Our goal is to balance individual clinical and psychosocial patient/family needs with the efficient and cost-effective utilization of resources while promoting quality outcomes.

Function/Duties of Position
Case Management is represented on every adult and pediatric unit and the Emergency department.
The Case Management central resource office is in Sam Jackson Hall 1226.
Each Case Manager has a workstation near the unit they are assigned to.
Communication factors: Case Managers are available 7 days a week between the hours of 0700- 2300. They are available by telephone, e-mail, and approved messaging applications.
ED RN Case Manager: In addition to the above basic CM functions, also provides the following -
Using approved criteria, conducts a comprehensive patient/family assessment and admission clinical review upon admission to the emergency department to ensure appropriateness of the care setting.
Provide early identification of high-risk patients and their needs.
Perform care coordination and discharge planning for patients in the Emergency department and Observation units.
Assess patients' potential to discharge from the emergency department with community care coordination.
Round with Advance Practice Providers at designated times BID.
Assist the transfer center with coordinating appropriate patient transfers out of the emergency department.
Educate providers regarding documentation requirements that support medical necessity determinations.
Deliver Medicare notifications including but limited to: Condition Code 44 notices, Observation notices, and Medicare Important Messages.
Document according to departmental policy.
Leads the effort to ensure compliance with CMS and other insurance regulations related to Care Management.
Other functions as assigned.
Evening Shift RN Case Manager
Provide nurse case management support to patients, families, and staff in both the inpatient and emergency department settings for the evening shift 7 days per week.
Receives referrals and hand offs from day shift case managers.
Complete hand off of information to day shift case managers.
Trouble shoot and resolve any discharge delays or barriers carrying over into evening shift.
Other functions as assigned.

Required Qualifications
Minimum three (3) years of clinical nursing experience in the last 7 years.

BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date.
ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date.

This distance nursing position is intended to provide a specific regional resource for home infusion patients in the Grants Pass and surrounding OR region and reduce the resource demand on non-regional RNs and staff. This RN will be expected to respond timely, usually within 30 minutes, of being notified of regional patient start-of-care or other existing patient care need while on-duty.

The nurse in this position will provide comprehensive care to home infusion patients in an outpatient setting, including, but not limited to, the administration of antibiotics, immunologics, TPN, chemotherapy, and supportive care. The RN will be required to travel to the patient's home and provide infusion therapy for patients. The nurse will be assigned to coordinate care and ensure the patient and family are receiving OHSU's highest quality of nursing care. This nurse should be available to work Monday through Friday and is expected to be part of the On-Call rotation.
Work assignment includes variable geography (OR/WA) to include two alternate sites of care.
Laptop, phone, vital sign equipment and organizational equipment provided for home infusion use only -to be returned upon departure from work department or extended leave.
FTE RNs to assume a mandatory On Call component; some distance FTEs may be exempt related to distance/potential response times.
Required Qualifications
Previous Experience in ambulatory or infusion.
Completion of Infusion Nursing Society Fundamentals of Infusion therapy (FIT) modules certification during orientation period
2 years direct care experience as an RN.

Current unencumbered Oregon RN and WA licenses (WA within 30 days of hire).

Position summary:
This Oncology Clinical Research RN is responsible for the clinical research activities for cancer clinical research studies conducted at the OHSU Knight Cancer Institute. The RN works collaboratively with other staff and provides services, as assigned by the supervisor, in furtherance of the university's mission and goals. Function/Duties of Position

The practice specializes in providing urologic care for adults. Services include catheterizations, specimen collections, prostate/testicular biopsies, cystoscopies, cystograms, lupograms, retro urethrograms, calypso beacon, gold seed placements, urodynamics, vasectomies, stent exchanges/removals/placements, voiding trials, medication administration (BCG, Lupron, Trimix, Xiaflex, testosterone pellets, Botox), Foley placement, catheter irrigations, and dilations.
Function/Duties of Position
This position requires some onsite presence with occasional travel to the clinic at Adventist Health Portland
A Registered Nurse (RN) working in the Department of Urology is a member of a multi-disciplinary team and has the primary responsibility for and provides direct and indirect professional nursing care for patients. This includes representing the practice both in-person and on the telephone, other OHSU departments, and non-OHSU facilities. The RN assesses patient and family needs, develops a plan of care, implements and evaluates the care delivered.
