CHA is a community-based distributed health system that includes the Cambridge, Everett, and Somerville Hospitals. The child and adolescent division provides outpatient care to youth and families in two outpatient clinics, three high school-based clinics, and via integrated care consultation in 11 primary care clinics. CHA is a primary teaching site for Harvard Medical School (HMS). Our system serves a culturally and ethnically diverse population; more than half of our patients (56%) identify as non-white and one quarter (25%) speak a primary language other than English. Our outpatient services include sub-specialty care in the assessment and treatment of both neurodevelopmental disorders and early psychosis.
Our child and adolescent psychiatry division serves children and adolescents with complex psychiatric and developmental conditions. Experience with assessing cognitive and socio-emotional functioning associated with severe mental illness (particularly complex trauma, psychosis, and clinical high risk), neurodevelopment, and neurological/medical complications from early childhood to early adulthood in culturally diverse community health settings is preferred. Skills in cognitive remediation are also preferred. In our culturally diverse community health setting, we value experience and skills in providing multilingual and culturally-sensitive neuropsychological services.
Duties include completing neuropsychological assessments, patient and family education, consultation with colleagues in other medical, neurological and mental health specialties. The role will also include occasional consultation to our inpatient service, which includes a neurodevelopmental unit, child unit, and adolescent unit.
In addition, our division combines advanced clinical care with academic engagement. The psychology division has a robust and vibrant training program that includes psychology practicum students, interns and postdoctoral fellows. The position includes teaching and supervising psychology interns and postdoctoral fellows and the faculty member holds an appointment at Harvard Medical School. Faculty are encouraged to engage in clinical research that is relevant to our community-oriented mission. We are seeking both early-career and mid-career applicants to provide a supportive and vibrant team experience. Neuropsychologists will join a vibrant team of adult and pediatric neuropsychologists in psychiatry for consultation and collaboration.

**Position Overview**
The **PACE Caseworker** plays a vital role in supporting participants and families by coordinating services in alignment with the PACE model of care. Working closely with the interdisciplinary team (IDT), the Caseworker helps develop, implement, and monitor participant care and discharge plans that reflect individual goals and team recommendations.
This position also serves as a navigator for new PACE enrollees and their families, helping them understand services, access concrete resources, and transition smoothly into the program. The Caseworker builds empathic, trusting relationships with participants, supports concrete service needs, and documents participant progress in the medical record.
**Key Responsibilities**
- Frame and implement interventions consistent with the PACE model of care and the participant's individualized plan of care
- Act as a participant and family navigator, particularly for new enrollees
- Provide psychosocial casework, referrals, and concrete services for participants and families dealing with medical, psychiatric, and substance use challenges
- Collaborate closely with the interdisciplinary team, including social workers and team leaders, to provide outreach, education, and basic counseling services
- Coordinate with community agencies, caregivers, and benefits/entitlement offices to ensure access to community-based supports
- Complete documentation, forms, statistics, and medical record entries accurately and in a timely manner
- Participate in activities of the CHA Department of Social Work
- Perform additional duties as assigned by the supervising Social Worker
**Qualifications**
- BS in related health field or relevant experience in lieu of a degree
- Demonstrated experience working in multicultural and multidisciplinary settings required. Experience working with relevant community groups preferred
- 1 year experience working with the geriatric population preferred
- At least 2 years experience with Care Coordination, Discharge Planning, Utilization Review, or similar preferred
- Bicultural and/or bilingual in the specific cultural/language group of the population/community served preferred. Fluency in Haitian Creole strongly preferred.
- Reliable transportation for home visits
- Flexibility, initiative, creativity, and excellent organizational skills
