Essential Work Functions:

Engage in quality assurance initiatives and professional development activities to remain current with best practices in physical therapy
Collaborate in discharge planning to ensure smooth transitions and continued patient progress in community or home-based settings

Required Essential Skills:

How you'll contribute

Performs an appropriate assessment on each patient and performs reassessments, as per policy. Performs accurate and timely QI scoring. The population is adult and geriatric patients requiring an adult medical rehabilitation program and services.
Able to assess patient pain interfering with optimal level of function of participation in rehabilitation; makes appropriate interventions to reduce pain; makes appropriate physician contact for intervention.
Formulates and updates patient specific plan of care for physical therapy
Reports patient progress and barriers to discharge in each patient team conference
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Formulates a teaching plan based on identified learning needs and evaluates effectiveness of learning; family is included in teaching, as appropriate. Performs appropriate documentation.
Demonstrates knowledge, competency and proficiency of physical therapy modalities.
Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families.
Manages and operates equipment safely and correctly. Maintains department cleanliness and safety.
Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.

The Therapist participates as an integral part of the interdisciplinary team, manages resources to meet the needs of the patient balancing outcomes to provide effective and efficient treatment. Supervises clinically all assistants and aides of patients to which the Therapist is directing or overseeing the delivery of services according to an established plan of care. Competencies:
Provides and directs patient rehabilitation care within all general accepted practice methods, meeting all center, state, federal, and accrediting standards.
Evaluates patient's needs and establishes effective and efficient treatment plans for the patient assigned.

Participates in the screening of all patients according to directed schedules and practices.

May travel or float to other CH communities to provide services.

Assists in the maintenance and cleanliness of the department and equipment.
Provides supervision to assistants and aides/techs.
Completes weekly reports and patient summaries.
Participates in weekly meetings.
Updates and renews patient orders and plan of care as indicated.
Participates in monthly meetings.
Assures complete monthly billing.
Renews patient orders and updates plan of care as indicated.
Participates in quarterly quality improvement initiatives as Directed by DOR.
Participates in quarterly reviews of long-term residents as directed by DOR.
Participates in annual review.
Assists DOR in annual review of other staff as appropriate.
Performs other duties as required.
Qualifications:
Must possess a current, unencumbered license to practice as a licensed PhysicalTherapist in the State of New Jersey.
CPR certification preferred.
Must be able to lift a minimum of 50 pounds without assistance.
Schedule: 8am-4pm, Monday - Friday.
Education:
Graduate of an accredited college or university with a bachelor's degree in physical therapy.
Must possess a current, unencumbered license to practice as a licensed PhysicalTherapist in the State of New Jersey.
CPR certification preferred.
Previous Experience: No previous experience required.

Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
Develop and implement evidence-based treatment plans tailored to the needs of each patient.
Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR).
Monitor patient progress and adjust treatment plans as needed.
Requirements:
Active and unrestricted license to practice as a Licensed Clinical Social Worker in State applying for.
Effective oral and written communication in English and Spanish
Basic proficiency with technology, including electronic health records (EHR).
Strong organizational and documentation skills, with attention to regulatory compliance.
Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
Full Vaccination and Booster Status may be required in some facilities.
Preferred Qualifications:
Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
History of treating Anxiety, Depression and Adjustment Disorders.
