The SocialWorker in the Continuing Care Department is an integral member of a hospital patient's care team, responsible for assessing and intervening to ensure the effective continuation of a patient's care after discharge from the hospital. The socialworker will be assigned to a unit or units, which may include medical, surgical, intensive care, OB/Maternity, pediatric, and/or Emergency Department, and will work closely with physicians, nurses, and other practitioners on the assigned unit. SocialWorker is responsible for interviewing hospital patients and family members/caregivers; facilitating family meetings to identify needs and develop continuing care plan; assessing psychosocial needs; counseling patients and family members to promote effective coping and treatment plan compliance. Other duties include assessing knowledge deficits; educating patients and family members about community resources, rights and responsibilities, advance directives, end-of-life care issues, health insurance, and navigating the health care system. The SocialWorker coordinates complex care plans involving a wide range of internal and external services including home health, social services, rehabilitation, and mental health providers; advocates for patients to receive needed resources and services. Functions as part of a multidisciplinary team. As opportunities arise, the SocialWorker will participate in department and hospital initiatives related to process improvement and patient care.
Education:
Masters' in Social Work; equivalent experience or certification considered

Who you are:
The LCSW-C (Licensed ClinicalSocialWorker) Case Manager anticipates and coordinates care / services / health education for targeted member populations with chronic disease conditions or high-cost specialty care. They demonstrate knowledge of complex case management processes and are responsible for ensuring that member goals are achieved within a specified time based upon individual needs, reasonable expectations, with well-documented outcomes
What you'll do:
* Maintains budgeted caseload.
* Identifies and analyzes staff/member/system issues that impact the plan of care
* Identifies and acts upon impending changes in member condition.
* Must be able to maintain accurate documentation that meets regulatory standards, policies and procedures.
What you'll bring:
* Minimum 3 years of clinical experience.
* Managed Care experience preferred
* Demonstrated clinical experience
* Knowledge of health care and managed care delivey systems
* Insurance benefit structures and related medical/legal issues
* Utilization management and quality improvement processes
* Member satisfaction
* Education- Master's Degree in Social Work
* Licensure:Current LCSW-C licensure in the state of Maryland
* Certification in case management (CCM) or pursues CCM or similar certification within 2 years of hire date
Salary Range: Minimum 31.92/hour - Maximum 52.69/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.

SUMMARY
In collaboration with patients/families, case managers, physicians and the interdisciplinary team, the Mental Health Therapist 1 provides direct psychosocial and practical care including psychological assessments, problem identification, goal setting, behavioral intervention and implementation of care plans. He / She Collaborates with all disciplines to optimize patient outcomes; scope of services may include, but is not limited to: developing action plans to mitigate psycho-social barriers to discharge; patient/family engagement and education; employing interventions (crisis intervention, grief counseling, suspected abuse reporting) to address immediate needs.
ESSENTIAL FUNCTIONS

* Completes diagnostic intake in accordance with joint commission requirements.
* Completes full psychosocial assessment and plan of care for patients and families who require intervention.
* Provides direct psychotherapy and psychosocial care for a full case load of patients in assigned area.
* Works in a collaborative practice model, facilitating care coordination with the multidisciplinary treatment team for care progression to optimize patient outcomes.
* Monitors effectiveness of medication and observes for and reports side-effects of medication. Notifies and consults with physician if medication changes are indicated.
* Adheres to departmental and regulatory documentation standards and formulates clear descriptions of the problems, goals and interventions. Adheres to timely completion of medical record documentation.
* Implements and regularly uses feedback informed treatment (FIT).
* Maintains a current and productive network of referral resources. Seeks out local, regional and national community resources to meet patient need.
* Assists with resource referral, capacity determination, legal guardianships, adoptions, psychiatric referral, and assessment of abuse/neglect/IPV, end of life and charity care.
* Identifies need for and facilitates family meetings.
* Leads or participates in interdisciplinary rounds if applicable for assignment area and articulates social work role to the team members.
* Participates in quality improvement activities.
* May be required to provide coverage during absences and vacancies.
* May serve on committees within the medical center and community.
* Provides on-call coverage as needed.
* Participates in supervision as mandated by COMAR regulations.
* Does not include clinical supervisory responsibility.
* Other duties as assigned.
QUALIFICATIONS
Education:
* Masters degree in applied psychology or related field.
Knowledge, Skills, and Abilities:
* Excellent interpersonal skills, including ability to work collaboratively and cooperatively within an integrated interdisciplinary team.
* Comprehensive knowledge of community resources.
* Knowledge of and ability to gather relevant data to synthesize and summarize information in making judgments regarding patient care.
* Excellent oral and written communication skills necessary to communicate/exchange and interact effectively and professionally with hospital staff, agency health care professionals, patients and families.
* Prioritizes work in a timely and effective manner.
* Proficiency with an electronic patient record, email, general office and communications equipment.
Required Licensure, Certification, On-going Training:
* Maryland LGPC required
Work Experience:
* One-year experience, healthcare related field preferred
* Field placement may be considered in work experience
