Summary:
We are seeking a candidate who would be responsible for the personal care of patients/families on a field team as a participating member of the hospice team. Tasks are assigned by and performed under the supervision of a Registered Nurse.
Qualifications: (one of the following)
Satisfactory completion of a training and competency evaluation program which complies with state/federal regulations and one (1) year experience preferred.
Be on the Texas Department of Human Services' Nurse Aide Registry with no findings against the Aide relating to client abuse or neglect or misappropriation of client property. (AND)
According to Nurse Aid Registry, no evidence/findings against the Aide relating to client abuse or neglect or misappropriation of client property.
No criminal history or employee misconduct record that would preclude hiring.
Current certification in basic life support.
Experience with terminal patients, oncology or home health services preferred.

Document care provided and response to care according to documentation guidelines.
Report changes in patient/family condition and/or needs to primary nurse.
Complete twelve hours continuing education per calendar year to be prorated according to date of hire.
Participate in weekend and holiday rotation.
Assists with new employee training.
Attend 100% of mandatory in-service meetings.
Understand and adhere to organizational privacy practices and compliance programs.
Assist in providing service that fosters the organization's mission, values, vision and goals.

Assist in personal grooming of residents, e.g. brushing hair, teeth, trimming fingernails and toenails. Monitor hygiene continuously and assist as needed to assure that each resident is clean and neat.
If resident is unable to reposition self, aide may be required to turn and reposition resident intermittently.
May be required to clean equipment such as bedpans, basins and empty catheter bags.
Maintain each bedside unit in neat and orderly condition. Clean table surfaces as needed. Store personal items such as dentures, eyeglasses, combs, brushes in sanitary and convenient manner.
Make continuous observation of housekeeping and maintenance needs, e.g. broken fixtures, burned out light bulbs, soiled floors and curtains, and reports needs promptly.
Maintain bedside care supplies, e.g. gloves, tissues, toothpaste, use charge system to record supplies obtained from facility stock. Notify charge nurse to have them call if items are to be supplied by family.
Maintain all service areas in clean and orderly condition. This may include washing dishes, sweeping, mopping, and vacuuming floors, clean counter tops, and minor straightening of resident apartments.
Assist in assuring the maximum quality of life for all residents by maintaining privacy, accommodating individual preference, promoting independence, encouraging and assisting participation in facility activities and events and communication respectfully at all times.
Respond to all call lights promptly. Keep call light within reach of non-ambulatory resident when unattended.
Maintain continuous communication with team leader. Participate in report; communicate all changes and unusual observation concerning residents physical and psychosocial functioning.
Attend unit meetings as scheduled, all mandatory in-services and optional in-services per individual needs and interest, for a total of at least 12 hours annually. Outside educational offerings may be substituted for optional in-service hours by providing proof of attendance. Mandatory in-service cannot be substituted.
