The Home/Community Provider implements a prescribed habilitation plan for eligible participants while providing routine direct care either in the home, community or center based. This position works closely with the family or guardian and the FRA Home/Community Services Manager to design, implement, and modify consistent intervention strategies that can ultimately be maintained by the family or guardian in their daily routines.
DUTIES AND RESPONSIBILITIES:
1. Depending upon the eligible participants needs provides direct care as per the Plan of Care (POC). Services may include acquiring, retaining and improving self-help, socialization and adaptive skills such as meal preparation, eating, toileting, bathing, dressing and recreational activities.
2. Assures that health and safety needs are met.
3. Prepares and/or assists the individual to prepare meals. Ensure that nutritional needs are met during the time of care.
4. Observes, supervises and records eligible participant activities. Discusses with Home/Community Manager and other staff members the participants' responses to intervention strategies and makes suggestions for modifying activities.
5. Maintains records and prepares reports on services provided.
6. Performs or requests emergency aide as needed or required.
7. Maintains current knowledge of developmental disabilities.
8. Inputs participant information and case notes into electronic medical record (Therap), in accordance with agency procedures, state and federal requirements when applicable.
9. Attends training seminars, in-service sessions and staff meetings as requested.
10. Responsible for maintaining training compliance in accordance with FRA policies and procedures, state and federal regulations.
11. Provides other support services and duties as necessary.
MINIMUM EDUCATION AND EXPERIENCE:
1. High School Diploma or equivalent.
2. Six months experience working with individuals who experience developmental disabilities is preferred, but not required.
3. Must meet agency insurance criteria for operating agency and personal vehicles on agency business (21 y/o and licensed 3 years).

Demonstrate proficiency in direct care activities. Ability to communicate effectively, both orally and written, with participants, families, guardians, supervisor, other agency staff and the general public. Triaging call for other villages may be required on occasion. Transports patients from the scene of the accident to clinic/airstrip using the emergency transport vehicle when assistance is required.
* Participates in Village Response Team activities including meetings, preparation, training and emergency response.
* Be available to travel to provide planned and/or emergency coverage in another Kodiak village clinic no more than four weeks (28 days) per year unless the CHA/P volunteers for additional assignments. Able and willing to travel periodically for training or other required events held in Kodiak, Kodiak Island communities, or off island.
* Maintains an orderly and confidential system of medical records, clinic logs and submit periodic reports. Enters all patient encounters in the KANA Electronic Health Record (EHR) system at the time of the visit or, in the case of emergencies or following a loss of power or internet access, immediately thereafter.
* Maintains the itinerant binder containing changeover forms, CHAP policies, emergency procedures, medical contact numbers, on-call schedules, current CHAP coverage calendar, EHR job aides, Medicaid travel process, village map and contacts, and other important information to share with incoming CHA/P itinerants and providers.
* Strictly adheres to the provisions of the Privacy Act, HIPAA and KANA policies. Maintains patient confidentiality and medical chart integrity.
* Keeps clinic quarters tidy, keeping personal items separated from the public spaces. Weekly discard any expired food items, fold and put away dry linens following an itinerant or provider stay, vacuum, wipe down sink, counter and appliances, put away all dishes.
* Verifies release of information (ROI) paperwork is complete before forwarding ROI requests to KANA Medical Records for processing.
* Reviews the current copy of KANA policy and procedure manuals, clinic MSDS data sheets, and required monthly paperwork for accuracy and completeness. Completes all weekly/monthly compliance paperwork and submit to the CHAP office.
* Monitors clinic and office supplies, order and restocks standard inventory as needed/directed to maintain clinic operations. Ensures a sanitary clinic environment.
* Assist with errands, including mail and package pick-up and delivery between the clinic, post office and airstrip. Transport providers and visitors to the clinic as needed. Reimbursed usage of personal vehicle may be required if the emergency response vehicle is not available.
* Follows up with Clinical Supervisor regarding any outstanding post-session learning needs and paperwork for completion, medical and EMS skills review, continuing education, and additional training needs and creates a plan to complete.
* Maintains current CHAP credentials (board certification) by keeping track of all required certification paperwork including training records (post session learning needs form, skills sheets, clinical evaluations, PEF reviews, medical traffic, encounter log), CME hours for medical and emergency skills, applications, training certificates, CHAPCB provider certificates, EMS certification, BLS/CPR card. Turns in a copy of all completed paperwork to the CHAP office.
* Assists in the education of other Health Aides as directed including Field Follow-Ups, Post-Session Learning Needs and in daily consulting as needed.

