Assists the patient in ambulation, exercise, and use of walker, cane or wheelchair as instructed. Assists with activity prescribed by physical or occupational therapist as instructed. Perform or assist a patient with the personal care and treatments needed to provide good personal hygiene and a healthy, safe environment as directed by the plan of care and by a registered nurse or therapist. Performs homemaking tasks as assigned. Assists with treatments as ordered by the physician and instructed and supervised by the nurse, such as change of a colostomy bag, shampoo, simple wound care, reinforcement of a dressing, simple urine tests for sugar, acetone, and albumin, and measuring intake and output. Encourages patient's mental alertness through involvement in activities of interest. Gives simple emotional and psychological support to the patient and other members of the household or facility. Encourages the patient to become as independent as possible according to the plan of care. Assists patients with bathing and personal care as directed by the plan of care. Monitors vital signs as directed by the plan of care. Monitors skin integrity and provide skin care as directed by the plan of care. Provides hair care including shampooing as directed in plan of care. Attends IDT meetings to provide patient care updates (as schedule allows)

We are currently seeking the professional skills of a CNA to work in our long-term care facility! This position is 1st shift, 6:30am - 3:00pm, every other weekend and rotating holidays.
Pay is $25.00/hour.
This position will be responsible for assisting residents with activities for daily living. This position requires an IDPH CNA certification. To provide outstanding resident care, experience in a geriatric setting is preferred with the demonstrated ability to work effectively with residents and personnel.

Personal Care Assistant (PCA)
An individual who, under professional supervision, provides assistance with nutritional and environmental support, personal hygiene, feeding and dressing to clients in their residence.
Organizational Relationship: Reports to the Nursing Supervisor
Risk Of Occupational Exposure To Blood Borne Pathogens: B: limited exposure
Qualifications:
1. Successful completion of a nurse aide training and competency evaluation program pursuant to the requirements of 42 CFR Part 483, Subpart D as revised or recoded, if applicable;
or
2. Successful completion of a competency examination for nurse aides recognized by the department;
or
3. Successful completion of a health care or personal care credentialing program recognized and approved by the department;
or
4. Successful completion or progress in the completion of a 40 hour training program provided by a private home care provider, which addresses at least the following areas:
a. Ambulation and transfer of clients, including positioning;
b. Assistance with bathing, toileting, grooming, shaving, dental care, dressing, and eating;
c. Basic first aid and CPR;
d. Caring for clients with special conditions and needs so long as the services are within the scope of the tasks authorized to be performed demonstration;
e. Home management;
f. Home safety and sanitation;
g. Infection control in the home;
h. Related activities to include the taking of vital signs; and
i. Proper nutrition.
Speaks, reads, and writes and able to communicate effectively in English.
Must have current CPR certification and Criminal Background check.
5. Never have shown by credible evidence, to have abused, neglected, sexually assaulted, exploited or deprived any person or to have subjected any person to serious injury as a result of intentional or grossly negligent misconduct.
6. Have not made any material false statements concerning qualifications requirements either to the Department or the provider.
Responsibilities:
- Personal Care - assists with:
- Bath (tub, shower or bed), Oral hygiene, Grooming and care of hair, Ordinary care of nails
- Routine skin care/ lotion massage, Positioning and turning of patents that cannot assist self- in bed and chair, Elimination, Assist with eating, dressing walking and toileting
- Remind client to take medications
- Homemaking - assists with:
- Meal planning and preparation in accordance with complex and modified diets
- Feeding the client, Linen change (occupied and unoccupied), Laundry, client's only
- Light housekeeping; washing dishes, clean kitchen, dust & vacuum, only those rooms
the client uses.
- List needed supplies and grocery shopping
PCA'S ARE REQUIRED TO:
- Follow universal precautions whenever giving any aspect of client care.
- Maintain confidentiality.
- Perform ONLY those functions specified for each individual client on the POC.
- Respond to the physical, emotional and development needs of clients.
- Follow emergency procedures in the event of any incident, e.g., accident, injury
or significant change in client's condition.
Essential Administrative Functions:
- Follow client rights at all times.
- Record each activity performed on each case on a daily basis
- Report any incident or change in condition of client or environment immediately.
- Submit Daily Activity Sheets and record accurately on a timely basis (WEEKLY).
- Follow instructions, is punctual and is at work as scheduled.
- Follow all appropriate Provider policies.
- Attend in-service education annually per Provider policy.
- Provide all information required for the maintenance of her/his personnel record as per
State regulations.
- Show interest, asks questions and seek help as indicated; is receptive to supervision.
