DUTIES AND RESPONSIBILITIES:
Collects all discharged patient charts from the patient care units on a daily basis.
Collects loose filling for those discharged charts and inserts such filing in proper charts.
Assembles discharged patient charts according to assembly list.
Places assembled chart in proper folder according to medical record number.
Analyzes medical records of discharged patients for completeness and accuracy according to department and hospital policies and procedures.
Assists in retrieving charts for physician completion.
Answers incoming calls to the department in a professional, timely manner.
Forwards all analyzed charts to the coder.
Pulls delinquency charts according to deficiency list.
Files or scans all charts back to main file.
Makes and attaches medical record numbers for new charts.
Communicates appropriately and clearly to the supervisor, nurse managers, co-workers and physicians.
Handles correspondence from insurance companies and attorneys. Obtaining requested medical records to fill those requests.
Completes annual education requirements.
Must be able to keep up with work load be able to process a minimum of 25 charts per day.
Reports to work on time and as scheduled, completes work within designated time. Reviews charts thoroughly to ascertain all diagnosis/procedures.
Performs quality review on records scanned in Evident.
Reconciles master list with the computer generated in house list on a daily basis. Must check admit and discharge dates for accuracy in the mainframe computer.
Requests charts daily from storage for next day procedures.
Files and clears all charts in the dictation room by the end of shift on a daily basis.
Responsible for knowing hospital policies and being familiar with hospital services that are made.
Demonstrates an ability to be flexible, organized and function well in stressful situations.
Treats patients and their families with respect and dignity; ensures confidentiality of patients' records.
Maintains a good working relationship both within the department and with other departments.
Participates in educational programs, in-service meetings and completes annual educational requirements.
Attends all other meetings as required.
Maintains patient confidentiality at all times.
Accepts and completes other duties as assigned or requested.
Retrieves transcribed reports check for errors on transcription notifies company if error is found, has transcription record fixed when updated.
Assured history and physical is on each patients chart prior to surgery.
Assures operative reports are dictated immediately after surgery.
Contacts physician for delinquent reports, if necessary.
Processes physician and other department staff requests in a timely manner.
Other duties as assigned.

JOB SUMMARY: The Technologist Assistant is responsible for assisting the Radiologic Technologist with patients during the imaging process. The assistant will be one of the first contacts for patients, physicians, and other medical staff.
DUTIES AND RESPONSIBILITIES:
The Technologist Assistant:
1. In a professional and friendly manner, answers incoming phone calls, facilitates scheduling of exams, dispenses exam preparations and
instructions, and responds to general imaging questions.
2. Assist Technologist with proper and comprehensive patient screening prior to examination.
3. Assists Technologist with having patients change into gowns (when necessary), screen patient and document, transport patients, and answer
questions and prepare the patient for imaging procedures to make sure the patient is comfortable and at ease during the imaging process.
4. Reviews physician orders and verifies we have all previous images used for comparison.
5. Assists in accepting and processing incoming images from other facilities to use as comparison.
6. Assists with documentation for appropriate patient information necessary for exam interpretation and patient care.
7. Assists the schedulers, technologists, and other coworkers in the department as needed.
8. Promotes effective working relationships and works effectively as a part of the department/team to facilitate the hospital and department's ability to meet its goals and objectives.
9. Troubleshoots technical problems and works with the vendors to resolve problems that arise.
10. Maintains flexibility as department grows with willingness to accept additional responsibilities requested by department director.
11. Acts professional in every situation: patient, co-worker or intradepartmental.
12. Recognizes that many people we see are at a low point in their lives and will show compassion for them even if it is only a smile.
13. Protects information that is sensitive, proprietary or confidential in nature. Adheres to Patient Rights Policy and patient confidentiality standards.

Qualifications
QUALIFICATIONS:
Education: High School graduate or equivalent. Must hold or obtain current BLS.
Experience: 1-3 months experience preferred, but not required.
Language/Communication: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to speak effectively, before groups of customers and or employees of the organization.
Mathematical Skills: Basic

DUTIES AND RESPONSIBILITIES:
1. Accurately room patients; take an adequate patient history; obtain accurate vital signs, collecting urine and blood specimen, ensuring proper patient identification and labeling of samples, and accurately enter all information into Electronic Medical Record (EMR) in a timely manner.
2. Accurately and timely assists physician or provider during medical exam and procedure and prepare test and prescription requisitions according to physician orders as requested.
3. Proficient in clerical responsibilities including: sending/receiving/retaining patient medical records; obtaining lab/x-ray reports, hospital notes, referral information, etc.; completing forms/requisitions as needed; scheduling appointments; verifying insurance coverage and patient demographics; managing charts to ensure information completed and filed appropriately; obtain pre-authorization and healthy connection referrals.
4. Accurately check schedules and organize patient flow; perform screenings per provider guidelines; relaying instructions to patients; answering calls and providing pertinent information and documenting conversations.
5. Performs facility responsibilities as assigned which may include: setting up equipment according to department protocol; cleaning exam/procedure rooms, follow standard precautions using personal protective equipment, cleaning/sterilizing equipment between patient visits to maintain infection control, rooms stocked with medical supplies and keep inventory according to departments acceptable par level.
6. Assists with referral management and clinic and surgery scheduling.
7. Interacts professionally with patient/family and involves patient/family in the formation of the plan of care. Treats patients and their families with respect and dignity and serves as a liaison between administration, patients, physicians and other healthcare providers.
8. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

DUTIES AND RESPONSIBILITIES:
1. Fulfills patient care responsibilities as assigned which may include the following: Checking schedules and organizing patient flow; accompanying patients to exam/procedure room; assisting patients as needed with walking, transfers, collecting specimens, preparing for exam, etc.; collecting patient history; performing screenings per provider guidelines; assisting physician with various procedures; charting; relaying instructions to patients; answering calls and providing pertinent information.

3. Follows hospital policies and procedures for transporting and delivering samples to laboratory. Knowledge of handling laboratory supplies and stocks supplies.
4. Ensure that patients receive appropriate competent, and quality clinical care and that customer service meets patient's expectation.

12. Completes CCM Roster on a monthly basis.
13. Stay current on CCM changes and requirements from CMS.
14. Act as a liaison between our provider and the patient when coordinating community resources, access to specialty services, follow-up healthcare, etc.
15. Follow-up and help complete identified referrals. Coordinate with specialists and help to integrate recommendations from our provider. Identify barriers for lack of follow through or potential barriers to community resources.
16. Other duties as assigned.

Qualifications
Education/Certification: High School diploma. Prefer graduation of a post-secondary medicalassisting program accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) or the Accrediting Bureau of Health Education Schools (ABHES) and/or 6-12 month clinical experience. BLS certification within two weeks of hire.
