As a Home Health Certified Nursing Assistant, here's a big-picture view of what you'll do:
Care for seniors and those with an illness, injury or chronic disease in their homes
Help with daily activities such as bathing, toileting, dressing, grooming, hygiene, nail/skin care, eating and nutritional intake
Assist with ambulation, transfers and range of motion exercises
Check vitals, measure intake/output, monitor blood glucose and document any changes
Work with a team of home health nurses and aides to ensure a patient's care plan is followed
Observe patient and notify supervisor of issues requiring a change in care, medication or medical equipment
A few must-haves for Home Health Certified Nursing Assistants:
High school diploma (or equivalent) and active Nursing Assistant Certification in the state of Colorado
BLS for Healthcare Providers/CPR certification (demonstration course required)
Compassionate nature, good communicator and ability to lift up to 50 lbs.

Assists with ambulation, range-of-motion exercises, positioning, turning and transfers according to patient's abilities, tolerance and preference.

Provides personal care to patients, including bathing, peri-care, shampoo, oral hygiene, denture care, nail care, shaving and assisting with dressing.

Assists participants with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed
Creates and participates in group(s) and one-on-one activities in memory impaired unit
Monitors and assists participants with meals at feeding table
Assists, throughout the day, in maintaining clean and orderly day rooms, kitchen and personal care areas
Assists the drivers with the loading and unloading of participants from vehicles
Demonstrates dependability through consistent compliance with scheduled work hours
Participates in participants care planning through cooperation with interdisciplinary teams
May assist other departments as a CNA as approved by the Day Center Supervisor
Assist with Meal Reimbursement sheets as needed
Documents services provided to participants consistently, accurately and in a timely manner via Electronic Health Record (EHR). Follows procedure for flow sheet completion
Participates in participants care planning as requested
Handles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautions
Displays a willingness and ability to be responsive in a warm and caring manner to all customer groups.
Participates in Quality Assurance and program improvement initiatives as assigned

Ability to work independently with minimum supervision
Knowledge of how to use and maintain medical equipment (adaptive, hoyer lift, etc.) is strongly preferred

ESSENTIAL JOB FUNCTIONS:
Travels to participant's homes to assist with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned/designated in the care plan
Assists participants with restorative and supportive care (including ambulating, positioning, transferring, skin care, catheters, enemas, home exercise program as instructed by physical therapist), as assigned/designated in the care plan
Assists in monitoring participant's health status (vital signs, weight, skin condition, appetite and changes in functional ability). Observes participant when they are taking prescribed medications
Reports any concerns or major changes to Home Care Coordinator in a timely manner
Assists participants with Instrumental Activities of Daily Living (IADLs) (grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care) as assigned/designated in the care plan
Documents services provided to participants consistently, accurately and in a timely manner via Electronic Health Record (EHR). Follows procedure for flow sheet completion
Participates in participant's care planning through with interdisciplinary teams
Handles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautions
Displays a willingness and ability to be responsive in a warm and caring manner to all customer groups.
Participates in Quality Assurance and program improvement initiatives as assigned

Qualifications
High School Diploma or equivalent. Must be licensed as a Certified Nursing Assistant (CNA) in the state of Colorado
Minimum of one (1) year of experience working with a frail, elderly or disabled population is required.
Current First Aid/BLS Certification is required
Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers
Must be efficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records
Ability to work independently with minimum supervision
Knowledge of how to use and maintain medical equipment (adaptive, hoyer lift, etc.) is strongly preferred
