Definition: The Floater Licensed Vocational Nurse (LVN) I provides a variety of administrative, clinical nursing, health education, and outreach services within the community health center. Basic skills for physical assessment include observation, auscultation, palpation and percussion. Basic mental status assessment includes observation of general behavior, appearance and attitude, collection of data relative to sensorium or orientation to time, person and place, and assessment of recall ability. When the LVN notes a problem, complication or abnormality, he/she must notify the physician or Registered Nursing Supervisor under whose direction the licensee functions. The LVN cannot independently evaluate or synthesize data.
Reports to: Direct & Administrative: Associate Director of Healthcare Operations Qualifications:1.Must be a high school graduate or have a GED equivalency.2.Satisfactory completion of a state approved program in vocational nursing, plus current license to practice in the State of California.3.Experience in practical nursing desirable, preferably in an ambulatory patient facility.4. (a) Pediatrics - CHDP
(b) Family planning, title 10 program
(c) Prenatal or Perinatal CPSP
(d) Diabetes
(e) Adult care, chronic disease
2. Must be a high school graduate or have a GED equivalency.
3. Satisfactory completion of a state approved program in vocational nursing, plus current license to practice in the State of California.
4. Bilingual Spanish/English is required in most positions and only preferred in others.
5. Valid CA Driver's License, current vehicle liability insurance, and an available vehicle required.

The Lead Case Manager for value based programs such as Chronic Care Management (CCM) and ACO services (for patients enrolled in Medicare) is responsible for coordinating with external CCM and ACO supervisors and programs, serving Fresno and Kern County. This role involves managing day-to-day activities, ensuring the efficient use of resources, and maintaining high performance standards while meeting the Centers for Medicare and Medicaid Services (CMS) program requirements. The Lead Case Manager will work closely with leadership, care teams, and external partners to drive team performance, ensure compliance, and contribute to achieving programmatic goals aligned with Centers for Medicare and Medicaid Services (CMS) initiatives.
Essential Functions:
Operational Oversight & Supervision:
Work with the CAO and other department directors to ensure compliance with agency, grant, and regulatory requirements, including OSHA, ADA, CLIA, HIPAA, HRSA, TJC, NCQA, DHCS, FQHC and all CMS specific standards
Report and address any legal, privacy, safety, or risk management issues related to CCM and ACO services.
Track and evaluate CCM and ACO program successes through data-driven decision-making, patient engagement, and quality assurance reviews.
Oversee CCM staff access, including client service specialists, medical assistants, case managers, and outreach workers, ensuring compliance with CCM and ACO program guidelines.
Monitor staff schedules and PTO requests to always ensure adequate coverage. (If applicable)
Ensure timely submission of staff payroll and accounting records to facilitate prompt payroll processing. (If applicable)
Perform introductory and annual employee reviews and competency assessments, and provide constructive feedback to support organizational and professional growth. (If applicable)
Collaboration & Communication:
Address patient needs, including complaints, problem-solving, and de-escalation, ensuring timely resolution per CSV policies.
Represent Clinica Sierra Vista's Value Based Programs or partnerships such as CCM.

The Licensed Vocational Nurse (LVN) I/II/III provides a variety of administrative, clinical nursing, health education, and outreach services within the community health center. Basic skills for physical assessment include observation, auscultation, palpation and percussion. Basic mental status assessment includes observation of general behavior, appearance and attitude, collection of data relative to sensorium or orientation to time, person and place, and assessment of recall ability. When the LVN notes a problem, complication or abnormality, he/she must notify the Physician or Registered Nursing Supervisor under whose direction the licensee functions. Definition: The Licensed Vocational Nurse (LVN) I/II/III provides a variety of administrative, clinical nursing, health education, and outreach services within the community health center. Basic skills for physical assessment include observation, auscultation, palpation and percussion. MLK Recuperative Care Center is an Interim Housing program that assists homeless, medically compromised individuals to recover and improve their health conditions. The role of each staff at MLK RCC is to assist the client in overcoming barriers and increasing coping skills to decrease negative behaviors.

Position Purpose:
The Licensed Vocational Nurse (LVN) plays a vital role in delivering compassionate, patient-centered care in a non-traditional setting that prioritizes harm reduction and trauma-informed care. Under the supervision of the Program Director and RN, the LVN provides direct patient care, performing routine clinical procedures, technical tasks, and essential patient support functions at the Crocker Care Campus. This role requires a flexible, adaptable approach to healthcare, meeting patients where they are-including non-clinical spaces such as drop-in centers, outdoor parklets, and community areas.
About the Crocker Care Campus & Harm Reduction Model:
The Crocker Care Campus is a radical, patient-centered space designed as a welcoming drop-in site for people who use drugs and those experiencing homelessness. It provides a safe, accessible environment focused on hospitality, community, and comprehensive care. Unlike traditional clinics, Crocker operates with flexibility, prioritizing low-barrier engagement and participant autonomy over rigid structures.
Our approach acknowledges the complex medical and psychosocial histories of our participants, including individuals who use drugs, engage in sex work, have been incarcerated, or have faced structural racism, poverty, and trauma. We view unhealthy drug use as a multifaceted issue shaped by systemic barriers-not simply as an individual choice. Principal Responsibilities:
