* Perform only assigned tasks as instructed by the Supervisor, as applicable and where permitted by state law; (Assigned tasks are those tasks (permitted by state law or regulation if applicable) for which the Personal Care Aide has received training and Interim HealthCare has accepted and verified as appropriate).
* Utilizes infection control measures such as hand washing and personal protective equipment.

Responsibilities:
* Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required
* Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change

Providing assistance and hygiene care as directed, including but not limited to: bathing, grooming, oral care, dressing, undressing, toileting activities and assisting with measurements, if necessary
Providing assistance with daily tasks, including but not limited to: feeding, meal preparation, linen changes, light housekeeping, providing transportation to doctor appointments, outdoor activities, and shopping

Requirements:
High school diploma or GED
Successful completion of a Home Health Aide Program
Minimum of one (1) year of documented health care experience (unless participating in an approved Mentor Program
Certified CPR and negative TB skin or chest x-ray
Licensed driver with an insured automobile
Self-directing with the ability to work with little direct supervision
Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds.

Duties Responsibilities
The Physical Therapist / PT is responsible for the assessment of referred patients that require rehabilitation services which includes; providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Physical Therapist / PT will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Physical Therapist / PT must set realistic and achievable goals for their patients, document and record the patient's condition, and educate patients and families in an appropriate physical therapy method.
Develops a plan of care for each physical therapy patient.
Provides skilled physical therapy services / interventions in accordance with physician orders.
Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
Collaborates with all disciplines to plan and evaluate team goals for each patient.
