Midland Care's Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities. Employment Type: Full Time | M-F 8am-5pm Location: Topeka, KS We are seeking a dedicated and compassionate Physical Therapist to join our Program of All-Inclusive Care for the Elderly (PACE) team. In this full-time role, you will work closely with an interdisciplinary team to provide rehabilitative services to frail elderly participants, helping them maintain independence and improve quality of life. Current unrestricted Kansas State license to practice as a physical therapist. Must possess a valid Kansas Driver's License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization's insurability standard as defined by Midland policy. Availability of reliable transportation is required for this position. One year of professional experience required, minimum one year experience working with the frail and elderly population, home care or geriatric experience. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity. Physical Therapist Works as part of an interdisciplinary team to help older adults maintain and improve their mobility, strength, balance, and overall physical function so they can remain safe and independent in their homes and communities. They assess each participant's physical abilities, develop individualized treatment plans, provide therapeutic exercises and mobility training, monitor progress, adjust care based on needs, and educate participants and caregivers on safe movement, fall prevention, and appropriate home strategies. The PT also collaborates with doctors, nurses, occupational therapists, and other team members to coordinate care and support quality of life for frail elderly participants enrolled in the PACE program. Qualifications Licensed Physical Therapist (PT) in the state of Kansas Graduation from an accredited physical therapy program Experience working with older adults or geriatric populations Knowledge of rehabilitation principles, mobility training, and fall prevention Ability to perform physical assessments and develop individualized treatment plans Strong communication and documentation skills Ability to work effectively as part of an interdisciplinary healthcare team CPR certification (or ability to obtain) Preferred Experience in a PACE program, home health, long-term care, or skilled nursing setting Familiarity with chronic disease management and age-related conditions Experience providing caregiver education and patient training Knowledge of adaptive equipment and assistive devices Strong organizational and time-management skills Apply today! OTs recommend adaptive equipment, home modifications, and safety strategies to prevent falls and injuries, while also addressing cognitive and fine-motor challenges.
