FTE: 1.0, Shift: Day, Schedule: Mon - Fri Position Summary The Director of Case Management is responsible for leading a dynamic team of clinical professionals in the execution of exemplary cross continuum care coordination. This position is responsible for the development and implementation of the case management process; coordinating and supervising the duties of the clinical case managers and social workers; promoting quality patient care outcomes while supporting appropriate utilization/resources management along the continuum of care; and facilitating the multidisciplinary case management process by coordinating the development and implementation of tools and systems for effective, efficient case management. This individual works with market leaders, physicians, peers, community colleagues, system leaders and others to identify opportunities to continually improve patient care and services, and to execute collaborative practices that exemplify MultiCare Health System's commitment to patient centered care and community engagement. Hospital-based leaders may have a dual matrixed reporting structure. Follows quality assurance guidelines and maintains accurate records for Washington State regulatory requirements. 9. Understands and follows all safety policies and reports any unsafe conditions or practices to the Supervisor/Manager, Administration or Safety Coordinator. 10. Assumes responsibility of self and others for proper and efficient operation of all equipment used. Report equipment problems, malfunctions, and/or need for repairs to supervisor. Maintain cleanliness of equipment for patient safety and/or function. 11. Obtains, maintains, and utilizes knowledge as is necessary for efficient and effective operational utilization of required computer systems. 12. Evaluates and maintains an appropriate level of responsibility for updating job related knowledge and skills level, including applicable Joint Commission requirements, in an effort to increase individual skills and quality patient care. 13.
