Essential Responsibilities:
Resident Support & Care
• Assist residents in accordance with individualized care plans, including life skills such as grooming, hygiene, time management, housekeeping, personal safety, and basic emergency response skills
• Promote resident independence, dignity, and respectful communication at all times
Health, Safety & Transportation
• Ensure the safety of residents and the apartment environment to minimize risk and prevent accidents
• Provide transportation to medical appointments, recreational programs, shopping, community activities and work
• Administer medications as assigned, when need coverage, in accordance with agency policy and training.
Documentation & Compliance
• Maintain accurate, timely, and professional documentation in program communication logs and resident files

Requirements
Qualifications:
• High school diploma or GED

Job Description Elwyn Adult Behavioral Health Services seeks a Recovery Support Worker (RSW) to build relationships with individuals and develop shared understandings of the individuals' goals and the steps necessary to achieve the goals.
Up to $21/hr, negotiable ESSENTIAL FUNCTIONS: Reviews the Screening, Assessment, Crisis Plan, Critical Needs Plan and IAP upon completion or update completed by the LPHA.
In conjunction with the Integrated Team/GLE/IGLE staff, assists persons with identifying and implementing meaningful goals for their Individual Action Plan focusing on the following areas: mental health recovery education, daily living skills, substance use, relationships, independent transportation, self-help groups, exercise/wellness groups and the Recovery Learning Center (RLC), vocational & leisure.
Demonstrates knowledge of prescribed medications, ability to monitor medications, and follows FHR state specific medication guidelines.

Responsibilities:
Patient care - Instill confidence and trust in patients in a competent and caring manner. Provide hygiene and daily living assistance. This could include toileting, bathing, and assisting patients with showers. Support the patient with meals as needed. Ambulate, turn, and position patients as required.
Medical needs - Check vital signs and weight. Perform sugar and acetone urine testing and specimen collection. Administer non-sterile dressings, surgical preps, ice packs, heat treatments.
Professional development - Maintain any specialized training and, if applicable, perform additional and specialized tasks pertinent to the assigned patient or area.
Requirements:
Successful completion of a nursing assistant course or the equivalent
High school diploma or equivalent is required
Previous hospital experience with acceptable references preferred
Excellent customer service skills
Prolonged periods of standing and walking
Must be able to lift 75 pounds at time
