Minimum one year's experience Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT. Current CPR certification. Also, the Physical Therapist / PT will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Physical Therapist / PT must set realistic and achievable goals for their patients, document and record the patient's condition, and educate patients and families in an appropriate physical therapy method. Develops a plan of care for each physical therapy patient. Provides skilled physical therapy services / interventions in accordance with physician orders. Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions. Collaborates with all disciplines to plan and evaluate team goals for each patient. Qualifications * License or Certification: * Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling). * If licensure is required for one's discipline within the state, individual must hold an active license. * Must meet eligibility requirements for CCM or ACM certification upon entry into this position OR within two years of entry into the position. * CCM or ACM certification required OR must be obtained within two years of being placed in the Case Manager II position. * Minimum Qualifications: * For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. Embrace Your Role as a Speech Pathologist Your impactful journey involves: Evaluating, assessing, developing, and administering personalized treatment plans for patients with speech and language rehabilitation needs. Identifying issues and modifying speech therapy treatment if necessary. Tracking and documenting patient performance, progress, and response to treatment. Celebrating patient victories along the way. Qualifications Current licensure or certification required by state regulations. Successful completion of SLP Certification of Clinical Competence (CCC). CPR certification required or must be obtained within 30 days of hire. Master's degree preferred, or Bachelor's degree with field experience.
