REHABILITATION COUNSELOR II
JOB SUMMARY: The AIMS Rehab Counselor II position is to facilitate consumers in the AIMS Program in their recovery; assist in the achievement of their maximum recovery, and independence in life.
ESSENTIAL FUNCTIONS and RESPONSIBILITIES:
* Teach goal setting and obtainment
* Teach skill interventions in classroom and in community settings to obtain those goals
* Teach illness management to aid in recovery; and teach job readiness and people skills
* The ability to comply with the regulations requiring a workplace of dignity and respect and equal employment opportunity compliance.

* Provide leadership to other staff
* Knowledge of Psychiatric Rehabilitation intervention skills; medication management; experience with Co-occurring disorder preferred.
* Employee must be empathetic, respectful, and treat consumers with dignity. Initiative, accuracy, and dependability are important. Must be able to advocate for consumer's rights and provide hope and belief that consumer is able to recover.
* Maintain ongoing communication with supervisor and collaborates with the treatment team on a regular basis. Must be able to assist consumer in preparation of Recovery Plan.
EDUCATION & EXPERIENCE:
* BA degree in Human Services required
* Minimum of Three year direct experience with persons having neurobiological brain disorder (SPMI) and substance abuse disorder preferred
* Experience in working with consumers involved in supported employment
TRAINING & DEVELOPMENT:
* Completes all requirements according to Individual Training Plan within the first six months of hire and annually.

The Specialized Foster Care program (SFC) is designed to provide specialized (Matrix Program) and therapeutic (CHIPS Program) services to support adolescents, ages 8-18, to manage emotional, behavioral, and psychiatric problems in a highly supportive, individualized and flexible foster home setting. The SFC program uses a multi-disciplinary approach, evidenced-based practice, specially trained foster parents, clinical counselors, collaboration and therapeutic interventions. The program also engages the adolescent's family as he/she works toward reunification or adoption.
Overview:
Primary duty is the performance of work requiring advanced knowledge (Master's Degree) in the field of Human Services, and to use such advanced knowledge to consistently exercise discretion and independent judgment in providing the highest quality client-centered therapeutic services. Must also use such advanced knowledge to serve as a consultant and mentor to Bachelor's Level Direct Care employees and Behavioral Technicians, among other staff. This position is entrusted with a high level of responsibility and insight in treating clients and ensuring services are provided and documented in compliance with applicable laws and regulations, in addition to LCI and division standards. Based on the knowledge and degree required, this position will serve as a lead position in the provision of direct care. Therapeutic services include but are not limited to, assessment, treatment planning, direct service provision, person centered services and management of client risk and safety.

LEAP TEAM (Linking, Engaging, Advocating, Assessing, and Planning) is a short term to intermediate term multidisciplinary community team designed to engage and treat individuals who often fall through a gap in the community mental health continuum of care. These individuals tend to be high utilizers of acute care mental health systems as well as state mental health treatment facilities. By definition, this population is often difficult to engage in services due to lack of readiness for change and symptom severity/complexity and lack of adherence to treatment. The LEAP team consists of case managers, nurses, peer specialist, counselor, and a physician's assistant for medication management. The team is interdisciplinary and does home visits to meet the individual's right where they are located.

The mission of the LEAP Team is to provide individualized care for individuals with a Serious mental illness that are high users of inpatient/acute care services. Overview
The Forensic Case Manager is responsible for monitoring and assisting individuals involved in the criminal justice system who exhibit serious impairment due to mental illness. The focus of the position is on serving individuals under court supervision adjudicated Incompetent to Proceed or Not Guilty by Reason of Insanity consistent with Florida Statute 3.212 and Rules of Criminal Procedure 916. The Forensic Case Manager's goal is to enhance the independence, autonomy and quality of life of the individual served, while ensuring compliance with all judicial requirements to ensure public safety.
Requirements
Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development).
Must have a minimum of two (2) years of full time or equivalent experience working with adults experiencing mental illness.
Must be eligible for enrollment as an Adult Case Manager by the State of Florida.
Must be a Certified Behavioral Health Case Manager (CBHCM) through the Florida Certification Board within six (6) months of start date
Must have knowledge of available resources in the service area for adults with mental illness.
Must have knowledge of and comply with state and federal statutes, rules and policies that effect the target population.
Must be able to travel locally to clients' homes, training locations and other locations as necessary.

Overview:

* Perform case management intake and ongoing assessment, including social history
* Work with consumer (family) to develop treatment plans with long and short-term goals
* If servicing youth, provide transition services for youth nearing 18 years of age to an adult system of care
* Assists the consumer (family) to develop connections to natural supports within their own network of associates, including friends and neighbors, community organizations, including service and religious organizations and through participation in clubs, organizations, and other civic activities
* Arranges for procurement of incidental expenses of any additional temporary supports that may be necessary to move the family towards greater independence, stability and achieve resiliency goals as documented in their Plan of Care
* Assists in coordination of any adjunctive services that are out of the scope of practice and therefore not provided by Staff
* Assists with transporting consumer (family) to medical appointments, court hearings, or other related activities outlined in the Plan of Care
* Communicate and liaison with the individual, family and service providers
* Must complete required Center, Agency (DCF), funding, legal, statistical and fiscal documentation in an accurate and timely manner
* Must be able to document accurately and remain current with all documentation
* Must be able to attend team meetings as scheduled on a regular basis
* Must be able and willing, to work flexible hours
* If servicing youth, must be able to assist with short term supervision of a consumer away from the family to offer temporary relief as a planned event or to improve family stability in a time of crisis for a maximum of four hours per day
* If servicing you, must be able to assist with the provision of tutoring and remedial academic instruction to enhance educational performance
* Maintain consumer files including all agreements, release forms, service plan and up to date progress notes at required by program guidelines
* Maintain confidentiality at all times

JOB SUMMARY: The FACT Peer Counselor is a member of a multidisciplinary team that has responsibility to carry out rehabilitation and support functions and assist in treatment; provides substance abuse services, education, support, and consultation to families; and provides crisis intervention under the clinical supervision of staff with professional degrees.
