Job Description:
Essential Functions:
Participates with the interdisciplinary team in specialized patient care. Cares for patient assignment as delegated.
Effectively establishes therapeutic relationships with patients and families on the unit including but not limited to effective communication and education.
Provides trauma-informed care, utilizes positive reinforcement and operates within a therapeutic relationship, maintains staff and patient safety through de-escalation, limit setting, and physical interventions
Collaborates with other healthcare professionals to develop treatment plans.
Acts as a resource to unit staff regarding diagnoses, behavior plans, relationship based care, protective equipment, crisis management.
Education Requirement:
High School Diploma or equivalent, required.
Current enrollment in and completion of half a Psychology, Social Work, or related undergraduate program, or equivalent experience, required.
Certifications:
Basic Life Support (BLS) certification from the American Heart Association required prior to finishing unit-based orientation (due within 90 days of hire), must maintain certification.
Therapeutic Crisis Intervention (TCI) certification provided by the organization, required prior to finishing unit-based orientation (due within 90 days of hire), and maintain certification.
First Aid certification by the organization, required prior to finishing unit-based orientation (due within 90 days of hire), and maintain certification.
Skills:
Effective written and verbal communication, interpersonal and problem-solving skills.
Experience:
One year of hospital experience in a mentalhealth role within the past three years, or equivalent education, required.

Behavioral Health Specialist, Community Case Manager

This position requires meeting clients in their homes, community, schools, etc. In addition, transporting clients in their own vehicle to/from appointments, school, community events, etc. Community Psychiatric Support Treatment (CPST) and Therapeutic Behavioral Services (TBS) provide services to children and adolescents with mental illness within all settings that meet the needs of the individual. These services may involve collateral contacts. CPST and TBS services provides an array of services delivered by community based providers. Services address the individualized mentalhealth needs of the client which vary in hours, type, and intensity of services. CPST/TBS services are designed to provide specific, measurable, and individualized services to each client served.
Activities of the service shall consist of one or more of the following:
Ongoing assessment of needs;
Assistance in achieving personal independence in managing basic needs as identified by the individual and/or parent or guardian;
Facilitation of further development of daily living skills, if identified by the individual and/or parent or guardian;
Assistance with accessing natural support systems in the community; and linkages to formal community service/systems;
Symptom monitoring;
Coordination and/or assistance in crisis management and stabilization as needed;
Advocacy and outreach;
Mentalhealth interventions that address symptoms, behaviors, thought processes, etc., that assist an individual in eliminating barriers to seeking or maintaining education and employment.
REPRESENTATIVE RESPONSIBILITIES
·Patient/Family Interaction and Education
Provides therapeutic intervention to assigned caseload of patients based upon an Individualized Treatment Plan (ITP) developed with the families, at the frequency specified in the ITP, under the supervision of the Clinical Manager. Critical thinking skills are employed in implementing these therapeutic interventions, which are provided in the patients home, school and other natural environments in the community as appropriate. Transports patient and/or caregiver in personal automobile (after conducting a safety assessment) at times to provide individualized supportive services and to help ensure attendance at all appointments. Provides coaching and psychoeducation to families in a culturally competent manner, including safety plans as indicated. Consistently integrates age specific and culturally diverse concepts into patient care, taking into consideration both the patient's chronological age and developmental functioning. Recognizes the effects that trauma may have on the patient and family. Utilizes Situation Awareness concepts as appropriate. Embraces relationship-based care.
·Interdisciplinary Teaming
Participate in regular team planning meetings as appropriate or needed. Assist the multidisciplinary team during patient care, and/or parent and staff training related to individual behavioral and education programs, data collection, and environmental modifications to promote patient success. Implement discipline specific interventions after discipline modeling and treatment integrity checks.
·Behavioral Intervention/Clinical Activity
Function as a member of the outpatient behavioral intervention program to assess behavioral function and implement behavioral and educational protocols. Implement behavior assessment protocols as designed by psychology staff. Provide insight into antecedent events and consequences that may affect behavioral treatment. Implement individualized and/or group behavior and education intervention plans as prescribed. Utilize safe management and crisis intervention techniques as appropriate. Participate in problem-solving of behavioral-medical treatment options. Participate in team staffing of children.
·Clinical Documentation
Documents accurately and timely the therapeutic interventions provided and response of patients/families/significant adults in the electronic medical record (Epic) per state requirements and established quality standards. Apply appropriate policies and procedures for work flow within Epic, including accurately implementing Epic Prelude, Epic Cadence and Epic Ambulatory.