The Home/Community Provider implements a prescribed habilitation plan for eligible participants while providing routine direct care either in the home, community or center based. This position works closely with the family or guardian and the FRA Home/Community Services Manager to design, implement, and modify consistent intervention strategies that can ultimately be maintained by the family or guardian in their daily routines.
DUTIES AND RESPONSIBILITIES:
1. Depending upon the eligible participants needs provides direct care as per the Plan of Care (POC). Services may include acquiring, retaining and improving self-help, socialization and adaptive skills such as meal preparation, eating, toileting, bathing, dressing and recreational activities.
2. Assures that health and safety needs are met.
3. Prepares and/or assists the individual to prepare meals. Ensure that nutritional needs are met during the time of care.
4. Observes, supervises and records eligible participant activities. Discusses with Home/Community Manager and other staff members the participants' responses to intervention strategies and makes suggestions for modifying activities.
5. Maintains records and prepares reports on services provided.
6. Performs or requests emergency aide as needed or required.
7. Maintains current knowledge of developmental disabilities.
8. Inputs participant information and case notes into electronic medical record (Therap), in accordance with agency procedures, state and federal requirements when applicable.
9. Attends training seminars, in-service sessions and staff meetings as requested.

Position Summary:

Participates in the resolution of customer problems or collects necessary data and follows through to appropriate person for problem resolution.
Maintains a complete patient record with admission, transfer, and discharge information, progress notes, and consent forms. Ensures that all reports and chart forms are scanned or filed into the patient's record. Ensures that charting forms are labeled and accessible if they are needed. Maintains adequate supplies of frequently used, patient education materials. Assists in the ordering of patient specimens. May transcribe and process physician orders effectively using the electronic medical record.
Maintains adequate levels of office, dietary, and medical supplies and completes requisition forms when supplies need to be ordered. Reconciles supply order upon delivery. Qualifications

Current BLS certification. * Strictly adheres to the provisions of the Privacy Act, HIPAA and KANA policies. Maintains patient confidentiality and medical chart integrity.
* Keeps clinic quarters tidy, keeping personal items separated from the public spaces. Weekly discard any expired food items, fold and put away dry linens following an itinerant or provider stay, vacuum, wipe down sink, counter and appliances, put away all dishes.
* Verifies release of information (ROI) paperwork is complete before forwarding ROI requests to KANA Medical Records for processing.
* Reviews the current copy of KANA policy and procedure manuals, clinic MSDS data sheets, and required monthly paperwork for accuracy and completeness. Completes all weekly/monthly compliance paperwork and submit to the CHAP office.
* Monitors clinic and office supplies, order and restocks standard inventory as needed/directed to maintain clinic operations. Ensures a sanitary clinic environment.
* Assist with errands, including mail and package pick-up and delivery between the clinic, post office and airstrip. Transport providers and visitors to the clinic as needed. Reimbursed usage of personal vehicle may be required if the emergency response vehicle is not available.
* Follows up with Clinical Supervisor regarding any outstanding post-session learning needs and paperwork for completion, medical and EMS skills review, continuing education, and additional training needs and creates a plan to complete.
* Maintains current CHAP credentials (board certification) by keeping track of all required certification paperwork including training records (post session learning needs form, skills sheets, clinical evaluations, PEF reviews, medical traffic, encounter log), CME hours for medical and emergency skills, applications, training certificates, CHAPCB provider certificates, EMS certification, BLS/CPR card. Summary of Job Responsibilities:
The Southcentral Foundation (SCF) Community Health Aide is responsible for providing acute, chronic, emergency, and preventive medical health care to Village residents under the direction of the medical doctors in Anchorage and according to the CHA/P Manual. The CHA Trainee (CHA-T) level is a training classification for those who do not yet qualify as CHA I. The CHA I, II, III, IV and Practitioner are expected to operate within the guidelines of the CHA/P Manual (eCHAM) and their level of training and the scope of practice consistent with their level of training. CHA II, III, IV and Practitioner are also expected to act within the limits of their Medical Standing Orders (MSO) when applicable.
This position has five (5) level designed to provide progressively more responsible and independent work experience and training. Hours: Full Time, Variable Shift
Under direct supervision this position is accountable to the RN to facilitate a safe and secure environment for the patient/resident, family and providers in the department in completion of the duties delegated to them for the shift and maintaining clear communication between the needs of the patient/resident through the RN. These activities focus on maintaining a safe, hazard free, clean environment which facilitates patient/resident focused care.

Required Qualifications:
+ Successful completion of Caring Reliably, Culinary Training, ServSafe, and Dining Assistance within four months of hire.
+ Completed training as a Certified Nursing Assistant
+ AlaskaNursing Assistant Certification License upon hire.
+ National Provider BLS - American Heart Association within 30 days of hire.
Preferred Qualifications:
+ 6 months of experience as a nurseaide, nursing assistant or personal care attendant in long term care or hospital.
