Job Summary: Under the direction and supervision of the Center Manager, the Personal Care Attendant (PCA) provides home care services to Jordan Valley Health Senior Care participants. The Personal Care Attendant (PCA) will also participate in other activities and tasks as assigned to assist the home care, therapy, and adult day health staff. Key Responsibilities: Provides personal care and escort assistance in accordance with individual treatment plans - including peri-care, grooming, dressing, showering, bathing, toileting, feeding, ambulation. Reports observed changes in participants' conditions to Center Manager and/or other center-based clinical staff. Proactively communicates participant health and safety concerns to Center Manager and/or other program staff. Demonstrates safe practices utilizing proper body mechanics and complying with universal precautions. Interacts in friendly, courteous, and professional manner with participants and their families/caregivers. Participates in social and recreation activities with participants as appropriate. Implements interventions, as appropriate to training, as directed by rehabilitation staff, including use of mechanical assistance for participant transfers and mobility. Occasionally accompanies participants to outside clinical appointments, when necessary. Assist with food service delivery and monitoring during mealtimes. Assist with treatments as delegated by the Registered Nurse per individualized care plan. Take and record temperature, blood pressure, weight, pulse and respiration as instructed. Report changes or concerns to the RN. Document services performed on computerized charting systems. Assist residents with prescribed program activities. Observe resident and report any changes in physical and emotional condition such as change in attitude, reaction, and appetite, to the supervisor. Assist the resident with personal care which may include the following: shampooing of hair, assist with showering/ bathing, fingernail and foot care, and medication administration. Pass medications. Perform additional tasks as needed. Caregiver Qualifications: Must be at least 18 years old. Current MN Nursing Assistant (CNA) certification preferred, but not required. Excellent communication and customer service skills. A genuine desire to work with our elderly population.
