The Hospital Prior Authorization Specialist is responsible for coordinating and securing prior authorizations for inpatient and outpatient hospital services in accordance with payer requirements and regulatory standards. A Brief Overview Responsible for precertification of eligible prescriptions. Ensures complete documentation is obtained that meets insurer guidelines for medical necessity and payment for services. What you will do Responsible for precertification of eligible prescription medications for inpatient and outpatient services based on medical plan documents and medical necessity. Ensures medical documentation is sufficient to meet insurer guidelines for medical necessity documentation and procedure payment. Reviews clinical information submitted by medical providers to evaluate the necessity, appropriateness and efficiency of the use of prescription medications. Assists with patient assistance and grant coordination for Patients for outpatient pharmacies from designated areas. Proactively analyzes information submitted by providers to make timely medical necessity review determinations based on appropriate criteria and standards guidelines. Verifies physician orders are accurate. Determines CPT, HCPCS and ICD-10 codes for proper Prior Authorization. Contacts insurance companies and third party administrators to gather information and organize work-flow based on the requested procedure. Collects, reads and interprets medical documentation to determine if the appropriate clinical information has been provided for insurance reimbursement and proper charge capture. Serves as primary contact with physicians/physician offices to collect clinical documentation consistent with insurer reimbursement guidelines. Establishes and maintains rapport with providers as well as ongoing education of providers concerning protocols for pre-certification. Communicates information and acts as a resource to Patient Access, Case Management, and others in regard to contract guidelines and pre-certification requirements. Performs research regarding denials or problematic accounts as necessary. Works to identify trends and root cause of issues and recommend resolutions for future processes. Lexington Regional Health Center Title: Staff Pharmacist Supervisor: Director of Pharmacy Services Department: Pharmacy FLSA Status: Exempt Principle duties and responsibilities Provides drug information and/or patient oriented services to patients (newborn, young adult, adult and geriatric), health care providers and other health care professionals. Implements and follows policies and procedures that promote the effective, appropriate and safe use of drugs in all patient care activities in the hospital. A med cart is used to distribute medications to the hospital, requiring up to 38 pounds of horizontal force to push over thresholds into rooms and 25 pounds of horizontal force in the hallways. Med cart is used up to 200 feet in distance from pharmacy area for distribution. If required to transfer a patient for better positioning, 35 pounds is required to be lifted for transferring. Frequently walks distance of 500 feet to clinic location to transport small quantities of medications and check refrigerator/freezer temperatures. Exposure to chemicals and other hazards related to patient care. Appropriate safety precautions, such as safety glasses, gloves, etc. must be used to minimize risk of injury. Direct exposure to body substances during patient care activities and on contaminated surfaces such as patient equipment. Hours are between Monday to Friday, six or eight hour shifts start time is 8AM. Weekend hours are 8AM to 12PM. On call is required._** **Primary Function/General Purpose of Position** The Pharmacist performs medication management activities to ensure safe, rational, and cost-effective therapy while maintaining compliance with all applicable rules, regulations, and standards as promulgated by Federal, State, and accrediting agencies or regulating bodies. The Pharmacist maintains established departmental policies and procedures and complies with department objectives. **Licensing/Certification** Maintains current license for pharmacist in the state(s) where practicing (required) **Education** Graduate of an ACPE accredited School of Pharmacy with a BS or PharmD (required) Advanced pharmacy degree - Doctor of Pharmacy, Masters, Residency (preferred) **Work Experience** Experience in field of pharmacy practice (required) One year of experience in field of pharmacy practice (preferred) **Training** Exhibit proficient use of all computer hardware and software (required) **Essential Job Functions** + _ _ Process orders and provide medication review for appropriateness, safety and efficacy during order processing and perform any necessary clinical support duties as needed + Ensure accurate preparation of compounded and non-compounded products to include safe handling that is consistent with regulations + Oversee distributive functions of the pharmacy department to facilitate accurate and timely medication availability to patients/staff + Oversee pharmacy technicians and support personnel activities based on department need + Maintain responsibility and accountability for the security of pharmacy inventory based on department need + Identify, document, and resolve potential and actual adverse medication events and may evaluate reports in event reporting system for trending and prevention purposes + Perform medication education to improve patient understanding and adherence consistent with department guidance + Work collaboratively and effectively to communicate verbally and in writing with pharmacy team members, healthcare providers, patients, caregivers, and leaders + Participate in departmental medication safety, quality assurance, drug utilization reviews, drug dosing, infection prevention, student precepting and other activities based on department need + Must be able to travel between locations, as needed.
