstrates ability to locate, utilize and advocate for patient/family access to hospital/ community resources
* Exhibits knowledge of legal/regulatory issues and an ability to be a resource to all team members regarding such issues
* Ability to navigate electronic medical records and effectively utilize basic computer programs.
* Knowledge of legal, regulatory and compliance requirements and an ability to be a resource to all team members regarding such issues and knowledge of current health care environment.
