To provide a health coaching service for eligible participants that supports people to gain and use the knowledge, skills, and confidence to become active participants in their health and wellbeing so that they can reach their self-identified health and wellbeing goals, including, but not limited to: Triage and assessment Agenda setting Lifestyle behaviour change support to (e.g.) stop smoking, move more, achieve a healthy weight/diet, reduce alcohol consumption levels. Problem solving Motivational interviewing Goal setting Signposting Deliver multi-channel, personalised health coaching support to individuals and groups in line with their needs and preferences including: Face-to-face, telephonic, and digital health coaching Multi-mix communications methods including in-app messaging, text, and email. Share helpful and supportive resources in line with goal, stage of journey, confidence, and motivation Advocate the use of behaviour tracking tools (online or offline) Appraise individuals and population caseloads daily to ensure participants are provided with the right intensity and frequency of support to drive sustained behaviour change A strength-based approach, that maximising opportunities to utilise local community assets and efficient utilisation of the wider health and wellbeing economy. To deliver services in line with quality standards, national / government guidelines, and role boundaries, with the ability to identify and act accordingly when coaching is not appropriate and being able to sensitively discuss alternatives or escalate appropriately - inclusive on supporting onward referral to wider services.
