A few details about the role:
Participate with members of the interdisciplinary team to review, plan, coordinate and evaluate resident's care.
Document the resident condition and nursing needs accurately and in a timely manner. Reports pertinent observations and reactions regarding residents in a timely manner.
Develop, direct, and monitor nursing assistant assignments and adjust based on census and level of care required.
Execute treatments as necessary and document reactions to medications and treatments.
Generate, prepare, administrate, and chart all medications.

Establish protocol to ensure care plans are continually reviewed for updated and accurate data that represents the individuality of the resident.
Coordinate admissions, discharges, and transfers in order to deliver quality customer service.

As an LPN at Fraser, you will work closely with families and psychiatry services staff to provide high-quality, personalized, and integrated healthcare as part of our CCBHC services. In this role, you will also serve as a liaison, managing administrative tasks that support client flow and the daily operations of the Psychiatry and RN Services team.
Qualifications:
Active LPN license in the state of MN.
Current BLS/CPR Certification required.
Experience working in the community with pediatric populations with behavioral health needs preferred.

Possession of a valid Minnesota driver's license and maintenance of a satisfactory driving record consistent with Fraser policy.

Responsibilities:
Provide high quality mental health client care as part of a collaborative healthcare team within CCBHC model.
Coordinate client management with members of Fraser Psychiatry Services and Fraser Care Team.
Assist Fraser staff with collecting information to help assess risk for incoming and ongoing clients of Fraser Psychiatry Services.
Help high risk patients with increased support, acting as a liaison with the primary care provider and other team members as needed.
Arrange for necessary support services consistent with level of risk and according to care plan.
Track problems, address barriers to care and support strategies that reduce ED utilization in high risk clients.
Participate in the scheduling, rooming, referral, prescription, and follow-up of psychiatric and internal and/or external services.
Support the completion of diagnostic evaluations/assessments, medication therapies and medical management with tasks, as requested by Fraser providers.
Complete thorough and accurate documentation of client encounters, maintain clinical files and up to date records.
Communication to other providers including information/education about Fraser Team Care as it pertains to the client.
Request of initial and refill prescriptions.
Support Psychiatry Services Team with:
Request of documentation/information from other providers.
Request of initial and refill prescription
Questions and support to families regarding management of medication, scheduling of follow-up visits in accordance with care plan.
Referrals.

Tuesday through Friday from 8:00 am - 4:30 pm or 8:30 am - 5 pm (working 32 hours a week)
The starting pay range for this role is $28.85 - $30.00 an hour depending on qualifications

Responsibilities
The licensed practical nurse (LPN) provides nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor and accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family. Qualifications
Graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program (e.g., some states allow RN students to take the LPN board exam after completing one to two years in the RN program). If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required.
One year LPN experience within the last five years preferred. Excellent communication skills (verbal and written).
Experience working in a team environment. Computer skills required, prior experience with electronic medical record systems preferred. Ability to work daytime hours, flexibility may be required to meet staffing needs. Ability to adapt to unpredictable situations within the work setting. Graduate of an accredited school of practical nursing preferred.
Current LPN license by applicable state requirements. Arizona and Florida: Maintains current Basic Life Support (BLS) for Health Care Providers. Midwest: Maintains current Basic Life Support for Health Care Providers from one of the following programs: American Heart Association or American Red Cross. Additional specialty certification/training as required by the work area.
