Responsibilities
* Helps build collaborative working relationships both internally and externally.
* Responsible for being a good steward of the company's financial resources by projecting a return on monies spent and managing to a marketing expense budget.
* Responsible for executing effective sales calls that identify and meet the needs of the referral community and clearly communicate the features and benefits of the LHC Group. * Attends staff meetings to give an update regarding accounts, customer needs, and progress towards agency growth strategies.
* Coordinate in-services with community and medical-based groups to educate them on hospice services on an as needed basis
* Responsible for checking e-mail daily and responding to phone messages within 24 business hours.
* Responsible for all sales administration duties including, but not limited to, CRM compliance, expense reports, payroll time sheets, medical director time sheets, strategic territory planner, PTO requests, paperwork (POC) delivery or pick-up when needed.

Responsibilities
The Executive Director (Registered Nurse, RN) in Hospice supervises all aspects of patient care, all activities of professional staff and allied health personnel, regulatory requirements compliance, financial performance, and quality assurance performance improvement activities for agency. The Executive Director or alternate will be available on-site during business hours and additionally, if needed.
* Responsible for compliance with all regulations, laws, policies, and procedures applicable to hospice and Medicare/ Medicaid issues when applicable daily.
* Immediately available (or has Alternate Designee) to be on-site during business hours or immediately available by telephone when off-site conducting agency business, available after hours as needed.
* Directs the daily operations of the agency and acts as the Care Management Process driver. The Executive Director or designee RN, such as a Patient Care Manager, will receive daily reports on patients from staff. This process also includes review of patient care paperwork, referral information, recertification processes, discharge information, and event reporting, etc. daily.
* Reviews monthly financial results relative to all aspects of the operation to assure that quality patient care is delivered in the most cost-effective manner.
* Participates in the QAPI planning and processes, reporting, and improvement action plans as indicated. This includes chart audits, patient satisfaction, financials, contracts, patient/family complaints, etc. and appropriate follow-up.
Education and Experience
License Requirements
* Must be a licensed physician; licensed registered nurse; licensed social worker; or a Bachelor's degree college graduate with at least three (3) years of documented success in discipline/field of study and a minimum of one (1) year of full time experience in a hospice, home health, or other health care delivery system setting.

Duties:
The Dietitian Long Term Care also performs business and supervisory functions, such as management of resources in a cost-effective manner, and participates in compliance and quality performance improvement efforts to ensure that nutrition care is safe and effective. The Dietitian Long Term Care is an integral part of the multidisciplinary team, is involved in team conferences, provides food and nutrition-related in-services, contributes to the development and revision of facility policies, and maintains professional competency and skills required for professional practice. The Dietitian Long Term Care may be a consultant or staff and also may provide support and guidance to foodservice management staff.
Qualifications:
Must have a Bachelor's degree in dietetics, foods and nutrition, or related area, granted by a U.S. regionally accredited college or university. Completion of a didactic program in dietetics and supervised practice program approved by the Commission on Accreditation for Dietetics Education (CADE) of the American Dietetic Association. A minimum of 1 to 2 years of experience as a clinical long term care dietitian preferred. Must be a Registered Dietitian with active registration by the Commission on Dietetic Registration of the American Dietetic Association; Certification/Licensure as required by state of practice. Board Certified Specialist in Gerontological Nutrition (CSG) preferred. Must be able to perform essential functions of the position with or without reasonable accommodation. Must qualify for employment, after criminal background check, per guidelines of the Minnesota Department of Human Services.