Job Responsibilities
* Milieu Management: Create a safe, stable, and nurturing environment that fosters support and consistency. Implement a token economy to reinforce positive behaviors. Support patients with a variety of severe psychological/psychiatric difficulties, including mood disorders, disruptive/aggressive behavior, self-harm, anxiety, psychosis, eating disorders, intellectual disability and neurodevelopmental disorders who often have histories including significant psychosocial stressors and trauma. Proactively prevent and respond to crises using Therapeutic Crisis Intervention (TCI) strategies, including verbal de-escalation and physical interventions when necessary due to imminent risk. Lead and intervene in behavioral crises, apply appropriate strategies based on the Stress Model of Crisis and individualized treatment plans. Conduct safety checks, room searches, and patient safety searches. Follow policies and collaborate with the interdisciplinary team.
* Plan of Care: Implement individualized care plans, including behavioral interventions and support for activities of daily living (bathing, feeding, toileting). Provide care primarily in group settings, with individualized programming as needed per treatment team recommendations. Maintain designated observation status (e.g., continuous observation, 15-minute checks, remaining within arm's-length of patient).
* Group Programming: Lead structured group programming and facilitate patient engagement; adjust delivery to meet developmental needs of patients, observe behavior and participate in problem-solving
* Therapeutic Relationships: Build rapport while maintaining professional boundaries. Recognize trauma impacts and use developmentally appropriate therapeutic communication. Contribute to team planning and patient-centered care discussions.
* Documentation: Complete accurate and timely documentation, including shift notes, incident documentation (e.g., restraint, seclusion), and safety checks. Summarize what happens before and after target behaviors. Actively participate in shift reports, safe handoff procedures, and data collection.

This position requires meeting clients in their homes, community, schools, etc. In addition, transporting clients in their own vehicle to/from appointments, school, community events, etc. Community Psychiatric Support Treatment (CPST) and Therapeutic Behavioral Services (TBS) provide services to children and adolescents with mental illness within all settings that meet the needs of the individual. These services may involve collateral contacts. CPST and TBS services provides an array of services delivered by community based providers. Services address the individualized mentalhealth needs of the client which vary in hours, type, and intensity of services. CPST/TBS services are designed to provide specific, measurable, and individualized services to each client served.
Activities of the service shall consist of one or more of the following:
* Ongoing assessment of needs;
* Assistance in achieving personal independence in managing basic needs as identified by the individual and/or parent or guardian;
* Facilitation of further development of daily living skills, if identified by the individual and/or parent or guardian;
* Assistance with accessing natural support systems in the community; and linkages to formal community service/systems;
* Symptom monitoring;
* Coordination and/or assistance in crisis management and stabilization as needed;
* Advocacy and outreach;
* Mentalhealth interventions that address symptoms, behaviors, thought processes, etc., that assist an individual in eliminating barriers to seeking or maintaining education and employment.
REPRESENTATIVE RESPONSIBILITIES
* Patient/Family Interaction and Education
Provides therapeutic intervention to assigned caseload of patients based upon an Individualized Treatment Plan (ITP) developed with the families, at the frequency specified in the ITP, under the supervision of the Clinical Manager. Critical thinking skills are employed in implementing these therapeutic interventions, which are provided in the patients home, school and other natural environments in the community as appropriate. Transports patient and/or caregiver in personal automobile (after conducting a safety assessment) at times to provide individualized supportive services and to help ensure attendance at all appointments. Provides coaching and psychoeducation to families in a culturally competent manner, including safety plans as indicated. Consistently integrates age specific and culturally diverse concepts into patient care, taking into consideration both the patient's chronological age and developmental functioning. Recognizes the effects that trauma may have on the patient and family. Utilizes Situation Awareness concepts as appropriate. Embraces relationship-based care.

