SocialWorker - Home Health Responsibilities: Job Summary: SocialWorkers are responsible for providing necessary support for each patient referred to their discipline according to health status, diagnosis, and patient and family specific needs. Hospital socialworkers advocate and help patients understand and navigate complex systems. With compassion and cultural awareness, they empower individuals to make informed decisions during challenging times.
* Collaboratively works with interdisciplinary teams to ensure completion of patient specific care plan and post-acute needs.
* Effectively collaborates with community resources to align patients with available resources. Assists with referrals for community-based resources.
* Completes required governmental assessments and level of care documents.
* Provides counseling services to patients and families when appropriate.
* Assists with various populations across the organization to ensure proper resources and support including but not limited to APS, CPS, DSS, disability based on identified patient safety and needs. Assists indigent population with needed resources to aid in prevention of readmissions and support quality in patient outcomes.
* Consistently handles customer complaints promptly and effectively when problems encountered. Utilizes proper escalation procedures when appropriate.
* Performs all other duties as assigned.
Work Schedule: 80 Hours Biweekly. Full Time. This position will cover Florence, Clarendon, Lee and Sumter counties.

A Brief Overview
Accountable for providing medical social casework to a wide array of patients ranging from newborns to geriatrics. A Brief Overview

Conducts individual, marital, family and group therapies as necessary. Coordinate care conferences as needed. Serves as the social work representative in multi-disciplinary team approach to patient care.
Utilizes research findings to improve outcomes and support, and provide quality of life for patients and their families. Works collaboratively with Case Management in providing coordinated services, which facilitates continuity of care for our patients.

Screens patient using high risk indicators but not limited to frequent hospitalization, inadequate support systems, readmissions, homelessness, abuse, neglect, exploitation and domestic violence.
Assesses for immediate discharge planning needs and develops and coordinates a discharge plan as determined by the patient's medical condition, level of independence, functional capacity, family/social support systems, and financial resources.
Assesses for long term and/or future patient care needs by identifying probable changes in level of independence or functional capacity in collaboration with patient/family and interdisciplinary care team. Provides information and education on community resources to empower patient/family decision making.
Communicates discharge plan to patient/family and interdisciplinary care team by timely documentation in the medical record, active participation in discharge rounds, and/or verbal dialogue as appropriate. Collaborates with patient/family to develop the discharge plan.
Documentation in the medical record is legible and conforms to hospital department policies and procedures including but not limited to screening, initial discharge plan, weekly status updates and whenever changes in the plan occur.
Assesses the patient's behavioral health needs in collaboration with physician, psychiatrists, and other members of the mental health team.
Provides counseling, crisis intervention, and psychosocial assessments to patients/families with appropriate medical staff supervision.
Collaborates with internal and external resources to develop a short term and long-term plan of care as appropriate.
Assumes leadership role in facilitating meetings and comprehensive care planning with interdisciplinary team when necessary.
Provide clinical oversight for graduate level MSW students.
Collaborates with the department's leadership team in assessing department needs and designing systems to meet department and organizational strategic initiatives.
Performs other duties as assigned.

This position has the primary responsibility of providing the full breadth of Child and Youth Behavioral (CYB) counseling services to children of all ages of military service members and their families at military installations to include civilian expeditionary work force and their families (when activated). CYB counselors will be assigned to child development centers, youth programs, summer camps, and on demand assignments. Provides non-medical, short-term, solution focused counseling directly to eligible children and youth of service members at assigned primary and secondary locations. Services include assessment, brief counseling and consultation, action planning, referral to resources (assuring linkage as appropriate), and follow-up as indicated.
CYB counselors will also have a focus on supporting the staff and personnel of Child Development Centers (CDC), youth programs and summer camps (if available/assigned) as well as providing parent support, training, and guidance.
Provides training and health and wellness presentations, participate in health fairs and other base/installation activities focused on children and youth.
Enters counselor activity data daily through smart phone or web application to assure that reporting is accurate from assigned installation while maintaining confidentiality and anonymity of service/family member.
Establishes and maintains working relationships with community resources focused on children and youth and provides appropriate linkages. Consistently visible within the assigned location(s) which allows MFLC services to be accessible. When working with children, counselor must abide by line-of-sight protocol. Communicates with Regional Supervisors and participates in regular individual and group supervision, sharing information regarding trends and issues on the installations and in facilities to which the counselor is assigned.
