Responsibilities:
* Responsible for assisting patient with and per plan of care, the following;
* Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc
* Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)
* Safe transfer and ambulation techniques
* Exercise according to the plan of care
* Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)
* Patient's laundry
* Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)
* Observation and teaching of home safety practices
* Recognition of an emergency situation and knowledge of emergency management procedures
* Must follow patients plan of care and complete all tasks assigned
* Communicates in a constant, clear, and effective way with the client, family/caregiver and office
* Observes and documents patient condition and reports any changes, to the assigned nurse supervisor
Requirements:
* Meets all training requirements in accordance with state and federal laws when applicable
* Ability to read, follow written instructions and document care given
* Possess and maintains current CPR certification in accordance with state regulation
* PPD or chest x-ray if applicable
* Successful completion of Maxim screening examinations
* Participation and completion of Maxim's competency program
* Must meet all federal, state, and local requirements

Job Responsibilities
Provide client-facing care as assigned
Provide assistance with daily tasks as directed in the client's personalized plan of care, including but not limited to: feeding, meal preparation, linen changes, light housekeeping, providing transportation to doctor appointments, outdoor activities, and shopping

High school diploma or GED

Certified CPR and negative TB skin test or chest x-ray
Must have a valid driver's license and auto insurance (proof of insurance is required)
Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds
Self-directing with the ability to work with little direct supervision

Responsibilities
* Provide nursing care for student(s) with medical needs (Trach, vent, G-tube Feedings, seizure monitoring with rescue medications, personal care, Diabetic care, etc.),