Schedule: 40hrs/week - 11am - 9pm
Valid Ohio driver's license and proof of auto insurance is required by hospital policy. Must pass a motor vehicle background inspection, insurance eligibility, driving qualifications and training set forth by Nationwide Children's Hospital and must maintain qualification of insurance guidelines.
Summary:
Facilitates crisis-oriented triage, assessment, and other appropriate mentalhealth screenings and interventions via face-to-face mobile response. Assists with the management of psychiatric or behavioral health concerns occurring within various settings, in collaboration with other NCH and community partners.
Job Description:
Essential Functions:
Collaborates to complete telephone consultation, screening, and triage processes that determine needs of those calling through the crisis hotline. Provides trauma-informed and culturally competent crisis intervention, risk assessment, de-escalation, collaborative safety planning, psychoeducation, and recommendations.
Recommends disposition for least restrictive level of care, utilizes case conceptualization, supervision and staffing as appropriate. Completes necessary assessments, documentation, follow up, and data collection. Remains updated related to required trainings, research, and literature in areas of expertise.
Collaborates to identify skills, supports, resources, and services that assist with stabilizing the crisis and preventing future crises from occurring.
Collaborates to further support throughout the stabilization and transition phases. Provides clinical direction and oversight for cases co-managed with non-licensed team members. Delivers a warm handoff to long-term behavioral health providers and supports, including completion of a transition plan.
Coordinates services for assigned families as defined by service goals. Provides care coordination and collaboration with current behavioral health providers, natural supports, and other systems involved with youth and family.
Initiates mandatory reporting of abuse or neglect to Child Protective Services or other applicable agencies per Ohio law.
Education Requirement:
BSW or higher, required.
Licensure Requirement:
LSW or higher, required.
Certifications:
Upon hire, must complete and pass NCH endorsed Crisis Management Training within 90 days.
Skills:
Demonstrated skills in providing behavioral health assessment and treatment to children and adolescents.
Demonstrated ability to exercise a degree of independence, initiative, judgment, and decision-making to prioritize workload and achieve program objectives.
Ability to work both independently and collaboratively as a team member to ensure consistent quality of services that support a high level of patient and provider satisfaction.
Demonstrated communication and interpersonal skills required to establish and maintain productive and collaborative team and multi-disciplinary relationships.
Demonstrated sensitivity, empathy, objectivity, professionalism, effective engagement and customer service skills, and sound clinical judgement in the course of all work duties.
Embraces principles of family centered care in which needs and perspectives of patients and families are acknowledged and honored.
Experience:
One year of providing crisis-based mentalhealth services with children and adolescents, preferred.
Previous experience facilitating crisis intervention services, preferred.

The Care Management Clinician plays a vital role in supporting youth and families engaged in the NCH Behavioral Health Intensive Care Management (ICM) program. This position is designed for a highly collaborative, clinically informed professional who is committed to improving outcomes for children and adolescents experiencing significant mentalhealth challenges.

Conducts assessments of patients' mentalhealth needs and develops individualized care plans. Monitors patients' progress and adjusts care plans as needed.
Coordinates client care with other service providers and collaborates with interdisciplinary team to ensure continuity of care.
Coordinates available NCH and community resources to enhance client care.
Provides education to patients, families, and care teams about mentalhealth conditions and treatment options.
Performs brief therapeutic interventions and counseling.
Completes clinical charting and other related administrative paperwork in accordance with policy standards.
Participates in quality improvement initiatives to improve patient outcomes.
Education Requirement:
Master's degree in Counseling, Social Work or Marriage and Family Therapy, required.
Licensure Requirement:
Appropriate Ohio licensure, required.Must remain current in required licensure and clinical or other training as required.
