Job Summary
Restores Patient Physical Functioning by interpreting physical therapy evaluations and test results; determining physical therapy treatment goals and plans; administering manual exercises; instructing, encouraging, and assisting patients in performing physical activities and in using assistant and supportive devices. They will be responsible for Physical Therapy Student and Physical Therapy Assistants as well as training new staff as needed. They will also participate in program development and research as required per site.

-Supervises PTAs according to organization policy and state regulations.
-Provides individual, family and group therapy sessions for clients engaged in outpatient therapy
-Participates in the interdisciplinary group for hospice patients and the case conference meetings for home health patients.
-Participates in discharge planning.
-Provide initial assessment of clinical needs using standardized tools and protocols and facilitate recommendations for treatment.
-Record all pertinent data in the patient's medical records

Responsibilities
Develops interventions to support goals of therapy and restore maximum function and strength promoting optimal health and minimal pain.
Evaluates results of physical therapy by evaluating by observing, noting, and evaluating patient's progress; recommends and implements adjustments as needed.
Completes patient admission to agency's services after thorough review of medical records.
Assures continuation of therapeutic plan following discharge designing and instructing patients, families, and caregivers in home exercise programs.
Qualifications
Bachelor's degree in Physical Therapy.
A minimum of two years therapy experience, either hospital or clinic experience.
Preferred Qualifications
Master's or Doctorate degree in Physical Therapy.
Homecare Experience.
Required
Registered as Physical Therapist with New Hampshire's Board of Nursing.
CPR Certification.
Satisfactory criminal record.
Satisfactory motor vehicle record.
