The NursePractitioner (NP) performs a variety of advanced clinical duties involved in assessing, treating, controlling, and preventing patient illness and disease within the facility. The NP provides general medical care and treatment to facility patients/residents under the direction of the medical director. The NP, independently and/or in collaboration with the treatment team, participates in the observation, assessment, diagnosis, intervention, evaluation, rehabilitation, care and counsel, and health education of patients/residents.
Job Specific Responsibilities:
1. Ensures that services provided are consistent and within the guidelines of the Nursing Practice Act
2. Obtains detailed history and performs physical examinations on assigned patients; assesses and treats acute/chronic medical problems and provides direct patient care to a specified patient population
3. Performs emergency, life-saving procedures
4. Notifies attending physician on call, house physician on call, or supervising physician regarding any unusual or unexpected events or problems requiring direct physician consultation
5. Practices proper safety, environmental, and/or infection control methods
6. Writes progress notes on patients' charts indicating patient status and treatment procedures performed; maintains other records as required
7. Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops
8. Performs complete physical and neurological exam of patients/residents by means of interview, health history, physical examination, and diagnostic tests
9. Orders appropriate laboratory and diagnostic procedures; interprets results and intervenes appropriately
10. Prescribes and administers medications in compliance with the Texas Physician Assistant Practice Act requirements and Facility guidelines
11. Recommends and refers to institutional and community resources to meet patient and family needs
12. Collaborates with physicians and other health care providers (within the facility and in the community) in managing acute and long-term medical needs of patients
13. Provides monitoring and ensures continuity of care between physician visits
14. Provides 24-hour medical call coverage on a rotating schedule
15. Provides continuing education and serves as a resource to staff
16. Discusses case with physician and other health professionals
17. Submits health care plan and goals of individual patients for periodic review and evaluation by physician
18. Prescribes or recommends drugs, as allowed per prescriptive authority, or other forms of treatment such as physical therapy, occupational therapy, etc.
19. Attends admissions meetings
20. Attends weekly treatment team meetings
21. Performs other duties as assigned
Qualifications:
EDUCATION:
• Master's in nursing as NP or master's degree in Nursing with completion of a post-masters NursePractitioner Certification Program.
EXPERIENCE:
• Preferably three (3) to five (5) years of experience in long term rehabilitation with neurobehavioral clients
LICENSURE/CERTIFICATION:
• Current Texas Licensure as a Registered Nurse
• Current Texas Advance Practice Registered Nurse licensure (APRN-NP)
Certified in BLS Current certification and in good standing during employment
Attend behavioral management and de-escalation techniques training as required by the facility.
• Certification in behavior management or related (SAMA, ABA, PIP, etc.)

Your Impact
Using your skills in this position will allow you to deliver personalized compassionate medical care to individuals mainly with CKD and/or ESRD/ESKD. You will also be responsible for caring for patients, maintaining accurate and current patient records and scheduling, and administering follow-up appointments to patients as required. Your gifts as a healthcare professional are urgently needed. In healthcare systems, the patient has too often become secondary due to processes and incentives that don't positively impact the patient for the long term. Roles and Responsibilities
Conducts assessments, which includes comprehensive annual wellness exams on patients both in the patients' home and in the virtual environment
Counsels and educates patients and families about benefits and programs available to help them live healthier lives
Documents items such as: appropriate chief complaint, all applicable diagnosis, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan
Responsible for the coordination of care with primary care providers, specialists, and appropriate ancillary services
Completes all documentation and paperwork in a timely manner
Maintains quality of care standards as defined by the practice
This position will not be office-based but will be remote in state in which employed and will need to attend periodic training/meetings outside of that state
Deliver evidence-based, timely care in a manner that reduces avoidable hospitalizations, maximizes quality of life, and puts patient health and satisfaction first
Prescribe medications, order tests, and collaborate with patient's Monogram physician
Perform effectively, as reflected by improved patient quality outcomes, which will be measured and reported daily
Facilitates closing gaps in care by educating patients about preventive monitoring and working with physician practices to schedule diagnostic testing
Assists patients with enrolling to access educational videos
Participates in the integrated care team meetings
Knowledge of disease diagnosis and prevention
Make assessment of patient's health status
Develop treatment plan
Implement a plan consistent with appropriate plan of care
Follow-up and evaluate patient's status
Other duties as assigned
Position Requirements
Bilingual (English/Spanish) highly preferred
Basic Life Support (BLS) certification is required in this role. The company will support your certification completion through onboarding.
Active and unrestricted Registered Nurse and NursePractitioner or Physician Assistant license
Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
Current and unrestricted DEA certificate
Ability to work without direct supervision and practice autonomously
Access to transportation, a valid driver's license, and car insurance
Must be proficient with medical instruments and equipment required by the work
Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology
Ability to communicate effectively in verbal and written form with retail and medical partners at various levels, patients, family members, physicians and representatives of the community
Sound understanding of all federal and state regulations including HIPAA and OSHA
2 or more years of direct patient care required
Managed Care/IPA/Health Plan experience
Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home

POSITION SUMMARY
The NursePractitioner supports physicians and patients by providing advanced nursing care to patients presenting with uncomplicated/acute clinical problems. NursePractitioner Responsibilities
Demonstrates clinical expertise and expanded knowledge through patient care activities.
Documents patient history and performs physical examinations on patients presenting with routine uncomplicated and acute medical/surgical conditions to establish baseline health profile.
Performs and/or assists in the administration of routine diagnostic/laboratory studies, clinical and therapeutic procedures.
Develops presumptive nursing diagnoses and implements treatment plans based on advanced nursing assessment/problem identification consistent with established practice guidelines.
Documents assessments of the patient?s health and illness status.
Evaluates patient responses to treatment plans and redirects patient care based on findings.
Assesses patient clinical/health education needs. Provides education regarding health matters using clear and compassionate communication and ensures patient understanding. Refers patients to clinical/health education classes for further services as necessary.
Provide telecare to assigned patients, as necessary.
Regularly participate in Clinical Department, TCA Health Staff, and other meetings as assigned.
Qualifications
Active Illinois License in good standing (RN, APRN, Control Sub)
Completion of an accredited School of Nursing program
Master?s degree as a clinical nurse specialist
Active DEA Registration
Current BLS certification
Previous experience in a public health setting preferred

Position Summary
The Pediatric NursePractitioner (PNP) provides advanced clinical care to pediatric patients in an inpatient hospital setting under the direction of the Medical Director and in collaboration with the interdisciplinary treatment team. This role focuses on inpatient medical management, patient assessment, diagnosis, treatment planning, and ongoing monitoring throughout hospitalization.
The PNP independently and collaboratively participates in the observation, assessment, diagnosis, intervention, evaluation, rehabilitation, care coordination, and health education of pediatric patients and their families.
Key Responsibilities
Provide inpatient medical services in accordance with the Texas Nursing Practice Act and facility policies
Obtain comprehensive patient histories and perform complete physical and neurological examinations
Assess and manage acute and chronic medical conditions for hospitalized pediatric patients
Perform emergency and life-saving procedures when clinically indicated
Order, interpret, and respond to laboratory and diagnostic studies
Prescribe and administer medications in compliance with all regulatory and facility guidelines
Document assessments, progress notes, treatment plans, and clinical outcomes in the medical record
Collaborate with attending physicians, nursing staff, therapists, and other healthcare professionals in managing inpatient care
Monitor patient status and ensure continuity of care between physician visits
Notify attending or on-call physicians of significant clinical changes or unexpected events
Participate in admissions reviews, patient rounds, and weekly interdisciplinary treatment team meetings
Recommend and coordinate internal and community-based resources to support patient and family needs
Provide clinical guidance, education, and support to staff as a medical resource
Participate in rotating 24-hour on-call coverage as assigned
Maintain compliance with safety, infection control, and environmental standards
Engage in ongoing professional development and continuing education
Perform other duties as assigned
Required Qualifications
Education
Master's Degree in Nursing as a NursePractitioner, or
Master's Degree in Nursing with completion of a post-master's NursePractitioner Certification Program
Experience
Preferred three (3) to five (5) years of clinical experience
Prior experience in:
Inpatient pediatric settings
Long-term rehabilitation or medically complex pediatric populations
Neurobehavioral or rehabilitation-focused care preferred
Licensure & Certification
Current Texas Registered Nurse (RN) license
Current Texas Advanced Practice Registered Nurse (APRN-NP) license
Active prescriptive authority in Texas
Current BLS certification (required and maintained throughout employment)
Completion of behavioral management and de-escalation training as required by the facility
Certification in behavior management or related methodologies preferred (SAMA, ABA, PIP, etc.)
