The Licensed Clinical Social Worker (LCSW) is a member of the interdisciplinary health care team, which functions to meet the objectives of the community medical center where they work. The LCSW will provide baseline mental health evaluations on patients at the start of their medical care at the clinic. Provides brief counseling interventions, crisis management and community referrals for patients demonstrating emotional, psychological and/or psychiatric symptoms. Provides referrals for psychiatric evaluation and treatment if appropriate based on mental health needs.
Reports to: Director of Behavioral Health Services and Clinic Administrator
Qualifications:
1. Active license as a Licensed Clinical Social Worker in the State of California per the legal requirements under the board.
2. Minimum of two years prior experience providing direct patient services in a medical environment, or prior experience in working with patients in a clinical or social services environment; including working with adults or adolescents in individual, group, and family therapy.
3. Able to work a flexible schedule of 20 to 30 hours per week or full time as designated by position.
4. A working knowledge of theories and interventions that are utilized to successfully work with patients in order to provide empathy, support, and encourage patients to initiate change.
5. Must have excellent oral and written communication skills and be able to communicate in large or small groups and public speaking skills.
6. Thorough working knowledge of business English, spelling, punctuation, and contemporary general office practices and procedures
7. Excellent leadership skills and working from a team-based perspective.
8. Typing/data entry skills sufficient to meet production requirements.
9. Effective verbal and written communication skills to communicate clearly and effectively with patients and others.
10. Effective work organization skills.
11. Ability to be flexible and work in a changing environment.
12. Sensitivity to the different cultures represented among members and staff.
13. Demonstrated ability to listen and communicate with others in a professional and caring manner including sensitivity with individuals from diverse cultures and lifestyles.
14. Demonstrated ability to set priorities for tasks to work effectively in spite of interruptions and under minimal supervision. Self-starter, reliable and dependable.
15. Demonstrated proficiency with the electronic health record database within three months of attending training session(s).
16. Fluency in English (speak, read and write).
17. Fluency in Spanish (speak, translate, read and write) is preferred.
18. Ability to solve problems and make routine recommendations.
19. Ability to maintain absolute confidentiality about patient(s) Protected Health Information.
20. Current California driver's license, appropriate insurance coverage and a driving record acceptable to the NEVHC's insurance carrier (if required to drive on the job).

Position Purpose:
The Licensed Clinical Social Worker (LCSW) plays a vital role in supporting the emotional and behavioral health of residents at Wesley Health Centers - Skid row Residential Care Facility, a licensed Residential Care Facility for the Elderly (RCFE) serving individuals in the unique location of Downtown Los Angeles Skid-row area. This full-time, in-house position focuses on mental health care, substance use recovery support, crisis intervention, and care coordination of individuals who are experienced homelessness. The LCSW collaborates closely with administrators, Certified Nurse Assistants, outside providers, and community agencies to ensure residents receive comprehensive, trauma-informed support that honors their dignity and enhances quality of life. The role of LCSW in this facility will assist us in a transformative process as individuals' transition from living in interim housing placements or the streets to a supportive living facility.
Program Population:
The Crocker Residential Care is a licensed Residential Care Facility for the Elderly (RCFE) serving older adults, including individuals with histories of homelessness, substance use disorders, behavioral health challenges, and/or chronic medical conditions. Residents may require support accessing community resources, engaging in recovery, or managing psychiatric symptoms in a supportive housing environment.
Principal Responsibilities:
Conduct comprehensive psychosocial and behavioral health assessments upon admission and as clinically indicated.
Provide crisis intervention and crisis stabilization which may include initiated a 5150 hold and working with County Mental Health regarding timely disposition.
Develop and implement individualized care plans addressing mental health, substance use, trauma history, and recovery goals.
Provide direct clinical services including supportive counseling, crisis intervention, harm-reduction support, and relapse prevention strategies.
Monitor resident behavioral health status and intervene in decompensation or high-risk situations.
Collaborate with program staff to ensure trauma-informed, person-centered practices are integrated into daily care.
Coordinate with County Mental Health, local outpatient services, and Medi-Cal Managed Care Plans for behavioral health referrals and continuity of care.
Assist residents in understanding and accessing benefits through Medi-Cal and CalAIM programs (e.g., Enhanced Care Management).
Maintain confidential, timely, and accurate documentation in compliance with RCFE and County standards.
Participate in resident care team meetings, quality assurance reviews, and staff training on behavioral health best practices.
Requirements:
License: Must hold a current, active Licensed Clinical Social Worker (LCSW) license issued by the California Board of Behavioral Sciences.
Must be Lanterman-Petris-Short (LPS) designated to initiate 5150 holds.
Education: Master's degree in Social Work (MSW) from an accredited institution.
Clinical Knowledge: Demonstrated experience or training in trauma-informed care, co-occurring disorders (mental health + substance use), harm reduction, and housing-first models.
Regulatory Knowledge: Familiarity with Medi-Cal, CalAIM Enhanced Care Management, and applicable RCFE and HIPAA regulations.
Skills: Excellent interpersonal, crisis de-escalation, and written communication skills. Ability to work with residents from diverse backgrounds in a respectful, empowering manner.

