JOB SUMMARY The Educational Behavior Specialist is responsible for providing 1:1 service, groups, consultation, and coaching that support educators, students, and families within an educational setting. This position works independently primarily on school sites along with services provided in homes and a variety of community settings. This position works directly with children and adults. RESPONSIBILITIES and DUTIES Clinical Under supervision of the Educational Clinical Coordinator, provides behavior consultation to teachers, school staff, and caregivers that focuses on MTSS, Positive Behavior Intervention Supports, antecedent strategies, function-based approach to interventions, and a hand off approach to de-escalation along with direct individual and or group services for skill building. Integrate feedback from supervisors to deliver effective quality clinical services that meet fidelity measures of implementation. Record data for students as applicable and IEP goals utilizing electronic data collection systems to support customer outcomes and ensure staff treatment fidelity. Model, coach, reinforce, and implement positive behavioral interventions and skill development plans with students, caregivers, educators, and support network. Assist supervisors with plan development (e.g., behavior guides). Assist teams with developing program materials (e.g., visual schedules, social stories, behavior guides, etc.) to develop individualized, strength-based strategies and interventions that will result in positive behavior change. Actively participate in supervision, team meetings, in-service, student progress meetings, and other key meetings as assigned. Proactively identify potential conflicts; clearly communicate with supervisor on customer-related services (e.g., barriers to implementation, lack of caregiver/educator participation, etc.) within 24 hours. Ensure effective coordination of services for students, caregivers, and educators with other providers, both internal and external, by facilitating the development of nurturing relationships, supporting quality environments, and collaboratively problem-solving barriers. Works independently with students, educators, and or caregivers in a variety of settings including school sites, homes, community, and/or treatment settings. Takes a hand off approach to de-escalation and provides consultation and modeling of positive strategies for educators. Works collaboratively with others on clinical teams to support creation and implementation of stabilization and service plans. Administrative Under supervision of the Educational Clinical Coordinator, timely and effectively meets agency and program timelines for clinical documentation, compliance trainings, assigned project deadlines, submission of timecards, and respond to phone calls, emails, texts, and chats. Creates and follows weekly schedule for assigned sites/caseload. Deliver services on flexible schedule based on customer need. Proactively communicate schedule changes, including part-time hours, vacation, or sick time. Engage in solution-focused communication and help maintain a positive work environment. Implement strong organizational skills to support customer outcomes and manage daily tasks. Stand-by or quick response to crisis. Works independently in families' home and a variety of community and/or treatment settings. This position works directly with children and adults at times and locations that are convenient to them. The Behavior Specialist III position will provide direct service by implementing behavior plans and fostering productive relationships through a range of activities with youth and/or their caregivers. Under supervision the Behavior Specialist III also assists with activities within their scope of practice and training including but not limited to Functional Behavioral Analysis, facilitating Child and Family Team (CFT) meetings, completing Core Evaluation Data Elements (CEDE), and writing of the Individual Child and Family Plans for assigned families. Under the clinical supervision of a Facilitator or Clinical Program Manager, Behavior Specialist IIIs will provide input to Assessment and Treatment plan documents. About our Program: TRUST: Is a Mobile Mental Health Evaluation Team that handles psychiatric emergencies for adults psychiatric or behavioral distress. Program includes a crisis hotline, mobile mental health response and follow up services Schedule: Saturday and Sunday 3pm-11:30pm JOB SUMMARY Works within the vision, mission, and philosophy of the agency. Within a team process, develops a strength-based relationship with client in the context of a mental health or behavioral crisis. Intervenes with individuals presenting with crisis behaviors including but not limited to agitation, aggression, active psychosis, suicidal ideation and/or self-injury. The position requires shift-based work in which individuals are responding to community or Crisis Stabilization Unit based crisis on immediate emergency requests from individuals, community partners, law enforcement or family members/natural supports. The position will require at times joint response with law enforcement officers. Will participate in the crisis assessment and triage including the screening for dangerous situations, weapons, and other high risk environmental factors. Responsible for crisis de-escalation and stabilization in situations that have the potential for becoming volatile in nature. Responsible for linking to community resources. May search, locate, and engage individuals' or youth's natural supports. May assists with transitioning/transporting individuals or youth to the lowest level of care needed. Works independently in the individuals' or youth's home and in a variety of community and/or treatment settings. May require certification in restraint certification. RESPONSIBILITIES AND DUTIES May support mental health crisis hotline by gathering information, supporting callers and identifying needs related to current crisis situation. This position works directly with children and adults. RESPONSIBILITIES AND DUTIES Demonstrates mastery of Family Specialist I job description. Provides direct billable mental health services, documents in alignment with MediCal regulations and achieves set productivity expectations. Coaches and mentors others in the principles and concepts of functional behavior analysis and the development of individualized engagement processes. Teaches others how to utilize strengths of children, families, and others to assist in the implementation and achievement of goals and outcomes. Initiates and leads groups in developing resources within the community to serve children and families. Mentors, coaches, and trains Family Specialists I Coaches others on how to engage caregivers in building youth-specific parenting responses that support positive behavior change and overall family relationships. Documents interactions and practices and maintain administrative expectations in a timely manner and provides documentation coaching to others. Provides leadership in developing program responses to needs identified across a number of youth and families where natural community resources are not available or appropriate. Performs other responsibilities, as assigned, to support department/business needs. JOB SUMMARY The Mental Health Outreach Specialist is responsible for outreach and engagement activities for people who are experiencing homlessness and have mental health disorders. Outreach Specialist will engage individuals living on the streets and will provide case management, housing navigation and benefit establishment services. Outreach Specialist will provide frequent follow-up with individuals served and provide regular communication on outcomes obtained. RESPONSIBILITIES AND DUTIES ESSENTIAL DUTIES AND RESPONSIBILITIES: * Provide strength-based outreach to encourage people who have mental health disorders and are experiencing homelessness to engage in available services including benefits, mental health, medical and substance use disorder services, and housing programs. * Develop rapport and maintain effective and positive working relationships with clients through frequent visits. * Assess client needs and characteristics; identify short-term and long-term needs, provide resources and referrals based on the client needs. * Drive agency-provided or personal vehicles and transport clients to facilitate linkages and placements as necessary. * Attend collaborative meetings with referring parties to discuss outcomes and address barriers encountered. * Document all services per funding and compliance requirements including maintaining log of daily activities. * Develop and maintain collaborative working relationships with various public and private service agencies, community groups, and the faith-based community to obtain services for clients. * Research community resources that benefit clients and enhance services provided. * Process applications or registration forms for services. * Gather and document various types of data to better understand client needs and assess client eligibility for community resources; enter data into the Homeless Management Information System (HMIS) and the Coordinated Entry System (CES). * Attend community meetings, project updates, and case management meetings as needed. * Attend and participate in staff meetings and trainings. * Model Pacific Clinics' approach, mission and core values in all communication and correspondence.
