Essential Functions
Works in collaboration with the Choices interdisciplinary team, overseeing day to day team and patient schedules, patient tracking, care needs, evaluating referred clients, facilitating admission, coordinating care among providers and other team members, providing nursing services both in person and virtually, and assessing and addressing the ongoing needs of patients and caregivers, in order to ensure the effective delivery of palliative care to the patient and family.
