The Family Home Visitor will coordinate enrollment for the CHOIC (Choosing Healthy Outcomes for Infants and Children) program and provide services to mothers referred by Children and Youth Services (CYS) in Beaver County. The full-time role will provide free in-home education services and support for families. The position will review and update a Plan of Safe Care in collaboration with the mother; assess infant's safety and monitor infant developmental stages; and offer guidance, training and referrals to parents in order to achieve positive support outcomes for each high-risk family receiving services. Travel throughout Beaver County is required, with mileage reimbursement available. The Family Home Visitor works full-time, Monday through Friday, 8:00am - 4:30pm. SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY * Provide clinical service to help individuals struggling with substances abuse and co-occurring disorders. * Provide clinical services such as individual therapy, group therapy, treatment planning, and aftercare coordination. * Maintain clinical documentation. * Provide assessment services and implement clinical interventions. CLIENT'S REQUIRED EXPERIENCE AND EDUCATION * Masters Degree required.
