Manages chronically ill patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. Job Responsibility + Provides direct services to patients including the completion of needs assessments, development of patient focused care plans, periodic reassessments and overall comprehensive service coordination.
+ Advocates the requirements of clients within the agency and with external service providers.

Essential Duties and Responsibilities include:
Works with the Center's physicians and other key personnel to operationalize grant deliverables, orchestrate change as indicated and ensure continuous quality improvement.

In conjunction with the Program Director, Program Manager and Center physicians, the Case Manager will participate in and conduct training programs for medical professionals that will include information about the diagnosis, treatment and care management of individuals with AD/D, the importance of early detection, management of behavioral issues, need to address palliative care and ethical issues facing someone with dementia and the importance of screening individuals over the age of 65 for cognitive impairment as a component of regular care using a validated cognitive screening tool.
As assigned by Program Manager, represents the CEAD at meetings, conferences and other events as required. Interface with any/all community agencies, which may serve to benefit the Center/CEAD or its constituency.
In conjunction with Program Manager, assists with maintaining consistent follow-up with primary care providers/medical specialty physicians, non-physician health care providers and college and universities (for student trainings) to strengthen relationships.

* Assesses educational needs and provides learning opportunities for health care professionals relevant to particular cases and selected patient care groups.
* Collaborates with case management leadership to compile and report aggregate variances and data for specific patient care services.
* Communicates and analyzes aggregate variances with members of the health care team and develops strategies for variance reduction.
Qualifications

The Albany Medical Center's Surgical Care program is growing to address the needs of our patients. We have expanded our surgical facilities and capabilities.
The Operating Room at our Main Campus has 35 state-of-the-art operating suites with the most advanced robotic technology and specialty rooms for cardiac, neurology, urology, vascular and pediatric patients. Hybrid operating suite combines minimally invasive and interventional surgical techniques and includes 3D and rotational CT imaging. Two-room intraoperative CT suite for highly complex procedures spine and brain surgery.