Position Purpose:
The Licensed Clinical Social Worker (LCSW) plays a vital role in supporting the emotional and behavioral health of residents at Wesley Health Centers - Skid row Residential Care Facility, a licensed Residential Care Facility for the Elderly (RCFE) serving individuals in the unique location of Downtown Los Angeles Skid-row area. Program Population:
The Crocker Residential Care is a licensed Residential Care Facility for the Elderly (RCFE) serving older adults, including individuals with histories of homelessness, substance use disorders, behavioral health challenges, and/or chronic medical conditions. Residents may require support accessing community resources, engaging in recovery, or managing psychiatric symptoms in a supportive housing environment.
Principal Responsibilities:
Conduct comprehensive psychosocial and behavioral health assessments upon admission and as clinically indicated.
Provide crisis intervention and crisis stabilization which may include initiated a 5150 hold and working with County Mental Health regarding timely disposition.
Develop and implement individualized care plans addressing mental health, substance use, trauma history, and recovery goals.
Provide direct clinical services including supportive counseling, crisis intervention, harm-reduction support, and relapse prevention strategies.
Monitor resident behavioral health status and intervene in decompensation or high-risk situations.

The Behavioral Health Therapist under the general supervision of the Clinic Manager plans and provides direct mental health psychotherapy and co-occurring services for adults, children, families and groups. Emphasis is on an interdisciplinary team approach, recovery oriented and strength based methods in providing comprehensive care to clients and their families. Supervised hours of experience are provided by a Licensed Clinical Social Worker, Marriage and Family Therapist, Licensed Psychologist or Board certified Medical Doctor to meet the pre-licensing accrual hours necessary for licensure in the Associate's related field under jurisdiction of the Board of Behavioral Sciences. All BH Therapist Associates must be credentialed by the Clinica Sierra Vista credentialing board before they may independently see clients and must abide by all clinical directives and guidelines provided to them by their licensed supervisor. Details of the supervisor/supervisee rights and responsibilities are outlined in the BBS terms and conditions for accrual of supervised hours of experience in a non-profit setting. Essential Functions:
* Provides individual, group and family psychotherapy services as allowed within the employees' scope of practice and the recovery model guidance for best practices at the service site.

Requirements:
Master's degree in Marriage and Family Therapy from an accredited institution.
Current AMFT registration with the California Board of Behavioral Sciences.
Experience working with women, children, and families affected by homelessness, child welfare involvement, substance use, mental illness, poverty, and/or incarceration.
Knowledge and training in evidence-based treatments, including Cognitive Behavioral Therapy (CBT), Matrix, Seeking Safety, and Motivational Interviewing.
Understanding of family systems theory, especially in the context of pregnant and postpartum women and their children.
Demonstrated understanding of the impact of trauma on behavioral and medical conditions.
Cultural competence in working with minority, low-income, and vulnerable populations.
Ability to work collaboratively within a clinical team.
Strong documentation, oral, and written communication skills.
Proficiency in computer use and electronic records.
Bilingual (English/Spanish) ability preferred.

Job Description
The Mental Health Therapist / Team Lead (AMFT) will provide individual therapy, case management, and group services at our Women and Children's Residential Treatment program in South Los Angeles. The Therapist / Team Lead will deliver trauma-informed, gender-responsive care using evidence-based practices, including Matrix, Seeking Safety, Helping Women Recover, and Parenting programs.
This position supports JWCH's mission to improve the health status and well-being of underserved populations in Los Angeles County through direct care, health education, supportive services, and research. The role is funded by the Substance Abuse Prevention and Control (SAPC) program through Los Angeles County.
Principal Responsibilities:
Maintain a client caseload in accordance with grant or contract requirements.
Facilitate groups such as Substance Abuse Education, Matrix, Seeking Safety, Parenting, and others.
Conduct mental health assessments, therapeutic interventions, and clinical evaluations.
Collaborate with the treatment team to deliver comprehensive services to women and children.
Regularly assess and provide case management services to assigned clients.
Develop individualized treatment plans in collaboration with team members and clients.
Participate in clinical meetings, case reviews, and team huddles.
Document all services provided within 24 hours using the Electronic Health Record (SAGE/PCNX)

Principal Responsibilities:
Work at a Federally Qualified Health Center multidisciplinary team to ensure that the patient's clinical treatment goals and behavioral management needs are met by attending case conferences, chronic disease management meetings, staff consultation, and individual meetings with staff to ensure quality of care.
Conduct functional and strength-based assessment along with comprehensive diagnosis for mental health, substance abuse, trauma, and psychosocial needs.
Develop student care plans that are coordinated and integrated with a comprehensive range of medical and psychosocial services to address social, emotional, and practical needs.
Provide in-person and remotely (via HIPAA Compliant software) psychotherapy, counseling, medical case management and psycho-education utilizing individual, family, and group modalities using evidence-based interventions for patients and their significant others that include social, emotional, and practical life skills to facilitate safe coping, distress tolerance, stress reduction, mindfulness, and relaxation as well as access to necessary resources with the goal of improved functioning and positive health outcomes.
Utilize both short-term and intensive longer-term interventions, as approved by funding source that target trauma, mental health symptoms, increase psychosocial functioning and reduction of substance use.
Interventions are client centered; strength based, and identified based on a comprehensive treatment plan that is developed, after completing a thorough biopsychosocial assessment.
Consult with primary care providers to enhance understanding of the patient, provide support for treatment approaches that incorporate behavioral health needs, and assist with implementation of bio-psychosocial treatment and care plans.
Work with ECM Case Management staff to coordinate care and provide referrals, coordinate services and reduce barriers.
Participates in weekly case conferences to assist in problem solving related to patients and to ensure that professional guidance and high-quality case management services are being provided.
Participates in community meetings pertaining to the needs of JWCH patients.
Participate in Multi-Disciplinary Team meetings that are held weekly in the clinics.
Be available as assigned to provide after-hours emergency psychiatric assistance.
Provide phone consultation as needed
Productivity goal is to average 8-10 patients per day.
Complete all charting as required by JWCH, including assessments, progress notes, care plans, and billing documentation. Encounters must be closed by the end of each shift.
Assist with provider education for integrated behavioral and primary care services
Participate in behavioral health quality assurance/improvement activities to maintain and strengthen standards of care, in accordance with the organization CQI Plan

Requirements:
Master's degree in social work, social welfare from an accredited college or university.
LCSW with current California BBS license
Demonstrated understanding of the impact of trauma on behavioral and medical conditions
Knowledgeable of evidence-based treatments (sand and play therapy, DBT, CBT, Seeking Safety, Motivational Interviewing)
Knowledge of principles, procedures, methods and techniques of mental health emergency procedures such as: 5150, CPS reporting, APS reporting, and Tarasoff reporting
Experience working with substance abuse disorders preferred.
Bilingual (English/Spanish) ability needed.
