Position Summary: Lifesteps is seeking a Disability Services Program Specialist ( known as Instructor / Program Specialist at Lifesteps ) to work with individuals who have special needs at the Adult Training Facility in Kittanning, PA. The role will assist in the development of Individual Goal Plans (ISP's) that ensure the needs and wishes of the individuals are met. The full-time role also works hands on with the individuals with disabilities in the Day Program and coordinates/assists with community participation as well. Office administration is also a main duty of the position, as the role completes and maintains documentation, ISPs, and reports for the program. Program Specialists ensure compliance with 2380 and Chapter 11 regulations. Schedule - Monday through Friday, 8:00 am to 4:00 pm. No evenings, No weekends, No holidays! QUALIFICATIONS: Required: Associate's degree OR 60 credit hours from an accredited college or university and 4 years' experience working directly with persons with disabilities. Degrees related to Education, Special Education, Counseling, Therapy, Psychology, Sociology, Social Services, Transition Programming, Nursing, Health Administration, or similar are preferred, but any major qualifies Possess a valid driver's license which legally permits operating a motor vehicle in Pennsylvania. Obtain and maintain certification in Crisis Prevention Intervention (CPI) - provided by Lifetsteps. Coordinates the training of DSP's in the content of health and safety needs relevant to each individual Developing and implementing provider services to meet State licensing requirements (relating to provider services) Must be fluent on all assigned Individual's ISP's. Performs MAR reviews and medication checks. Provides face to face ISP training to Program Coordinators and DSP's. Acts as Certified Investigator on an on-call basis when necessary. In this role, you will coordinate and implement Individual Support Plans (ISPs) for individuals with intellectual, developmental, and/or behavioral health needs. You will work closely with participants, families, and staff to ensure services are delivered effectively, promote independence, and support each individual's dignity, well-being, and community integration. The Program Specialist is responsible for coordinating, implementing, and monitoring Individual Support Plans (ISPs) for assigned consumers. This role ensures services meet individualized support needs, comply with regulatory requirements, and promote consumer growth, independence, and well-being. The Program Specialist works closely with consumers, families, Direct Support Professionals (DSPs), and the interdisciplinary team to provide high-quality care and support. Key Responsibilities Coordinate and complete initial, annual, and revision assessments for assigned caseload. Identify individualized consumer support needs for direct supervision or unsupervised periods. Disseminate assessments to ISP team members at least 30 days prior to meetings for ISP development or revisions. Provide training to DSPs on consumer Support Plans, including medication protocols, before working with the consumer. Participate in development, review, and revision of ISPs, ISP Annual Updates, and all ISP revisions.
