POSITION SUMMARY: Under daily supervision of the Registered Nurse/Charge Nurse and directly to the Nurse Manager, the Patient Sitter observes the patient for any unusual behavior and assists in the prevention of any harm to the patient. JOB SPECIFIC RESPONSIBILITIES: Ensures that any change in patient condition or unusual circumstances is reported immediately to the charge nurse. Practices correct isolation techniques to maintain a safe environment and to control the spread of infection Maintains the cleanliness of various instruments and equipment per hospital policy. Keeps patient rooms in a clan and orderly condition; makes beds, replaces soiled linens, straightens beside table and prepares room for occupancy. Exchanges and communicates patient care activities and meets all patient needs within shift parameters Corrects audit deficiencies of medical records within allotted time frames. Resolves with assistance from charge nurse, areas of concern or conflict with patient families and/or co-workers in a professional, proactive and courteous manner. Accepts direction from supervisors and other management team members in a positive and professional manner. Situated on 26 acres, our campus features six resident homes, a 24,500 square foot state-of-the-art rehab facility, and 38 different programming areas. Our primary goal of targeted post-acute rehabilitation and therapy is to help our residents return to work, school, or home and lead productive lives through holistic treatment. JOB SPECIFIC RESPONSIBILITIES: Completes and records routine patient vital signs as per instructed by RN/LVN. Performs patient rounds as ordered, if unable to perform, primary nurse is notified. Knowledgeable of vital sign ranges and reports any deviation to the primary nurse immediately. Ensures that fluid/food intake and output are appropriately measured and recorded in patient charts every shift. Bathes patients as scheduled, if patient declines, nurse and program director are properly notified. Performs oral care at least once per shift and documents on patient's chart. Provides hair care at least once per shift and documents on patient's chart. JOB SPECIFIC RESPONSIBILITIES: • Understands and agrees with Nexus Neurorecovery Center's philosophy and goal to maintain, improve and/or enhance each resident's quality of care and quality of life. • Administers and accurately records the administration of medications for residents as prescribed by the physician following established policies and procedures and state/federal regulations • Observes, records, and reports to the Staff Nurse symptoms and conditions of residents that could be related to medication interactions, adverse drug reactions, or medication side effects • Determines and records effectiveness of medications in a timely and accurate manner as required by state and federal regulations • Provides and promotes resident's rights during the administration of medications • Stores medications safely and accurately under established policies and procedures and state/federal regulations • Monitors medications to ensure adequate accountability measures are taken when medications are ordered, received into the facility, administered, and at the change of shift as required by policies and procedures and applicable state/federal regulations • Routinely inspects, cleans, and monitors equipment and supplies to ensure resident safety • Reports any equipment or supply issues to the Staff Nurse for correction or repair • Obtains, records, and reports vital signs as indicated during the administration of medications • Demonstrates awareness of proper infection control practice by using Universal Precautions and following policy and procedure • Communicates to residents, staff, and others accurately and factually and seeks or refers questions to Staff Nurse when indicated • Performs other duties as assigned POSITION QUALIFICATIONS: EDUCATION: Minimum of high school degree or equivalent Completion of the one-year certificate program at an accredited school Bilingual preferred EXPERIENCE: One year experience in an Assisted Living facility preferred LICENSURE/CERTIFICATION: Current Certificate for Medication Aide from the National Council of State Boards of Nursing (NCSBN) Certified in AHA BLS and ABC 101
