Assists patients in the process of navigating post-acute care with an overall goal of creating a positive impact on patient outcomes and referral source satisfaction. Integrates evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered, promoting quality and efficiency in the delivery of post-acute care. Represents the area branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities.
Qualifications
Education and experience, essential
Must be a graduate of an approved school of nursing, therapy or social work.
Must be licensed in the state where they currently practice.
Must have two years' demonstrated field experience.
Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice.
Must have basic demonstrated technology skills, including operation of a mobile device.
Education and experience, preferred
A registered nurse or physical therapist is preferred.
Three years of field experience is preferred.
Previous experience in home health or healthcare sales is preferred.
Requirements
Must possess a valid state driver license
Must maintain automobile liability insurance as required by law
Must maintain dependable transportation in good working condition
Must be able to safely drive an automobile in all types of weather conditions

Enhabit Home Health & Hospice is searching for a Registered Nurse (RN) or Physical Therapist (PT) to join our team as a Care Transition Coordinator.
Responsibilities include:
Assist patients in the process of navigating post-acute care.
Assess, plan, implement, coordinate, monitor, and evaluate options and services with a primary goal of providing a safe transition from acute care to home for home health or hospice services.
Integrate evidence-based clinical guidelines, preventative guidelines, protocols in development of transition plans that are patient-centered, promoting quality and efficiency in the delivery of post-acute care.
Promote adherence to post-acute plans and ensure ordered services are completed.
Represent Enhabit in transitional care activities and strategic relationships with health systems, hospitals, inpatient facilities, and physician groups.
Monitor execution of transitional care services through ongoing quality assurance visits with referral sources.
Meeting and/or exceed referral and admission goals.
Clinical liaison responsible for care transitions program admission activity for territory, while positively impacting patient outcomes and referral source satisfaction.
Qualifications
The right person for this role will be a Registered Nurse (RN), Occupational Therapist (OT) or Physical Therapist (PT) that is goal driven, sales motivated, and has previous home health or hospice experience.
Must be a graduate of an approved school of nursing or therapy and be licensed in the state of employment.
Must have a minimum of 2-3 years field experience.
Strong understanding of customer and market dynamics, as well as transitional care best practices.
Good understanding of the Federal, State, and local laws and regulatory guidelines governing home health and hospice operations.
Excellent communication skills and the ability to interact well with diverse individuals.
Experience with territory management, strong presentation skills, performance management, building relationships, emphasizing excellence, negotiation, results driven, sales planning and execution.
Should be self-starter who requires minimal supervision.
Previous experience in home health or healthcare sales is preferred.

Preferred Experience
Two years related experience working in a clinical healthcare setting or previous experience working in a clinical liaison role preferred.

**Key Responsibilities:**
+ You will work with physicians, discharge planners and patient's families to plan the best continuum of care after an ICU or post-surgical stay by utilizing your strong clinical background, payer knowledge and problem-solving skills.
+ Perform clinical assessments of referred patients, then counsel with patient's families to assist them in making the best care decisions for their loved one.
+ Partner with hospital case managers and physicians to create a smooth pre-admissions and transfer processes through removing payer-related and operational barriers for the patients getting into our highly effective level of care.

**What we're looking for in a Clinical Liaison**
**Required Skills:**
+ Registered Nurse (RN) or Respiratory Therapist (RT) License.
+ Will consider other designations like LPN, PT, OT etc., upon further review.
