The Pharmacy Department utilizes a multidisciplinary approach to patient care, where the emphasis is on therapeutic efficacy and safety. We provide comprehensive, clinical and dispensing services in both inpatient and ambulatory care settings through a decentralized satellite pharmacy services model.
We are looking for a confident, independent individual to join our Lymphoma Pharmacy Specialist team. In this role, you will serve as a fully integrated member of the inpatient multiple myeloma service and ambulatory clinic at our main campus to provide advanced clinical pharmacy services, including:
* Drug therapy design and monitoring, patient medication counseling, and participation in Collaborative Drug Therapy Management (CDTM)
* Committee participation and development of treatment guidelines and pharmacy policies
* Preceptor-ship of PGY2 Pharmacy residents and pharmacy students
* Education of hospital staff through Grand Rounds presentations and in-service training
Key Qualifications:
* Doctorate in Pharmacy, Oncology experience preferred
* NYS license (or eligibility)
* Completion of an ASHP-accredited pharmacy practice residency and an ASHP-accredited oncology residency, or equivalent experience are required.
Additional Information:
* Regular hours of work fall between Monday - Sunday, 37.5 hours per week

The Emergency Department (ED) Clinical Pharmacist is responsible for the provision of comprehensive clinical pharmacy services for the ED and all associated areas, including the pediatric ED (Hopewell campus) and trauma bay (Regional Medical Center). The ED Pharmacist works in collaboration with Pharmacy staff to promote a safe and cost-effective medication management process through policy/protocol development and liaison with medical, nursing, and ancillary providers.
MINIMUM REQUIREMENTS
Education: Graduate of an ACPE-accredited school of pharmacy. Doctor of Pharmacy (Pharm D). Board of Pharmacy Specialties (BPS) certification preferred. ASHP accredited PGY-2 Pharmacy Emergency Medicine or Critical Care Residency certification preferred.
Experience: ASHP accredited PGY-1 Pharmacy Residency or three years equivalent experience preferred. ED/Critical Care experience preferred.

Identifies and resolves problems with medication orders or therapeutic regimens. Clarifies issues with prescribers and suggests order adjustments or alternative medication therapies when needed to prevent adverse events or adheres to institutional policies.
Provides concise, applicable, comprehensive, and timely responses to requests for drug information from patients and healthcare providers. Supervises and coordinates pharmacy technician work processes as appropriate for patient care assignments. Ensures proper drug utilization by adherence to formulary criteria and pharmacy policies. Supports clinical pathways and quality assurance programs.

Performs prospective and retrospective drug utilization reviews (DURs); discusses the recommended interventions with the patient and the patient's Primary Care Provider (PCP).

Provides ongoing clinical and disease management trainings to HJA staff and patients that address common diseases such as diabetes, hypertension, hyperlipidemia, and asthma.
Assists with the development and implementation of collaborative practice agreements for management of chronic disease states and collaborates with HJA and external health care professionals to plan, monitor, review, and evaluate patient care.
Analyzes prescribing trends to monitor patient compliance and to prevent excessive usage or harmful interactions.
Dispenses and reviews prescriptions to ensure accuracy, and compliance with professional, state and federal regulatory requirements.
Participate in HJAHC committees as they relate to clinical pharmacy activities.
Fulfills his/her obligation to contribute to the general knowledge that advances health and quality of life through attendance at conferences, development and delivery of post and platform presentations, and/or publication in reputable, peer-reviewed journals.

REQUIREMENTS:
EDUCATION & EXPERIENCE:
Graduate of an ACPE-accredited school of pharmacy with a B.S. or PharmD degree required
Successful completion of ACPE-accredited PGY1 residency or equivalent experience required

New Jersey Board of Pharmacy Pre-Approval to engage in collaborative practice agreement preferred
LICENSURE AND/OR CERTIFICATIONS:
