Provides clinical leadership to promote increased compliance with a range of quality and cost measures and standards of care. Manages service delivery of inter-professional and para-professional team members working on an individual case or population of cases. Acts as a key resource in providing clinical and operational guidance and support to assigned teams and other staff to achieve and enhance team outcomes. Provides advanced nursing clinical care for patients in accordance with current State and Federal rules and regulations for nurse practitioner's scope of practice and national standards of care. Works under the supervision of the Clinical Director for the Nurse Practitioner Program.

What You Will Do
Manages and provides full scope of advanced nursing practice for targeted patient populations. Evaluates patient responses to therapy / interventions. Ensures revision of the inter-professional plan of care as necessary to achieve quality outcomes.
Identifies need for new/revised clinical protocols. Collaborates with physicians and others within the practice to develop protocols and provides training as appropriate.
Manages and provides comprehensive advanced nursing care including physical examination, comprehensive history, screening for physical and/or psychological conditions, emergent interventions, pharmacological and non-pharmacological interventions, ordering treatments and DME, preventative health maintenance activities, care management, referrals, discharge planning, counseling and patient education. Establishes a treatment plan based on clinical findings and. Determines when further evaluation by collaborating physician, specialist or emergency care is warranted.
Collaborates with patients, families, primary care physicians and other team members to provide assessment and care planning. Assesses, plans, and provides intensive and continuous care management across client settings.
Manages and provides clinical services in compliance with standards of Patient-Centered Medical Home standards, meaningful use of medical record data, HEDIS and QARR quality of care measurements.
Qualifications
Licenses and Certifications:
License and current registration to practice as a Registered Professional Nurse in New York State required
Certificate (license) and current registration to practice as a Nurse Practitioner in the State of New York, with a specialty in adult health, family health or gerontology required
Valid driver's license, as determined by operational/regional needs may be required
Maintains credentialed status with VNS Health Medical Care at Home and associated managed care plans required
Maintains NPI, Medicaid and Medicare provider numbers preferred
Maintains a collaborative practice agreement with a physician in compliance with New York State regulations preferred
Must be certified by ANCC or another accrediting Nurse Practitioner body - in order to bill Medicare and meet credentialing requirements required
For Psychiatric Nurse Practitioners only:
Current PMHNP-BC certification required
Education:
Master's Degree of Science in Nursing, or other graduate degree from a nurse practitioner educational program registered by the New York State Education Department as qualifying for NP certification (licensure) required
Current ANCC or AANP certification as an adult, family or geriatric nurse practitioner required
For Psychiatric Nurse Practitioners only:
Master's Degree in psychiatric-mental health Nurse Practitioner required
PhD in psychiatric-mental health Nurse Practitioner preferred
Work Experience:
Minimum of two years of experience as a nurse practitioner utilizing full scope of practice preferred
Clinical home care experience or two years managerial experience preferred
Demonstrated knowledge of Hedis and QARR quality measures, ICD-10 and CPT coding for reimbursement of services required
Bilingual skills, as determined by operational needs required

JOB SUMMARY :
Applewood Centers is currently hiring a Licensed Group Facilitator for our Group Therapeutic Services Program. The Licensed Group Facilitator is a member of a multi-disciplinary treatment team and is responsible for the development, implementation and management of curriculum and treatment of youth referred to the program.
RESPONSBILITIES INCLUDE:
Supervise client activities and is accountable for whereabouts and safety of clients.
Responsible for administration and group/team leading and clinical planning of appropriate therapeutic groups to youth within various clinical topics as directed via curriculum based on individual client treatment needs. Provide case management and TBS.
Implement clinical planning for youth as designed by the treatment team and individualized treatment plan (ITP) and documentation of derivations/changes from clinical planning for clients.
Responsible for the development, planning, of curriculum writing of groups being lead. These curricula are to be completed and approved by the supervisor prior to all group sessions and are due prior to the commencement of any new therapeutic group.
Assist in crisis intervention, problem resolution and process discussions with clients.
Responsible for daily/progress notes for therapeutic service hours delivered, as shared with other facilitators within the groups.
Responsible for weekly summaries of assigned clients.
Contribute to the development and maintenance of the clinical record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards.
As needed, transport clients to and from GTS program in agency provided vehicle.

Qualifications:

Position Summary:
As a Youth Mental Health Specialist, you will have the opportunity to care for our clients while working alongside licensed professionals and gaining excellent clinical experience. You will monitor and maintain awareness of our client's emotional condition and facilitate the daily care of clients by providing emotional support, assistance and motivation.
Responsibilities Include:
Participate in client's progress by monitoring and evaluating client response to treatment, attending staff meetings and clinical rounds, and completing required status reports and paperwork.
Facilitate treatment programs and cottage groups on various topics (i.e. anger management, relationship building, self-esteem, etc.).
Protect clients from physical and emotional harm emanating from themselves, others, or the environment including use of the Therapeutic Crisis Model of Intervention
Provide a structured environment and provide day-to-day care of clients

POSITION SUMMARY:
Under the administrative/clinical supervision of the Clinical Director of UMTYS, the Community Independence Programs (CIP) Therapist,as a member of a treatment team, provides diagnostic assessment, behavioral health therapy and occasional therapeutic behavioral services (TBS)/community psychiatric supportive services (CPST) to clients and their natural supports. The Community Independence Programs (CIP) Therapist is expected to meet the program's productivity target for billable service each week. Services take place in the home, office, and community. Work hours are flexible, some evenings may apply.
RESPONSIBILITIES INCLUDE:
Provide assessments, counseling/psychotherapy, and community support program services for clients, their natural supports, and significant others as needed, including: community organizations, schools, other health and social service professionals, the justice system, and other agencies involved with the client.
Accept caseload assignments from UMTYS leadership and jointly develop tasks and interventions that implement treatment goals.
Provide Mental Health Assessment, Behavioral Health Therapy, therapeutic behavioral services (TBS), and Community Psychiatric Supportive Services (CPST) including, but not limited to:
evaluations, assessments;
participation in the development of the client's Individual Treatment Plan (ITP);
provides assistance and support in crisis situations;
support; including education and consultation for families and/or significant others regarding the client's mental illness;
individual interventions; symptom monitoring and self-management of symptoms; assistance in increasing social support skills and networks;
symptom monitoring and self-management of symptoms;
assistance in increasing social support skills and networks;
coordination of necessary evaluations and assessments;
coordination of all services identified on the client's ITP;
necessary follow-up to determine if services are adequately meeting the needs of the client;
access resources in the community; and
assist in identifying apartment, securing lease of apartment, and moving into the apartment.
Facilitate a psychotherapy group.
Assess each client to determine their proficiency in life skills and their ability to function in a group.
Offer several modalities of teaching necessary life skills including daily living experiences, group and individual instruction.
Utilize group work skills to supervise a safe, growth-producing environment with which clients may address their educational and emotional needs.
Provide adequate and appropriate supervision for all clients. This includes monitoring and observing a client's home residence at least twice a month.
Recognize the significance of the primary natural supports in the client's life and demonstrate the skills to engage the client, primary natural supports and others as clinically indicated in the treatment planning process.
Works a flexible work week and provides evening and weekend on-call coverage on a rotating basis. A minimum of 50% of their week is devoted to productivity.
Contribute to the development and maintenance of the clinical record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards.
OTHER DUTIES:
Attend scheduled staff meetings, supervision, and on-going training.
All required trainings, certifications and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards.
Respect the privacy of clients and hold in confidence all information obtained during the client's treatment. All client-related documents should be handled in accordance with Agency guidelines on confidential material.
Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures.

QUALIFICATIONS:
Education: Minimum Master's Degree in Social Work, Clinical Counseling, or related field.
Licensure: Valid Ohio LSW, LPC, or MFT required.

Performs psychosocial assessments and provides therapeutic services to patients and their families.
Work closely with Behavioral Health Medical Director to manage complex patient cases and transitions of care
A resource for the WellBe communities in managing complex Behavioral Health cases.

Maintains constant communication with patients when addressing their concerns and goals and helping them keep positive attitudes
Builds rapport with patients and their families and relay all concerns to WellBe provider/IDT

Monitors patient progress and adjusts patient treatment plan as needed
